Regulatory capture describes the capture of regulators by the industries they are supposed to regulate. Epistemic capture goes much further: it describes the capture of the institutions and processes through which knowledge is created, legitimized, and disseminated.

And that is what this series is about: how every major layer through which medical knowledge is created, legitimized, and disseminated has been captured by the pharmaceutical industry.

Why epistemic capture matters

Try questioning a widely accepted medical claim, and you will quickly encounter a familiar pattern of objections.

“But I trust my doctor.”

“Where are the peer-reviewed studies?”

“If that were true, I would have heard about it.”

“If it were really that bad, the government would do something about it.”

Objections like these all rest on the same underlying assumption: that the institutions being invoked are sufficiently independent and trustworthy to serve as arbiters of medical truth.

But if the institutions being invoked have themselves been captured by the pharmaceutical industry, then appealing to them does not settle the question. It simply moves the question upstream.

Epistemic capture does not mean that everything coming from mainstream medicine is necessarily false. It means that institutional consensus should not, by itself, make a claim more credible. Claims should be judged on the quality of the evidence behind them—not simply on who is making them.

The pharmaceutical industry is a multi-trillion-dollar industry with enormous financial incentives to protect its markets. If a medical claim seriously threatens those interests, there is no reason to assume that captured institutions will evaluate it in good faith.

Through lobbying, campaign contributions, regulatory funding, revolving-door employment, advisory committees, and government partnerships, pharmaceutical companies have built a network of influence that extends from lawmakers and federal agencies to global regulatory systems.

Political capture

Pharmaceutical companies don’t just influence the regulators who oversee them—they also influence the politicians who appoint those regulators, fund government agencies, and write the laws governing the industry. The pharmaceutical industry is one of the largest and most powerful lobbying forces in Washington.

Buying political influence

Between 1999 and 2018, the pharmaceutical and health products industry spent approximately $4.7 billion lobbying the U.S. federal government—more than any other industry. But this wasn’t simply health-sector spending; it was driven primarily by pharmaceutical interests. PhRMA, the industry’s largest trade association, was the biggest spender at $422 million, followed by pharmaceutical giants Pfizer, Amgen, and Eli Lilly. The industry also contributed another $1.3 billion in federal and state campaign contributions, bringing its disclosed political spending during that period to more than $6 billion.

This money isn’t spread evenly across Congress—it follows power. Among the 40 largest recipients of pharmaceutical campaign contributions between 1999 and 2018, 39 served on healthcare committees, and 24 held leadership positions on those committees. These are the lawmakers who write the rules governing pharmaceutical regulation and the healthcare system itself.

Political influence becomes public policy

Over the past four decades, Congress has repeatedly reshaped the legal framework of medicine through policies that granted the pharmaceutical industry an extraordinary combination of protections and institutional advantages.

In 1986, Congress created a vaccine injury compensation system that effectively eliminated lawsuits against vaccine manufacturers. In 1992, the Prescription Drug User Fee Act (PDUFA) allowed pharmaceutical companies to directly fund the FDA’s drug-review process. In 2005, Congress passed the PREP Act, creating broad liability protections for pharmaceutical manufacturers during declared public health emergencies. Those protections were invoked in 2020 through the COVID-19 PREP Act Declaration, shielding pharmaceutical companies from liability for the harms caused by their products.

These examples represent an unusually extensive combination of legal protections and institutional advantages—from funding the regulator overseeing them to receiving sweeping liability protections rarely seen in other industries.

Research priorities

Government doesn’t just regulate medicine—it plays a major role in determining what gets studied.

Scientific progress doesn’t occur in a vacuum—it follows funding. The National Institutes of Health (NIH) is the world’s largest funder of medical research, distributing tens of billions of dollars in grants each year. Those funding decisions shape the direction of modern medicine. They determine which diseases receive attention, which questions get investigated, and which medical interventions gain institutional momentum.

But government-funded research doesn’t remain purely academic. The commercialization of publicly funded research accelerated after the 1980 Bayh-Dole Act, which allowed federally funded institutions to patent discoveries and license them to private companies. This created financial incentives tied to the commercial success of pharmaceutical products. A 2020 Government Accountability Office review found that NIH research generated roughly $2 billion in royalty revenue through licensing agreements. Several individual licenses generated more than $100 million each. The NIH doesn’t merely fund medical research—it also receives royalty payments when publicly funded discoveries become commercial pharmaceutical products.

These incentives also extend to individual NIH scientists. A 2005 review found that 916 current and former NIH researchers received royalty payments, averaging nearly $10,000 annually, with some individuals receiving up to $150,000. More recently, a 2022 investigation estimated that NIH scientists received roughly $350 million in royalty payments between 2010 and 2020.

The result is a shift in institutional incentives. Through funding priorities, commercialization policies, and royalty arrangements, government institutions have become financially connected to the success of the pharmaceutical products that emerge from publicly funded research.

Regulatory capture

Regulatory capture occurs when the institutions meant to regulate an industry become structurally aligned with the interests of that industry. The Food and Drug Administration (FDA)—the agency responsible for deciding which drugs reach the market—is the clearest example.

FDA is funded by pharma

A regulator funded primarily by the industry it regulates faces an inherent conflict of incentives.

That funding relationship began in 1992 with the passage of the Prescription Drug User Fee Act (PDUFA), which allowed pharmaceutical companies to fund the FDA’s prescription-drug review process in exchange for negotiated review timelines. What began as a supplemental funding mechanism became the foundation of the program: user fees financed just 7% of the FDA’s drug-review program in 1993, but approximately 77% by 2025. Today, pharmaceutical companies seeking approval for their products fund more than three-quarters of the program responsible for evaluating them.

PDUFA did more than fund the FDA—it created a system in which the FDA repeatedly asks the pharmaceutical industry what it must do to secure its funding. Every five years, the FDA and industry negotiate the user-fee agreement that sets funding levels, performance goals, and review timelines.

The consequences are measurable. A 2008 study of more than 300 FDA drug approvals found that each one-month reduction in review time was associated with an increased count of serious adverse drug reactions, while finding little evidence that the additional user-fee funding improved safety reviews .

When the regulator depends on the industry it regulates for most of its funding—and that industry helps shape the terms of that funding—it becomes structurally aligned with industry interests.

The revolving door

Financial dependence is only one form of regulatory capture. The revolving door between pharmaceutical companies and the agencies that regulate them is another.

Officials routinely move between senior positions in government and the pharmaceutical industry. A 2023 Health Affairs study found that among senior Department of Health and Human Services appointees between 2004 and 2020, 32% left government for industry positions, while only 15% entered government from industry, creating a substantial net flow from public service into the private sector. The imbalance was greatest at the CDC, where 54% of appointees later moved into industry positions, compared with only 8% entering government from industry.

The pattern is particularly pronounced among FDA leadership. All but one FDA commissioner from the early 1980s through 2019 later moved into roles within the pharmaceutical industry. Former FDA Commissioner Dr. Scott Gottlieb joined Pfizer’s board in 2019, less than four months after leaving the agency.

The pattern extends beyond senior leadership. A 2018 Science investigation found that 11 of 16 departing FDA medical reviewers later accepted positions with the very companies whose products they had recently evaluated.

The issue is incentives. When regulators know that the companies they oversee may become their future employers, the regulatory system favors continuity with industry rather than independence from it.

Advisory committees

Many of the FDA’s most important decisions are influenced by advisory committees composed of outside physicians and researchers. Their recommendations frequently shape the agency’s final decisions.

Financial relationships with industry are common among committee members. A 2006 JAMA analysis found that at least one member was financially conflicted in 73% of FDA drug-advisory meetings, while fewer than 1% of conflicted scientists were recused. A 2014 study examining more than 15,000 FDA advisory-committee votes found that members with financial ties to the sponsoring company were significantly more likely to vote in that company’s favor.

Advisory committees are not merely symbolic. A 2023 JAMA Health Forum study found that the FDA’s final decisions aligned with advisory-committee recommendations in approximately 88% of cases.

The issue is not individual misconduct but structural incentives. When regulators depend on industry funding, rely on advisers with industry ties, and share a revolving door with the companies they oversee, pharmaceutical influence becomes embedded within the institutions responsible for regulating it.

Public health recommendations

Government influence over medicine doesn’t end when a drug is approved—it extends to the recommendations that determine how drugs are used in practice.

Unlike the FDA, the Centers for Disease Control and Prevention (CDC) doesn’t decide whether pharmaceutical products reach the market; its influence operates through public health recommendations. Through bodies such as the Advisory Committee on Immunization Practices (ACIP), the CDC develops vaccine schedules that determine the policies and practices of physicians, hospitals, insurers, schools, employers, and public-health agencies throughout the United States. These recommendations effectively become the standard of care.

This makes the CDC one of the most influential institutions in modern medicine. Regulatory approval allows a product to be sold. Government recommendations determine how widely it is used.

