But if you&#8217;re already living with a memory change, or caring for a parent who is, that same advice isn&#8217;t very helpful.</span></p><p><span>I wanted to create something that works for each stage of aging. That way, everything inside my Substack runs on one simple map, and it will help you navigate whatever stage you or your loved one is in.</span></p><p><strong><span>Direction one: the domains.</span></strong></p><p><span>Aging doesn&#8217;t arrive all at once in a single lump. It shows up in separate areas of your life, and they each move at their own pace. You might have a razor-sharp mind and cranky knees. You might have a strong body and a social world that&#8217;s shrunk.</span></p><p><span>I sort the areas into domains, the areas where aging shows up. The ones I&#8217;ll come back to most are your thinking and memory, your physical function and independence, and your social connection.</span></p><p><span>Every post will tell you which domain we&#8217;re in, so you&#8217;re never guessing (see image below).</span></p><p><strong><span>Direction two: the seasons.</span></strong></p><p><span>This is the part I&#8217;m most excited about. Inside any domain, you&#8217;re always in one of three seasons.</span></p><p><span>The first season is when everything is working and nothing is wrong. Your job here is prevention, the powerful habits that keep this season running as long as it possibly can.</span></p><p><span>The second season is when something starts to shift. A few more memory lapses than feel normal. A flight of stairs that used to be nothing is now tough to climb. This is the season people ignore out of fear, and it happens to be the one where acting early changes the most.</span></p><p><span>The third season is when a change has fully arrived. A dementia diagnosis. A body that needs significant help to get through the day. People assume this season is only about loss. It isn&#8217;t. There is still a great deal you can do to live well inside it, and to make life better for the person you are caring for.</span></p><p><span>Take your brain. Season one is a sharp memory and mind, and the work is prevention, maintenance, and future planning. Season two is mild memory or thinking trouble, and the work is catching it early, slowing it down if possible, and setting up supportive systems. Season three is full-blown cognitive impairment, and the work turns to meeting them where they are, supporting their safety and dignity, and to holding up the family doing the caring. One domain, three completely different sets of instructions.</span></p><p><span>I&#8217;m still working on perfecting the visual, but this is what I have so far.</span></p><div in each domain, in each season, there is something inside your control. Not everything! I will never pretend a season is gentler than it is, or that willpower fixes what biology won&#8217;t. Some parts of aging you simply don&#8217;t get a vote on. There is always a lever, though, and my job is to put the right one in your hand for the square you&#8217;re standing on.</span></p><p><strong><span>One more layer: where you are in life.</span></strong></p><p><span>Someone at 40 reading about brain health needs different marching orders than someone at 75, who needs different ones again than a person caring for a spouse with dementia. So wherever it helps, I&#8217;ll split the advice by stage, and you can go straight to the part that fits your life and skip the rest.</span></p><p><span>So here&#8217;s how to read along. When a new post opens, notice which domain it&#8217;s in, find your season inside that domain, then pin down your stage of life. That&#8217;s your square on the map. Someone in their 90s would come into the hospital and a colleague would shrug. &#8220;Well, they&#8217;re just old.&#8221; As if that settled it. As if we wouldn&#8217;t fight for them the way we&#8217;d fight for someone half their age.</span></p><p><span>I watched the opposite mistake too, older people kept going on machines long past the point it was helping anyone. And underneath both, the small things kept getting missed. The hearing loss nobody thought to check for. The medicine cabinet that had quietly become packed full and was now its own illness.</span></p><p><span>None of it sat right with me. A child isn&#8217;t a small adult, and every pediatrician knows it. An older adult isn&#8217;t a regular adult with extra birthdays, and far too few doctors are trained to see the difference.</span></p><p><span>So I went and got the training. I&#8217;m board certified in internal medicine and in geriatrics, and here&#8217;s a number that surprises people: there are only about 8,000 geriatricians in the whole country. It&#8217;s a specialty most people have never heard of, holding knowledge most doctors were never taught.</span></p><p><span>When you pay attention to the details of an aging body, you can change the entire story. One in three older adults leaves the hospital with a new disability they did not arrive with. A lot of that is preventable with something as unglamorous as getting a person up and walking every day. Small hinges swing big doors. That is the whole premise of my work.</span></p><p><span>Most of you found me through my videos </span><a href="https://www.instagram.com/agewellwithdrsteph/"><span>@AgeWellwithDrSteph</span></a><span>. This is where I get to slow down and go deeper than a 60-second video clip will allow, where I can take what I read in the medical journals and hand it to you in plain language you can use tomorrow.</span></p><p><span>A quick word on what this is not. You&#8217;ve met the longevity influencers, the ones optimizing every biomarker with a cabinet full of supplements and a small fortune to spend. A lot of their science is real. Very little of it is livable for a normal person with a budget and a life.</span></p><p><span>Aging is a beautiful process and it&#8217;s different for everyone. Physical changes, disability or illness over a long, beautiful life are not failures.  They are part of a normal process where you still have the ability to improve, delay or adapt and still live an amazing life.</span></p><p><span>What I want to give you is the realistic version. Clear-eyed about what aging brings, and hopeful, because the research sides with hope more often than most people would guess.</span></p><p><span>That hope rests on one idea I will come back to again and again: you have agency at every age. What you do in your younger decades sets up the ones that follow, and it is never too late to bend the path. People adapt to what aging brings far better than they fear they will. Research shows a hearing aid or a walker costs a lot less happiness than most people brace for.</span></p><p><span>So if you&#8217;re here to age well yourself, or to care for someone you love who is aging, you&#8217;re in the right place. Same goes if you&#8217;re a clinician who wants to do right by older patients. Pull up a chair. Let&#8217;s age well, together.</span></p><p><span>I&#8217;ll leave you with this. Every year I do something called a misogi, a hard physical challenge, just one, meant to remind me what my body can do if I ask it to. This year it was the Escape from Alcatraz Triathlon. I&#8217;m not telling you that to impress you. I&#8217;m telling you because it&#8217;s the same idea I want you to walk away with today: aging is something you get to keep working with, on your terms, for hopefully a lot longer than you