Like other government advisory bodies, ACIP relies on outside advisers. Many have conducted industry-funded research, consulted for pharmaceutical companies, or collaborated with industry. A 2025 analysis of 25 years of conflicts among ACIP and FDA vaccine-advisory committee members found that officially reported financial conflicts had declined over time, but the analysis relied on a narrow 12-month reporting window and measured only formally disclosed conflicts. A 2021 BMJ investigation found that independent reviews of committee members’ industry ties identified significant financial relationships that fell outside official disclosure requirements.

The consequences of state capture

When oversight agencies become too closely aligned with the industry they regulate, dissent becomes costly, oversight becomes protection, and protecting the interests of the pharmaceutical industry becomes intertwined with protecting the credibility of the agencies responsible for overseeing it.

Dissent is punished

Agency employees who raise safety concerns or question positions aligned with industry interests face professional consequences rather than institutional support.

A 2006 survey of 997 FDA scientists found widespread concerns about political interference and retaliation. Sixty-one percent said they knew of cases where political appointees had inappropriately intervened in FDA determinations or actions. More than one-third said they didn’t feel they could express safety concerns even within the agency, while 18% reported being asked, for non-scientific reasons, to exclude or alter technical information or conclusions in FDA documents.

These concerns were not limited to the FDA. A 2022 Government Accountability Office review found similar problems across the Department of Health and Human Services, including the FDA, CDC, and NIH. Although employees described observing what they believed to be political interference in scientific work, the agencies reported no formal internal allegations between 2010 and 2021. Employees told investigators they chose not to report their concerns because they feared retaliation.

The consequence isn’t only individual retaliation, but institutional distrust. A 2004 Department of Health and Human Services Inspector General survey found that two-thirds of FDA scientists lacked confidence that the agency adequately monitored the safety of prescription drugs.

In 2009, a group of FDA scientists took the extraordinary step of bypassing agency leadership and writing directly to President Obama, warning that scientific concerns were being suppressed and that political and institutional interests had taken precedence over public safety.

The experience of Dr. David Graham illustrates this dynamic. In 2004, Graham, Associate Director in the FDA’s Office of Drug Safety, testified before Congress that senior FDA officials attempted to intimidate him and discourage him from publicly raising concerns after he concluded that the painkiller Vioxx had contributed to tens of thousands of heart attacks and strokes.

Oversight becomes protection

In a healthy regulatory system, violations should result in consequences severe enough to discourage future misconduct. In a captured system, that relationship is reversed. Oversight becomes less about holding industry accountable and more about preserving institutional stability and managing reputational damage.

Pharmaceutical companies have repeatedly paid billions of dollars in civil and criminal settlements for unlawful marketing, fraud, and regulatory violations. Yet the penalties imposed are often only a fraction of the financial benefits generated by the misconduct. A 2026 JAMA Network Open study examining pharmaceutical anti-kickback settlements found that companies paid a median penalty equal to only 2.2% of the U.S. revenue generated by the drugs involved during the period of violations. Settlements are treated as a cost of doing business rather than a meaningful deterrent.

The pattern is not limited to individual cases. A separate 2020 JAMA analysis of 26 major pharmaceutical companies found that 22 faced financial penalties between 2003 and 2016, totaling $33 billion. The violations were not isolated mistakes: all but one company faced penalties linked to illegal activities spanning four or more years, and GlaxoSmithKline alone faced 27 separate penalties. The persistence of misconduct across major firms shows that these penalties do not function as a genuine deterrent.

The same pattern appears in product safety. An analysis of drugs approved by the FDA between 2001 and 2010 found that roughly one-third later received major safety warnings, required significant regulatory action, or were withdrawn because of previously unrecognized safety problems. A system designed to protect the public shouldn’t repeatedly discover serious safety problems only after widespread exposure has already occurred.

International spillover

The United States is the world’s largest pharmaceutical market and home to many of the world’s largest pharmaceutical companies. As a result, its political, regulatory, and research institutions play a disproportionate role in shaping global standards for how drugs are evaluated, approved, and used.

When industry influence becomes embedded within these institutions, the consequences extend far beyond American borders. The same companies operate across jurisdictions, and the same regulatory frameworks are increasingly adopted across countries.

FDA’s global influence

The FDA is not merely an American regulator. It is a central institution in the global pharmaceutical system, with decisions that influence how medicines are evaluated, approved, and monitored around the world.

A 2026 analysis of 396 novel therapeutics found that 70.7% received their first approval from the FDA, placing the agency at the front of the global regulatory pathway for most new medicines.

Because of this position, many regulatory authorities rely on FDA reviews when evaluating new medicines. Rather than repeating the entire process themselves, smaller regulators often use the FDA’s assessments, clinical data, and regulatory conclusions as the basis for their own decisions. The result is regulatory spillover: decisions made by the FDA often become the foundation for regulatory decisions made elsewhere.

The FDA’s influence extends beyond individual approvals. Its regulatory philosophy also shapes the standards adopted by other regulators, influencing evidence requirements and approval pathways globally.

Global regulatory harmonization

Organizations such as the International Council for Harmonisation (ICH) develop shared standards for pharmaceutical development and regulatory review. These standards are intended to reduce duplication and facilitate international drug approval, but they also mean that regulatory approaches developed within the world’s largest pharmaceutical markets increasingly shape global practice.

Unlike a purely governmental body, ICH brings together regulatory authorities and pharmaceutical industry representatives to develop these standards. Once adopted, they become embedded in national regulatory systems around the world.

The result is regulatory convergence: standards developed in the world’s largest pharmaceutical markets become embedded in regulatory systems globally.

When the institutions that shape global pharmaceutical standards become aligned with the industry they regulate, the same institutional incentives are exported alongside the standards themselves.

Conclusion

Pharmaceutical capture doesn’t end with research, education, or clinical practice. It extends into the institutions responsible for regulating, funding, and governing medicine itself.

The system doesn’t require every politician, regulator, or scientist to act in the interests of industry. It only requires incentives to be structured in ways that make alignment with industry the path of least resistance.

Lobbying shapes legislation. Industry funding shapes regulatory priorities. The revolving door shapes institutional culture. Advisory relationships shape decision-making. International harmonization spreads those standards beyond national borders.

The result is a regulatory system where the institutions meant to protect the public are increasingly dependent on, connected to, and influenced by the industry they oversee.

The pharmaceutical industry doesn’t need to control every decision. It only needs to shape the system in which those decisions are made.

The media is not a neutral pipeline for medical information. It operates within an information ecosystem shaped by financial incentives and institutional relationships that systematically favor pharmaceutical interests—determining which stories receive attention, which voices are amplified, and how evidence is understood.

Ownership and governance ties between media and pharma

Media coverage is shaped not only by journalists and editors, but also by the corporate structures behind the organizations that produce it. Ownership and governance ties between media companies and pharmaceutical companies create incentives that influence what issues receive attention and how they are covered.

Common ownership

The largest U.S. asset managers—namely BlackRock, Vanguard, State Street, and Fidelity—are the largest shareholders of both the largest media companies and the largest pharmaceutical companies—not just in the United States, but around the world.

This common ownership creates a structural alignment of interests. Asset managers do not need to telephone editors or issue instructions to newsrooms. The ownership structure does the work for them.

These asset managers wield enormous influence through corporate governance. Their ownership stakes give them the leverage to shape who sits on corporate boards, how executives are rewarded, and other major company decisions. Public-company executives know that maintaining the confidence of their largest institutional shareholders is essential, while losing that confidence can have serious consequences for both leadership and share price.

The result is a powerful system of institutional discipline. Media executives know who their largest owners are and what those owners expect. They do not need explicit instructions—incentives alone are often sufficient. A media company whose largest shareholders also own billions of dollars in pharmaceutical companies has little incentive to subject that industry to sustained adversarial scrutiny.

Overlapping board members

Beyond common ownership by asset managers, media and pharmaceutical companies have also been connected through overlapping board members—the same individuals can hold leadership positions in both industries. Corporate boards influence executive selection, strategic priorities, and major corporate decisions.

A 2009 analysis examined nine major U.S. media corporations and found that six had directors who also served on the boards of pharmaceutical companies. Including health insurance companies, eight of the nine media corporations had overlapping directors with either the pharmaceutical or health insurance industries—only CBS had no such overlap.

A more recent example is James C. Smith, who served as Thomson Reuters’ president, CEO, and a member of its board of directors from 2012 to 2020. In 2014, while leading Thomson Reuters, Smith joined Pfizer’s board of directors and was appointed to Pfizer’s Corporate Governance Committee and Science and Technology Committee. Thomson Reuters owns Reuters, one of the world’s largest and most influential news organizations, whose reporting reaches audiences worldwide and is widely republished by other media outlets.

Media’s dependence on pharmaceutical advertising revenue

The pharmaceutical industry is one of the largest sources of advertising revenue for U.S. media. Unlike many industries that have shifted heavily toward digital marketing, pharmaceutical companies devote an unusually large share of their advertising budgets to television, print, and radio, making them one of the most important financial supporters of legacy news organizations.

This relationship expanded dramatically after the FDA’s 1997 guidance unlocked large-scale direct-to-consumer prescription drug advertising on television and radio. Over the following two decades, spending on these advertisements grew nearly fivefold, from $1.3 billion in 1997 to $6 billion in 2016.

Commercial media organizations depend on advertising revenue to finance their operations. When an industry is one of their largest sources of revenue, that financial dependence will shape what gets investigated, how issues are framed, and how much scrutiny that industry receives—even without direct interference. The result is a structural incentive to limit coverage that could damage valuable commercial relationships.

This isn’t merely theoretical. A 2016 study analyzing more than 80,000 newspaper articles found that pharmaceutical advertising was associated with significantly less critical coverage. Articles about advertisers’ drugs used fewer negative words, and for every additional $100,000 a newspaper received from a pharmaceutical company, coverage of safety issues for that company’s drugs fell by 23–33%, while mentions of side effects declined by 9–20%. A follow-up 2020 study found that this bias was strongest when companies faced negative news events. Advertising doesn’t merely increase visibility—it provides protection from scrutiny.

These findings capture only the most measurable effects of advertising influence. Studies can quantify changes in coverage, language, and tone, but they cannot easily measure issues that are never investigated or perspectives that never enter mainstream discussion. Financial dependence influences not only how an industry is covered, but also the boundaries of what coverage is considered viable in the first place.

How pharmaceutical PR becomes medical news

Before health stories reach the public, they have often already passed through an extensive public relations pipeline. Pharmaceutical companies invest heavily in PR, and healthcare is the largest specialty sector in the U.S. public relations industry, with healthcare and pharmaceutical clients accounting for roughly one-third of revenue at large PR firms. Press releases, media briefings, broadcast packages, and sponsored content provide pre-packaged narratives that enter news organizations before independent reporting begins.

Press-release journalism

Health reporting often relies on press releases supplied by pharmaceutical companies and PR firms. This practice is known as “churnalism”—the production of news by recycling press releases and other pre-packaged material. A 2006 UK study found that health reporting was the news category most dependent on PR material: 37% of health news stories were based mainly or wholly on PR, compared with 20% of newspaper stories overall. Health news stories frequently reproduced press-release material with limited independent verification or additional reporting.

The problem is not merely that journalists rely on press releases, but that the framing of those releases is often carried directly into news coverage. A 2014 BMJ study examining 462 health-related press releases and 668 resulting news stories found that 33% to 40% of press releases exaggerated findings from the underlying research. When press releases contained exaggerated causal claims, resulting news stories were 20 times more likely to reproduce the same exaggeration. When press releases overstated the relevance of animal research to humans, news stories were 56 times more likely to reproduce the same exaggeration. Much of the distortion in health news was already present in the press releases that shaped subsequent coverage.

The PR pipeline also determines which information gets omitted. A 2021 BMJ Open study found that while 94% of scientific papers disclosed their funding sources, only 29% of press releases and just 9% of resulting news stories did. But when press releases included funding information, news outlets were three to seven times more likely to include it as well—showing that what public relations leaves out often disappears from the news altogether.

Broadcast-ready public relations

Pharmaceutical public relations extends beyond written press releases. Pharmaceutical companies and their PR agencies also produce video news releases (VNRs)—professionally produced television segments designed to appear in news broadcasts with little or no additional reporting.

A 2006 study found that major U.S. television stations broadcast pharmaceutical-sponsored VNRs as news while almost never disclosing their source. The investigation tracked 87 instances of VNR use—only one included any disclosure, and even that identified the PR firm rather than the corporate sponsor. The VNRs tracked reached more than half of the U.S. population, showing that pharmaceutical PR reaches large national audiences rather than remaining confined to isolated outlets.

The problem was not only that stations used pre-packaged material, but that they often transformed it into something that appeared to be independent journalism. In more than 60% of documented cases, stations re-voiced VNRs using their own anchors or reporters, sometimes repeating the original PR script word for word. Some stations also added their own graphics and branding to further obscure the corporate sponsor.

Sponsored content and native advertising

The boundary between journalism and advertising has also blurred through sponsored content, advertorials, and native advertising. Unlike traditional advertisements, these formats are designed to resemble editorial content, adopting the publication’s style, language, and appearance while carrying messages on behalf of corporate sponsors.

Many readers struggle to distinguish native advertising from independent journalism. A 2016 study found that fewer than 8% of participants correctly identified native advertisements as advertising, even when disclosures such as “sponsored content” or “advertisement” were used. When readers were made aware of the commercial nature of native advertising, they became more skeptical of the content—suggesting that its effectiveness depends on being mistaken for independent information.

For pharmaceutical companies, this creates a way to present promotional messages in the language of medical education rather than traditional advertising.

Sources, framing, and narrative control

Pharmaceutical influence extends beyond the production of medical knowledge to how that knowledge is communicated, interpreted, and understood by the public.

Industry-connected commentators

Many health stories rely on physicians and researchers presented as independent authorities, yet these commentators often have financial relationships with the pharmaceutical industry whose products or research they are discussing.

A 2017 CMAJ study of 591 international medical news stories found that 32% of commentators described in coverage as independent had financial conflicts of interest related to the topic being discussed. When those financial conflicts aligned with the findings being reported, 93% offered favorable commentary, compared with just 17% when they did not.

These financial conflicts of interest are often invisible to audiences. A 2000 New England Journal of Medicine analysis of 207 news stories about new drugs found that half of the cited commentators had financial ties to pharmaceutical manufacturers. Yet in most cases, audiences were not told: the relationships were disclosed only 39% of the time. The failure was even more pronounced in television coverage, where none of the 14 reports that cited commentators with industry ties disclosed those financial conflicts of interest.

Patient groups

Patient organizations are frequently used as sources in health reporting, providing journalists with human stories, personal testimony, and public-facing perspectives that shape how medical issues are understood.

Industry funding is widespread among patient organizations. A 2017 New England Journal of Medicine analysis of 104 of the largest U.S. patient advocacy organizations found that 83% received financial support from drug, device, or biotechnology companies, while 39% had a current or former industry executive on their governing board. Transparency was limited: only 57% disclosed donation amounts, only 10% disclosed how donations were used, and only 12% had policies addressing institutional conflicts of interest. A 2023 BMJ Open study further found that pharmaceutical payments to UK patient organizations were overwhelmingly directed toward groups aligned with companies’ existing products and drug-development pipelines.

This creates a form of credibility transfer: pharmaceutical interests can be amplified through organizations portrayed by the media as independent patient advocates, leading audiences to view industry-aligned positions as grassroots perspectives rather than advocacy shaped by corporate funding.

Disease-awareness campaigns

Pharmaceutical influence in the media extends beyond promoting specific products. Disease-awareness campaigns shape public understanding of medical conditions without directly advertising a drug.

These campaigns often frame everyday symptoms and experiences through a medical lens, encouraging people to seek diagnosis and treatment. By shaping perceptions of disease before a product is ever mentioned, they can expand demand for medical interventions. FDA research has found that these communications can blur the line between education and drug promotion, causing consumers to associate disease information with pharmaceutical solutions even when no product is named.

The growth of disease-awareness campaigns has been dramatic. A 2019 analysis published in JAMA found that the number of U.S. disease-awareness campaigns running annually increased more than ninefold, from 44 in 1997 to 401 in 2016. In 2016 alone, pharmaceutical companies spent $430 million on unbranded disease-awareness campaigns.

Ultimately, these campaigns shape how people define illness and decide what warrants treatment.

Selective framing of medical evidence

The selection of sources is only one part of the process. How that information is framed also shapes how the public understands treatments, risks, and evidence.

Health news frequently omits critical context. HealthNewsReview.org, which evaluated 500 U.S. health news stories in a 2008 analysis published in PLOS Medicine, found widespread failures in reporting the information needed for informed decision-making. Between 62% and 77% of stories failed to adequately address harms, benefits, evidence quality, or alternative options. Only 33% adequately covered potential harms, while only 28% adequately quantified benefits.

When benefits are reported, they are frequently presented in ways that exaggerate their apparent importance. News coverage often emphasizes relative risk reductions without providing sufficient context about absolute risk reductions. This distinction matters because relative risk can make treatments appear far more effective than their actual impact. A treatment that reduces a risk from 2% to 1%, for example, can be described as cutting risk by 50%—a technically accurate statement that can nevertheless create a very misleading impression of the magnitude of the benefit.

Harms are also frequently filtered out. A study of health news stories featured on Google Health News found that 25% omitted adverse events from media coverage even though those same harms were explicitly reported in the underlying papers. More broadly, 88% of news stories contained at least one form of “spin,” including exaggerated claims, unsupported interpretations, or conclusions that went beyond the evidence.

The result is a pattern in which medical news consistently amplifies benefits while minimizing uncertainty and harms.

Digital expansion of pharmaceutical marketing

Pharmaceutical influence is no longer confined to newspapers and television. As audiences have migrated online, pharmaceutical marketing has followed, expanding into search engines, social media platforms, and other digital channels.

Healthcare and pharmaceutical digital advertising is projected to reach approximately $26.2 billion in 2026, compared with roughly $6.9 billion for traditional advertising channels. By 2027, digital formats are projected to account for approximately 82% of healthcare advertising spending. This matters because digital platforms increasingly determine how people discover and consume health information.

Digital platforms

Digital platforms are not passive distributors of health information—they determine visibility. Search engines rank results, social media platforms recommend content, and algorithms decide which information receives attention—and which remains effectively invisible. In traditional media, editors and publishers controlled distribution; in digital environments, that function is performed by ranking systems, recommendation algorithms, and platform policies.

Google’s 2018 “Medical Update” illustrates how these systems can reshape access to health information. A study across ten European countries found that health and medical websites experienced substantial declines in search visibility after the update, with median visibility scores falling by approximately 48% within a year. Ranking systems do not merely determine what people see—they can shape what people believe. A 2014 study showed that manipulating the proportion and order of vaccine-related search results shifted people’s vaccination attitudes, with greater exposure to anti-vaccination content increasing skepticism toward vaccines.

Social media health influencers

Pharmaceutical companies increasingly market through patient influencers, physician influencers, and other health content creators, making commercial messages appear as personal experience, education, or independent medical advice rather than advertising.

A 2023 study described patient influencers as a form of interactive direct-to-consumer advertising, embedding pharmaceutical promotion within personal stories and disease narratives. Physician influencers represent another extension of this model. A 2024 JAMA analysis found that 61% of physicians’ medical endorsements on X (formerly Twitter) were sponsored testimonials, while nearly half failed to disclose industry compensation.

International spillover

Although the United States has one of the most permissive environments for pharmaceutical promotion, pharmaceutical influence over health media is not uniquely American. Countries that restrict direct-to-consumer drug advertising still face many of the same underlying dynamics, and global news organizations, wire services, and digital platforms allow narratives shaped in major pharmaceutical markets to spread internationally. Pharmaceutical influence therefore travels across media systems even where direct-to-consumer advertising is restricted. As Dr. Ike Iheanacho, editor of the UK-based Drug and Therapeutics Bulletin, has noted, these interactions between the pharmaceutical industry and the media are “universal and perennial.”

Conclusion

Pharmaceutical influence over media does not require controlling every journalist, newsroom, or story. It operates through structures that shape incentives, visibility, and credibility.

Ownership ties, advertising dependence, PR pipelines, industry-linked voices, patient organizations, and digital platforms all influence which medical narratives reach the public and how they are understood.

The result is a media environment that functions less as an independent watchdog of pharmaceutical power and more as part of the system that normalizes it.

Clinical practice guidelines are the documents that tell physicians what to do. They define the standard of care. They determine what doctors prescribe, what insurers cover, and what courts consider acceptable medical practice—making them arguably the single most consequential documents in medicine.

And the organizations that write those guidelines are largely funded by and populated by the industries whose products those guidelines evaluate.

The pharmaceutical industry dictates research. Research dictates guidelines. Guidelines dictate the standard of care. Doctors who depart from that standard risk professional and legal consequences—meaning pharma ultimately dictates not only medical knowledge, but medical practice itself.

Clinical practice guidelines

A clinical practice guideline is a document that tells physicians how to diagnose, treat, and manage disease. For condition X, start with treatment A. If that fails, move to treatment B. Avoid treatment C.

Who writes them

The vast majority of clinical guidelines are written by private specialty societies, including the American Heart Association, the American College of Cardiology, the American Diabetes Association, the American Psychiatric Association, and others.

These are private membership organizations—not government agencies. They have no statutory authority, yet their recommendations effectively govern medical practice.

Why they matter

Guidelines do far more than summarize medical evidence. They define the standard of care and shape insurance reimbursement, hospital protocols, electronic medical record prompts, physician training, and malpractice litigation.

Writing a clinical guideline is effectively writing a prescription for millions of people.

Technically, they are recommendations. In practice, they function as the rules of modern medicine.

Global reach

American clinical guidelines influence medical practice far beyond the United States. Many countries rely on or adapt recommendations produced by major American medical societies. Decisions made by a relatively small number of guideline panels influence healthcare systems around the world.

If those guideline processes are captured, that capture is exported.

The commercial prize

For a pharmaceutical company, influencing clinical guidelines is one of the highest-return investments it can make.

A single guideline recommendation can make a drug the default treatment across entire healthcare systems. Once embedded in clinical guidelines, it no longer has to be sold to individual doctors—it becomes default practice, reinforced by insurance coverage, institutional protocols, and legal standards of care.

The explosion of clinical guidelines

Clinical practice guidelines didn’t always occupy the central role they do today. Before their widespread adoption in the late twentieth century, physicians relied primarily on clinical judgment, training, textbooks, and local practice.

As healthcare became increasingly standardized, guidelines evolved from reference documents into the infrastructure of medical practice. Today, thousands of clinical guidelines exist across medical specialties, with hundreds of new or updated guidelines published every year.

Guidelines derive their power not from law alone, but from the systems built around them.

In malpractice litigation, they are routinely used as a proxy for the standard of care, increasing legal risk for physicians who depart from them. Hospitals and insurers reinforce the same recommendations through reimbursement policies, clinical protocols, and electronic medical record systems.

As a result, clinical decision-making is increasingly shaped by institutional systems rather than individual judgment. Guidelines no longer simply summarize medical evidence—they define the practical boundaries of acceptable medical practice.

Capture of clinical guidelines

Both the organizations that produce clinical guidelines and the people who write them are captured by the pharmaceutical industry.

The guideline bodies

The organizations that write clinical guidelines are funded by the companies whose drugs those guidelines recommend.

A 2016 PLOS Medicine study examined financial relationships between organizations that produce clinical guidelines and the medical industry. The majority had financial relationships with industry. Most had conflict-of-interest policies on paper, but few had procedures for enforcing them. The policies existed—the enforcement didn’t. Only 1% of guidelines disclosed the producing organization’s own financial relationships.

A 2020 BMJ study found that 72% of leaders of major U.S. medical associations had financial ties to industry—rising to 80% among physicians. Across the top ten disease areas, total payments to leaders approached $130 million over five years. At the top end, median individual payments exceeded $500,000 for leaders at the American Society of Clinical Oncology. Not one organization had a leadership free of industry ties.

This is not just an American phenomenon. A 2023 study of executive board members of Japanese internal medicine societies found that 99% had received personal payments from pharmaceutical companies over five years. Total payments exceeded $70 million, with a median five-year combined payment per board member of $150,849.

The American Heart Association receives approximately $40 million annually from pharmaceutical and device companies. The American Diabetes Association receives tens of millions in industry funding. The American Psychiatric Association is heavily funded by antidepressant and antipsychotic manufacturers. These same organizations write the cardiovascular, diabetes, and psychiatry guidelines that define the standard of care.

The panel members

The organizations that produce clinical guidelines do not write them directly. They appoint committee members to decide what the guidelines will recommend. Those committees are no less intertwined with industry than the organizations themselves.

A 2023 BMJ Open study comparing self-reported disclosures to the Open Payments database found that, over a three-year period, 74% of U.S. physician authors of clinical guidelines received industry payments, but only 37% disclosed any conflict. The median payment was over $27,000. The top 25% of authors received more than $254,000. In total, these authors received $98.7 million, including $24 million that was not disclosed.

Guideline chairs were especially compromised: 82% had financial conflicts of interest, and not a single one disclosed them accurately.

A broader 2021 Mayo Clinic Proceedings review of 37 studies covering 14,764 guideline authors found that 45% had at least one financial conflict of interest, with rates reaching 100% in some specialties. Thirty-two percent had undisclosed industry payments.

A 2002 JAMA study found that among 100 respondents representing 37 clinical practice guidelines, 87% reported interactions with pharmaceutical companies, 58% had received research funding, and 38% had served as employees or consultants. Authors had ties to 10.5 different companies on average. Fifty-nine percent had relationships with companies whose drugs were directly considered in the guidelines they were writing, and 96% of those relationships predated the guideline process. Only 2 of 44 guidelines disclosed any of these relationships.

Disclosure as theater

Disclosure doesn’t eliminate bias. It normalizes it. When financial conflicts are universal, disclosure turns them into the baseline rather than an exception.

Disclosure also functions as “moral licensing”—giving physicians psychological permission to be more influenced by industry relationships, not less, because they feel transparency has absolved them of the conflict.

Corruption of clinical guidelines

Corruption in clinical guidelines does not just come from who sits on the panels—it’s embedded in how evidence is selected, interpreted, and turned into recommendations.

How recommendations become distorted

Guideline recommendations are only as good as the evidence they’re built on. That evidence base is not neutral. It has been shaped at every stage by the industry that stands to benefit from the recommendations.

Many guideline recommendations are based on changes in laboratory measurements rather than whether patients actually live longer or feel better. A drug can receive a strong recommendation simply for improving a test result.

The GRADE system—the main framework used to judge the quality of evidence in clinical guidelines—is supposed to distinguish strong evidence from weak. In practice, studies that look rigorous on paper can still receive high ratings despite being funded by industry, relying on indirect measures of health, or ending before long-term harms can be measured.

Roughly half of guideline recommendations are based on opinion from guideline authors rather than high-quality evidence. That opinion becomes the guideline. The guideline becomes the standard of care.

The drug bias

Guidelines systematically favor pharmaceutical interventions over non-pharmaceutical approaches. The reason is structural: drugs have companies behind them to fund trials and influence panels—lifestyle interventions don’t.

This creates a built-in imbalance in the evidence base. Drugs are extensively studied and heavily promoted, whereas diet, exercise, and behavioral interventions aren’t. Guideline panels are dominated by specialists whose careers are built around pharmacological treatments.

The result is a system that predictably produces drug-based recommendations over non-drug alternatives.

Industry’s effect on recommendations

There is a consistent association between financial conflicts of interest and more favorable recommendations for a sponsor’s drug or device.

A 2020 BMJ review found that conflicted guidelines were 26% more likely to make favorable recommendations for a sponsor’s drug or device—and this is likely a substantial underestimate. Many of the included studies relied on only disclosed conflicts of interest, which underreport industry relationships. Guideline recommendations are also built on clinical trials and reviews that are themselves influenced by industry funding. A separate Cochrane review found that industry-funded trials were 34% more likely to report favorable conclusions. The result is a layered system of bias: industry-funded trials feed into systematic reviews, which feed into guidelines. The measured effect therefore represents only the absolute floor of industry influence.

Threshold manipulation: creating new patients

Guideline panels don’t just recommend treatments—they define who is sick. By shifting diagnostic thresholds for disease, they create millions of new patients.

Pre-diabetes. Pre-hypertension. A person can move from “healthy” to “at risk” simply because a guideline committee changes a diagnostic cutoff. Someone whose blood pressure was “normal” yesterday can become “pre-hypertensive” today because the threshold moved.

Each new threshold expands the population eligible for treatment. Pre-diabetes creates a market for blood sugar-lowering drugs. Pre-hypertension creates a market for blood pressure medications.

The ratchet effect of guidelines

Guideline recommendations almost always move in one direction: expansion.

Treatment thresholds are lowered, new disease categories are introduced, and screening recommendations extend to broader populations. Once these changes are made, reversals are exceptionally rare.

Doctors are less likely to follow guidelines for themselves

If clinical guidelines reflected optimal medical practice, you would expect doctors to adhere to them most closely for themselves—but the opposite is true. Doctors are less likely to adhere to clinical guidelines than the general population.

A nationwide study of the Swedish population examined adherence to 63 prescription drug guidelines across the entire population, comparing the general public with physicians and their close family members. Across all guidelines, physicians and their families followed recommendations just over 50% of the time—less than the general population.

Conclusion

Pharmaceutical companies fund the organizations that write clinical guidelines. Those guidelines become the standard of care. Doctors who depart from them risk being sued.

Clinical guidelines are the point where upstream corruption becomes downstream harm—captured research becomes captured clinical practice. The result is a medical system where the rules are written by those who profit from them.

The guidelines aren’t the guardrails. They’re the cage.

The most important documents in medicine are the ones most patients will never see and most doctors will never question.

You don’t need to control the doctors if you control the rules they’re forced to follow.

This doesn’t mean that every doctor is corrupt, but it does mean that the institutions responsible for training doctors are overwhelmingly influenced by the pharmaceutical industry—and that influence shapes what doctors are taught, what information they are exposed to, and what treatments they are encouraged to use.

Medical school

Medical school is subject to pharmaceutical industry influence at every level. It begins with the accreditation bodies that decide what medical schools are required to teach, and extends to the administrators who set institutional priorities, the faculty who deliver the curriculum, the content that is included or excluded, and even the informal culture that shapes what students absorb outside formal instruction.

This capture wasn’t inevitable. Before it could happen, medical education had to be centralized.

That centralization largely began in 1910 with the Flexner Report. Funded by the Carnegie Foundation and supported by the Rockefeller Institute, it led to a sweeping reform of American medical education. Medical schools that did not conform to a single standardized model of medicine were closed or forced to adapt. Within two decades, roughly half of all medical schools in the United States had shut down.

When medical education was spread across hundreds of competing schools with competing approaches to medicine, no single set of institutions could define what doctors learned. Once the system was narrowed, influencing a small number of institutions was enough to shape it.

Accreditation

The Liaison Committee on Medical Education (LCME) determines what every MD-granting school must teach. Without LCME accreditation, students cannot access federal loans, sit for licensing exams, or enter residency. The LCME is jointly sponsored by the Association of American Medical Colleges (AAMC) and American Medical Association (AMA). The AMA receives tens of millions of dollars from pharmaceutical advertising, grants, and partnerships—and generates hundreds of millions more through its control of the billing codes and physician databases that pharmaceutical companies rely on to reach prescribing doctors. The AAMC represents medical schools whose department chairs have extensive industry ties. LCME committee members are drawn from these institutions.

Leadership & faculty

A 2007 JAMA study found 60% of academic department chairs had financial relationships with industry. Department chairs set curriculum, control hiring, and shape institutional culture—and they also teach. Of the hundreds of thousands of physicians in the U.S. who receive industry payments annually, academic leaders and department chairs are consistently among the highest-paid recipients.

These relationships extend beyond individual faculty. Pharmaceutical companies also fund medical schools through grants, donations, sponsored programs, research partnerships, and endowed chairs.

Curriculum

The preclinical curriculum is heavily weighted toward pharmacology, while nutrition receives roughly 20 hours of teaching across four years of medical school.

Students memorize large volumes of drugs and their side effects, while largely neglecting the skills needed to evaluate the evidence behind what they are taught. A 2007 JAMA study found that medical residents often struggle with basic statistical concepts—skills that would allow them to identify when a clinical trial is designed to make a drug look better than it actually is. Industry-funded trials routinely use techniques that inflate apparent benefits, such as measuring a lab value instead of whether patients actually live longer or feel better, or running trials too short to detect long-term harms—techniques many medical students are never trained to recognize.

A 2018 AJOB Empirical Bioethics study found widespread undisclosed industry conflicts among textbook authors across multiple specialties, including many who also served as advisors or speakers for pharmaceutical companies. A 2021 study in Community Mental Health Journal found that most editors and authors of major psychopharmacology textbooks had received industry payments, totaling over $11 million among a small group of contributors. Even core reference materials used in training come from pharmaceutical companies with direct commercial interests, such as the Merck Manual, widely used in medical education and published by Merck, which also manufactures many of the drugs it describes.

The goal for medical students is to learn what will be tested—regardless of whether it’s true. The exams required to become a licensed physician place enormous emphasis on drug knowledge, treating it as a core measure of medical competence. The committees that write these licensing exams are governed by physicians drawn from the same academic institutions whose department chairs and faculty have extensive industry relationships.

Hidden curriculum

Beyond what is formally taught in the classroom, students also learn from the culture and practices of the institutions around them. The hidden curriculum is the unwritten signals that tell students what is rewarded, what is tolerated, and what is expected. It works through role models, evaluations, career opportunities, and everyday institutional practices rather than explicit instruction. A 2005 JAMA study found that the average medical student is exposed to pharmaceutical industry marketing once every week. Many of the physicians teaching them have documented industry ties. Students learn by watching those in authority positions, and what they observe becomes what they interpret as standard care. Students who question prescribing norms face poorer evaluations and damaged career prospects.

Residency

Residency is where prescribing habits are formed for life. After four years of medical school, new doctors enter three to seven years of supervised training in their chosen specialty. The pharmaceutical industry is embedded throughout this training.

Accreditation

The Accreditation Council for Graduate Medical Education (ACGME) sets the standards for all residency programs. The ACGME is governed by representatives from the AMA, AAMC, and the American Hospital Association—organizations with extensive industry ties. The curriculum residents must complete is shaped by requirements set by these organizations.

Specialty societies

Between the accreditation bodies and the training programs sit the specialty societies—the American College of Cardiology, the American Psychiatric Association, the American Diabetes Association, and others. These organizations create educational materials, host annual conferences, publish journals, and influence the curricula that residency programs follow—and they’re heavily shaped by industry funding. The American Heart Association receives $40 million annually from pharmaceutical and device companies. The American Diabetes Association receives tens of millions. The educational content that reaches residents—conference presentations, training modules, board review materials—is produced by organizations structurally shaped by funding from the companies whose products are being taught.

Industry influence in residency training

Residency training sessions, teaching rounds, and discussion meetings are often sponsored by pharmaceutical companies. Food, materials, and speakers are frequently provided by the company whose product is being discussed. Pharmaceutical representatives are allowed into many training programs, where they provide free samples, meals, and educational materials.

Free samples of prescription drugs influence prescribing behavior. A 2017 BMJ Open review found that accepting free samples is associated with residents prescribing more expensive brand-name drugs instead of cheaper, equally effective alternatives.

Even where direct access by sales representatives is restricted, the influence continues through educational funding and sponsored programs. A 2000 JAMA review found that physician-industry interactions are linked to more frequent prescribing, higher costs, and lower-quality prescribing decisions. A 2005 Journal of General Internal Medicine review found the same pattern specifically among residents: exposure to pharmaceutical representatives was associated with increased prescribing of the promoted drugs.

The mechanism is not large payments. A 2016 JAMA Internal Medicine study analyzing federal payment data and prescription records found that a single industry-sponsored meal costing less than $20 was associated with higher rates of prescribing the promoted brand-name drug. The industry does not need to buy doctors outright. Lunch is sufficient.

How training shapes prescribing

Residents practice under the supervision of senior physicians in clinical training environments. Many of these physicians have industry ties. A 2013 BMJ study found that physicians trained at institutions with stricter limits on pharmaceutical marketing prescribed fewer newly marketed and heavily promoted drugs years later in independent practice. A 2020 Health Affairs study found that restricting pharmaceutical marketing in medical school was associated with lower opioid prescribing. When exposure to pharmaceutical marketing and industry influence during training is reduced, future prescribing behavior changes in predictable ways. The habits formed during training are not temporary. They persist. The drugs residents learn to prescribe become the drugs they prescribe for decades.

Continuing medical education (CME)

After residency, after the decade-plus journey from pre-med to practicing physician, the indoctrination continues—for their entire career. Continuing medical education is mandatory for doctors to keep their medical license. Fail to accumulate enough credits, and you cannot practice. And it's the pharmaceutical industry that has historically funded and continues to exert substantial influence over the majority of this mandatory education.

Industry funding

At its peak in 2006, direct commercial support accounted for over 60% of all CME funding in the United States. While subsequent regulatory changes reduced the percentage of direct funding by pharma, the industry adapted by rerouting its money into advertising, exhibition fees, and targeted sponsorships of the clinical instruction hours that medical schools depend on to stay solvent.

Medical education companies, which derive 80% of their revenue from pharmaceutical companies, also determine which courses receive CME credit, adding another layer of industry-shaped gatekeeping between funding and accreditation.

The illusion of independence

The Accreditation Council for Continuing Medical Education (ACCME) rules state that industry funders cannot control content. In practice, CME providers depend on industry funding. They know which topics and which speakers attract funding and which do not. They do not need to be told what to avoid. The incentive structure does the work automatically. Speakers know who pays them. If a speaker develops a reputation for discussing drug harms too candidly or presenting non-pharmacologic alternatives too enthusiastically, the speaking invitations stop.

A former pharmaceutical sales representative with nearly two decades of experience put it bluntly:

“Key opinion leaders were salespeople for us, and we would routinely measure the return on our investment by tracking prescriptions before and after their presentations. If that speaker didn’t make the impact the company was looking for, then you wouldn’t invite them back.”

15–25% of pharmaceutical marketing budgets are physician speaking fees—“education” in name, marketing in reality. No explicit instructions are needed—the incentives are sufficient.

Speakers paid by industry

Many of the same physicians who serve as CME faculty receive speaker fees from the companies whose products they teach about. The payments are disclosed through the Open Payments database, but disclosure does not eliminate the incentive. Instead, disclosure functions as “moral licensing”—giving physicians psychological permission to be more influenced by industry relationships, not less, because they feel transparency has absolved them of the conflict.

A 2020 Annals of Internal Medicine study found that industry payments to physicians were associated with increased prescribing of promoted drugs. A 1988 Journal of Continuing Education in the Health Professions study found the same pattern for CME: industry-funded continuing education was associated with increased prescribing of the sponsor’s drug.

The opioid crisis was a CME operation

Purdue Pharma financed more than 20,000 pain-related medical education programs. The Federation of State Medical Boards (FSMB)—the body responsible for disciplining physicians—published a book called Responsible Opioid Prescribing, widely distributed as continuing medical education for opioid prescribers. Opioid manufacturers largely wrote it, then paid the FSMB over $250,000 to distribute 163,000 copies to physicians. A 2024 BMJ investigation reviewing roughly 900 contracts from opioid manufacturer Mallinckrodt found a coordinated effort to shape medical attitudes through recruiting physicians as influencers, organizing conference presentations, and developing CME courses.

The education doctors relied on to maintain their licenses was marketing material for a product that helped drive an epidemic that has killed hundreds of thousands of people in the United States.

Why they can’t escape

The pharmaceutical industry’s influence over medical education is structural capture. Even doctors who see through it are unable to practice differently.

Accreditation

The LCME controls who gets to operate a medical school. The ACGME controls who gets to operate residency programs. Without accreditation, graduates cannot get licensed, enter residency, or practice medicine. Accreditation standards enforce conformity to the prevailing model of medical education. There is no pathway for a genuinely independent medical school that takes a more cautious approach to pharmaceutical use to enter the system. The gatekeepers are drawn from the same institutions they oversee and answer to the same funding structures. The monopoly that Flexner established in 1910 is still in place, enforced through accreditation. Competing medical paradigms can’t gain a real foothold.

Professional fallout

The hidden curriculum does not end at graduation. The physician who questions prescribing norms faces professional isolation. The physician who practices outside the guidelines faces malpractice liability. This is where the capture of specialty societies becomes a legal trap. The same organizations structurally shaped by industry funding—the American Heart Association, the American Diabetes Association, and others—write the clinical practice guidelines that impact the legal standard of care in a malpractice courtroom. A physician who attempts to reverse chronic disease through nutrition rather than prescribing the guideline-mandated drug regimen is legally exposed. The system punishes deviation at every level—economically, legally, and socially. The cost of non-compliance is designed to be prohibitive.

Debt

The average medical school graduate carries over $200,000 in educational debt. Many carry significantly more. This debt burden does not just trap physicians after graduation—it shapes what kind of physician they become. Higher debt pushes graduates toward higher-paying specialties. The most lucrative specialties are also the most procedure-heavy and prescription-heavy. Debt steers doctors toward the specialties that generate the most pharmaceutical revenue before they ever write a prescription. After training, the debt burden traps them in high-volume, guideline-concordant practice. A doctor with six-figure debt can’t afford to open a low-overhead cash practice focused on lifestyle medicine. The debt also creates psychological lock-in. Someone who sacrificed their twenties and took on crushing debt to enter the profession is not easily going to admit that much of what they learned was industry propaganda. The sunk cost is too high.

Institutional barriers to reform in medical education

A better system would require ending the accreditation monopoly created by the Flexner Report—an arrangement in which a small set of gatekeepers determines what every doctor must learn in order to practice medicine. These gatekeepers are drawn from institutions funded by the same industries whose products physicians are trained to prescribe. Before 1910, competing medical schools meant competing medical paradigms. No single institution could dominate the profession because no single institution controlled it.

Conclusion

Every stage of the medical education pipeline is captured by pharmaceutical industry interests.

The system does not need to conspire—it simply needs to keep incentives structured so that everyone inside it does what the incentives reward. The student who keeps their head down gets the residency. The resident who prescribes like their supervising doctor gets the fellowship. The senior doctor who stays on-message gets the speaking fees. The CME speaker who doesn’t go off-script gets invited back.

This is not education designed to produce the best health outcomes for patients—it’s education designed to produce physicians who practice medicine in ways that maximize revenue for the medical system. Given that healthcare spending now accounts for roughly one-fifth of the U.S. economy, it’s functioning exactly as designed.

To be clear, the COVID protocols discussed here are separate from the COVID vaccine that became widely available in 2021. I am referring to the treatment protocols—the testing, the drugs, and the mechanical ventilation of people’s lungs—that defined the 2020 hospital experience.

This article examines the financial incentives the medical system had to kill patients and blame those deaths on “COVID.” What follows is an analysis of the money, the hospital protocols, and the death toll—regardless of one’s view on COVID itself.

Hospitals awarded $100,000+ for killing you

When a person with flu-like symptoms was advised to go home, rest, and hydrate, reimbursement was limited to a minimal consultation fee. But if that same person was admitted under a COVID diagnosis, administered remdesivir, and placed on a ventilator, the hospital could charge well over $100,000—and sometimes more than $400,000.

The COVID hospital protocols functioned as a highly lucrative federal payment pipeline, where the treatments that generated the most revenue were the ones most likely to kill you.

I. Testing (up to $1,419 per COVID test)

A positive COVID test result was the triggering event that unlocked the federal COVID payment pipeline—allowing hospitals to transition patients into an extremely lucrative clinical pathway. While Medicare paid hospitals at lower rates for tests, the private market was effectively a free-for-all. Under the CARES Act, providers would post a cash price for testing, which insurers were required to pay for out-of-network claims. Labs charged as much as $1,419 per test, and hospitals repeatedly tested patients to create a recurring revenue stream. By treating testing as both a standalone profit center and the necessary key to unlock subsequent, high-value inpatient billing codes, hospitals used COVID testing as a key driver of revenue.

II. Admission (20% premium for COVID diagnosis)

A positive test result funneled patients into the hospital under ICD-10 code U07.1—the universal designation for COVID. The federal government paid a 20% premium on top of the standard Medicare payment for COVID-related hospital stays. Because this premium was tied exclusively to the COVID diagnosis, hospitals had a huge incentive to prioritize the COVID designation over other conditions and pursue repeat testing until a positive result was achieved.

III. Remdesivir (65% premium for wrecking your kidneys and liver)

Hospital protocols prioritized the administration of remdesivir, a highly toxic drug costing $3,200 per treatment course. CMS incentivized this choice through the New COVID-19 Treatments Add-On Payment (NCTAP), which provided hospitals another massive bonus payment—calculated as 65% of the costs exceeding the standard billing rate.

Remdesivir—dubbed “Run, Death Is Near” by the doctors and nurses who watched patients deteriorate after receiving it—is a highly toxic drug that causes organ failure. It was a failed Ebola drug that was rebranded as a treatment for COVID, despite the fact that it was associated with a staggering 53% mortality rate in its original Ebola trial.

In a randomized, double-blind, placebo-controlled trial of remdesivir published in The Lancet, patients received either remdesivir or a saline placebo. Remdesivir is so toxic that patients in the remdesivir group were forced to stop treatment early due to adverse events at 2.4 times the rate of those receiving the saline placebo.

The drug’s toxicity is not a secret. It’s documented in the FDA’s own prescribing information, which mandates monitoring for liver damage because the drug is known to lead to liver failure. And it’s not just the liver. In a study published in Frontiers in Pharmacology, patients receiving remdesivir were nearly four times more likely to suffer acute kidney injury than those receiving other treatments. And once the kidneys failed, the fatality rate for those patients was over 36%.

The World Health Organization found that remdesivir provided no survival benefit and recommended against its use—yet American hospitals continued administering it for years.

IV. Mechanical ventilation ($50,000+ for destroying your lungs)

After wrecking a patient’s kidneys and liver with remdesivir, the next stage of the COVID protocol was the most lucrative and deadly of all: mechanical ventilation.

The average cost to treat a non-ventilated COVID patient was $12,700, but by placing the patient on a ventilator, the average cost skyrocketed to over $65,500.

Mechanical ventilation was a death sentence. Despite the well-known risks of ventilator-induced lung injury, hospitals ignored decades of best practices—which favored non-invasive oxygen support—and pivoted to early, aggressive intubation. Patients were sedated, paralyzed, and kept in a chemically induced coma while ventilators forced high-pressure air into their lungs.

The results were catastrophic. In many hospitals, the death rate for patients placed on ventilators was above 80%. In New York, the death rate for people over the age of 65 was 97%.

V. Death ($100,000+ altogether for killing you)

After wrecking your kidneys and liver with remdesivir, then finishing you off with a ventilator, the hospital could collect well over six figures for killing you—and in some cases hospitals charged more than $400,000. But the death certificate said “COVID.”

This is another example of how the medical system launders the deaths it causes—with misleading ICD codes and death certificates that erase the hospital’s culpability. It was a self-reinforcing feedback loop: hospital-caused deaths inflated the official COVID death count, which was then used to justify the very protocols that caused those deaths.

The financial incentives didn’t require the people working at hospitals to consciously think “let’s kill this patient for more money”—but they handsomely rewarded the sequence of interventions most likely to kill you. The protocol that maximized reimbursement was also the protocol that maximized mortality.

At least half a million killed

About a half million people in the United States were killed by COVID treatment protocols in hospitals—but similar protocols were adopted around the world, making the global death toll much higher.

According to an analysis of all-cause mortality data by and colleagues, there were 653,463 excess deaths in the United States during the first 50 weeks of the COVID period, before the COVID vaccine was widely available. This figure represents deaths above the pre-COVID baseline during the period when COVID hospital protocols—including remdesivir and mechanical ventilation—were in effect. While Rancourt attributes these excess deaths to the broader government and medical response, the hospital protocols were the most direct means by which many of these deaths occurred.

The spike in excess deaths during the pre-vaccine COVID period was synchronous with the protocol rollout, and the jurisdictions hit hardest were those that applied the protocols most aggressively, while places with different approaches were largely spared. This same pattern of synchronous, response-induced mortality hotspots was observed around the world, not just in the United States.

John Beaudoin also estimates that about a half million people in the United States were killed by COVID treatment protocols in hospitals. He obtained 1.6 million death certificates through FOIA requests and reviewed thousands of them line by line, including doctors’ notes and cause-of-death narratives. Beaudoin compared what certifying physicians wrote with what the CDC’s parser generated and found massive discrepancies.

Beaudoin documented large increases in deaths from kidney failure and respiratory failure in 2020—conditions consistent with the harm caused by remdesivir and mechanical ventilation. These deaths then declined as hospital protocols became less aggressive and ventilators were used less indiscriminately.

If you search for the “leading causes of death” in the United States, you will be directed to CDC data listing heart disease and cancer at the top—healthcare-related deaths aren’t even included on the list. But those statistics come from death certificates and ICD codes (International Classification of Diseases) that are structurally incapable of recording the massive number of deaths caused by the medical system.

How the medical system launders the deaths it causes

The CDC’s mortality statistics run on ICD codes that don’t properly track the medical system’s own failures. When a doctor or a pharmaceutical drug kills a patient, the cause is recorded as a biological endpoint—such as cardiac arrest or organ failure—and grouped under broader categories like heart disease or cancer, effectively erasing the medical system’s culpability in the death.

The scale of this erasure is massive. A 2000 study in The American Journal of Medicine compared two federal databases and found a 34-fold difference: the FDA’s own reporting system captured many more adverse drug reactions than were recorded on official death certificates—and it is those dramatically lower death certificate numbers that feed the CDC’s mortality rankings.

It’s a self-reinforcing feedback loop. The deaths the medical system causes inflate the very disease categories used to justify the treatments that caused them.

When you move past the official death certificates and into the audits conducted by the system’s own researchers, you find that healthcare is the leading cause of death.

Half a million deaths per year: the bare minimum

The medical system kills at least half a million people in the United States every year—a figure derived from the system’s own data that represents only the absolute floor of a death toll that is, in reality, much larger. This figure is an extremely conservative synthesis of data published in the world’s most prestigious medical journals—including JAMA, The BMJ, and research from institutions like Johns Hopkins.

This death toll is compiled from distinct, non-overlapping categories of harm—ranging from medical errors to adverse reactions from properly prescribed medication. The researchers behind these papers emphasize that these are conservative estimates, and that the true scale of mortality is much higher.

I. Inpatient medical errors (>250,000 deaths per year)

At least 250,000 patients in the United States die in the hospital setting every year due to medical errors. These include errors of commission (harmful action), errors of omission (failing to provide necessary care), communication breakdowns (ineffective or missing information sharing), contextual errors (disregarding the patient’s life circumstances), and diagnostic errors (missed, delayed, or incorrect diagnoses).

The benchmark for this 250,000+ figure is systemic mortality models, namely a 2016 Johns Hopkins study published in The BMJ, which calculated a rate of 251,454 deaths per year due to medical errors by meta-analyzing decades of patient safety research. This estimate is corroborated by a 2013 analysis in the Journal of Patient Safety, which identified objective clinical triggers—such as unplanned returns to the operating room or sudden cardiac arrest in low-risk patients.

Critics will cite lowballed estimates for deaths due to medical errors, such as a 1999 Institute of Medicine (IOM) report (44,000–98,000 deaths) or a 2020 Yale meta-analysis (~22,000 deaths). These estimates are derived from physician reviews and an overly narrow threshold for establishing causation. This physician-review model is prone to institutional bias and systematically underestimates deaths. Doctors naturally want to protect their own profession, so reviewers typically default to blaming a patient’s illness for a death, even when a colleague’s error was the catalyst. By measuring only the most obvious, singular acts of negligence rather than the preventable failure of the entire hospital system, the physician-review model significantly underestimates the true death toll of inpatient care.

II. Inpatient medication reactions (>125,000 deaths per year)

Over 125,000 patients in the United States die in the hospital setting every year due to adverse reactions from pharmaceutical drugs that were properly prescribed and followed. This category is distinct from medical errors; these are not procedural mistakes, but deaths caused by the inherent toxicity and unpredictable pharmacology of drugs deemed “safe” for clinical use.

The foundation for this death toll was first established by a 1998 JAMA meta-analysis (106,000 deaths) and further substantiated in 2013 by a paper in the Journal of Law, Medicine & Ethics, which calculated a baseline of 128,000 inpatient deaths per year. The researchers behind the 128,000 figure emphasized that this represents a significant undercount, noting that structural issues—like the FDA’s prioritization of rapid drug approval over long-term safety monitoring—leave tens of thousands of additional deaths unaccounted for.

This death toll is compounded by the modern explosion of polypharmacy, where the standard of care increasingly involves prescribing multiple medications to manage the side effects of other medications. With over a quarter of adults in the U.S. now taking four or more prescription drugs daily, we have created a medical environment where the risk of fatal drug interactions has climbed dramatically—yet remains invisible to mortality monitoring systems.

III. Outpatient deaths (>100,000 deaths per year)

The medical system’s least-measured—and likely largest—category of fatalities occurs outside hospital walls. These deaths, at home or in nursing facilities, escape all oversight: no charts are reviewed, no root cause analyses are triggered, and no questions are asked about the role of medical intervention.

No single paper cleanly isolates the outpatient death toll, but two assessments using the same cost-of-illness model, published decades apart, converge on the same order of magnitude. The original 1995 study estimated approximately 199,000 annual drug-related deaths in the outpatient setting—a figure later cited in a 2000 JAMA analysis of the U.S. health system. A 2018 update subsequently estimated a total of 275,689 deaths from non-optimized drug therapy in both inpatient and outpatient settings combined. Subtract the roughly 128,000 inpatient adverse drug reaction deaths documented in the previous section, and the residual—approximately 148,000 deaths per year—represents the drug-related death toll occurring outside the hospital setting.

Both studies were authored by advocates for expanding clinical pharmacy services. These are not critiques of the pharmaceutical model; they are arguments for more medication management. Yet, by their own math, the outpatient death toll still comes out in the six figures.

Many more deaths beyond the admitted numbers

The half-million-plus deaths tallied above come from the medical system’s own published research—and even those authors admit their estimates are low.

The NIH’s own StatPearls review concedes that only about 1% of serious adverse drug reactions are ever reported to federal regulators. A Harvard study likewise found that less than 0.3% of adverse drug events get reported.

But beyond these acknowledged gaps are entire categories of death that mainstream research will not touch at all—because acknowledging them would implicate core profit centers. These deaths are triple-laundered: excluded from clinical surveillance, miscoded on death certificates, and kept off the research agenda that determines what the public is allowed to know.

When people realize this, the question inevitably arises:

“Okay, but how can I actually buy honest meat?”

The good news is that there are still a handful of genuine workarounds that let farmers and consumers trade directly—so you can buy clean, local meat outside the chemical baptism animals receive in the USDA‑controlled slaughterhouses described in my earlier piece.

Why bother?

All the flashy labels at the grocery store—organic, grass‑fed, humane, free‑range—address how animals are raised, not how they’re slaughtered. No matter the label, every carcass still moves through the same industrial kill lines controlled or standardized by the same handful of processing giants.

Inside that system, supermarket meat almost always comes from animals raised in distant feedlots, trucked hundreds or thousands of miles to centralized slaughterhouses, and treated with harsh chemical rinses. Animals are pushed through crowded lines at breakneck speed, stress hormones flooding their muscles, carcasses dipped in acid or chlorine, dozens of bodies spinning together in communal chill tanks.

By contrast, when you buy through any of these three legal loopholes, you’re buying meat raised and processed outside that pipeline—typically cleaner, more flavorful, and with longer shelf life.

1. Custom exempt slaughter

This is the main way to bypass the USDA’s corporate bottleneck.

Under the Federal Meat Inspection Act, every retail carcass must be killed and processed in a USDA‑inspected plant—with one exception: if you already own the animal before it’s slaughtered.

You and a few friends can buy a share of a cow, hog, lamb, or other livestock. The farmer can process it on-site or deliver it to a small, locally licensed butcher who handles it exclusively for the owners. No minimum share is required—you can buy an eighth of a cow or the whole animal.

No federal inspector hovers over the carcass. No chlorine sprays, acid baths, or harsh chemicals like those mandated in USDA‑approved plants.

Labels read “Not for Sale.” You’re not buying retail meat—you’re reclaiming your own property.

Custom‑exempt slaughter delivers both quality and savings. A share of beef is often cheaper and superior in flavor and freshness compared with grocery-store “grass-fed” meat.

If freezer space is tight, split a share with friends or neighbors. Many small farms coordinate multi-household purchases, so each buyer gets a modest box of wrapped cuts instead of a full side of beef. Some farms run meat CSAs (Community Supported Agriculture) on the same principle—members pre-buy shares and receive smaller monthly boxes instead of a large quantity of meat all at once.

2. Poultry exemptions

This is the poultry‑specific slaughter exemption from the USDA’s corporate bottleneck.

Poultry—chickens, turkeys, ducks, and other birds—are governed by a different law: the Poultry Products Inspection Act (1957). That law carved out two exemptions allowing small farmers to process birds outside USDA‑approved slaughterhouses.

• 1,000‑bird exemption: Ideal for small homesteads—up to 1,000 birds a year for direct sale to consumers, without USDA inspection.

• 20,000‑bird exemption: For larger farms with basic sanitation facilities—up to 20,000 birds per year for sale within the same state, without a USDA inspector on‑site.

Farmers using these exemptions can pluck, chill, and package by hand—no acid baths, ammonia tanks, or factory crowding.

Prices often match or even beat supermarket “organic” chicken, yet you’re buying a bird that actually lived outdoors, ate bugs and forage, and was never given the antibiotics and vaccines routinely used in industrial poultry.

3. Private membership associations

Private Membership Associations (PMAs) aren’t a new slaughter exemption—they’re a legal distribution solution.

They rely on the same custom‑exempt processing described above but address the larger issue of scale: creating a framework that allows meat to reach many households, not just those who hold a share of an animal, while remaining legally outside the USDA retail system.

Some regenerative farms operate as PMAs—private food clubs built on contract law and the constitutional right to association. Members sign an agreement acknowledging that all exchanges are private transactions between members, not retail sales to the public.

Within that framework, beef, pork, raw milk, and other farm foods move freely among participants without USDA oversight. Most PMAs use custom‑exempt processors but organize drop‑sites, CSAs, or intrastate deliveries to make access simple. Think of it as a farm‑to‑table co‑op with built-in legal shielding.

For people in small apartments or who travel often, PMAs are the easiest doorway into clean, local meat: compact monthly boxes, shipped within your state, arriving straight from the farm network.

Everything else is window dressing

State‑inspected slaughterhouses operate under the same rulebook, the same chemical rinses, and the same bureaucratic procedures as the USDA‑approved corporate plants described in my previous article—just on a smaller scale.

Organic, Grass‑Fed, “Humanely Raised,” Free‑Range—and similar labels—are marketing programs layered on top of the same industrial pipeline. Auditors certify how animals live—feed, space, outdoor access—but not how they’re slaughtered. Every carcass still passes through USDA‑approved plants with the same mandated chemical baths and high‑speed lines. Better feed and happier barns don’t change who controls the kill floor.

If your meat doesn’t come through one of the three exemptions above, it remains inside the federal system.

The bigger picture

Every purchase through these exemptions chips away at the meat monopoly. Every custom share, every on‑farm bird, every delivery through a private membership club moves ownership a step away from the corporate bottleneck and back to the local farmer and eater.

When you buy this way, you’re not just choosing better meat—you’re choosing a different food economy, one built on transparency instead of bureaucracy.

And that shift is beginning to show up in law. A handful of states—Maine, North Dakota, Utah, and Wyoming—have already passed Food Freedom laws allowing direct sale of milk, eggs, and farm‑made foods inside state lines. Meat still sits behind the federal wall, but these initiatives show where things are heading.

Skip the labels. Skip the middlemen. Find a farmer, claim a share, join a co‑op, support state food‑freedom laws—and help put honest meat back on the table.

Most of the animals that become American meat spend their lives on a pharmaceutical drip—pumped with antibiotics, vaccines, and artificial hormones. They’re raised shoulder-to-shoulder in industrial warehouses, politely called “Concentrated Animal Feeding Operations” (CAFOs), and fed a federally subsidized diet of corn, soy, and chemical additives their biology was never meant to digest.

Given a choice, many Americans would prefer meat from animals that lived outdoors, ate natural forage, breathed clean air, and weren’t loaded with petrochemical drugs.

But that choice hardly exists. The same cartel of corporations that profits from industrial meat also uses federal law to crush its alternatives. In a fair market, clean, local, chemical-free farming would thrive—but our corrupt food system makes it nearly impossible, and mostly illegal.

Today, you’re free to buy drug-laden meat from a handful of mega corporations, shipped from halfway across the country. You’re not free to buy clean, high-quality meat from your neighbor down the road.

The illusion of choice

Four companies dominate America’s meat supply.

Walk into a supermarket: hundreds of cuts, dozens of brands, every color of packaging—virtually all of it traces back to Tyson, JBS, Cargill, or National Beef.

In 1980, the top four meatpackers controlled 36% of the U.S. beef supply. Today, roughly 85% of the U.S. beef supply passes through the hands of the big four.

In the late 1970s there were more than 600 beef slaughterhouses in the United States. Today there are roughly 150.

What used to be a web of local slaughterhouses and regional distributors has been replaced by a narrow pipeline stretching from feedlot to freezer.

Farmers may still own their land and livestock, but their independence is constrained by a system dominated by a handful of processors that control slaughter capacity, shape prices, and set the terms of the market. Across the industry, animals are raised on industrial feed and routinely treated with pharmaceutical inputs such as antibiotics, vaccines, and artificial hormones.

The illusion of consumer benefit

The industrialization of meat production in the mid-20th century originally lowered costs: animals grew faster, required less feed, and meat became cheaper for everyone. Early efficiency gains seemed like a win-win.

But that didn’t last. As the industry consolidated, the gains stopped flowing to farmers or shoppers. Profits for U.S. cattle producers fell significantly over time, while retail beef prices rose—turning early efficiencies into corporate rent extraction, leaving consumers to pay more for industrialized meat.

The absurdity of our current situation

Picture a small ranch: grass under hooves, sunlight overhead, plenty of room to graze and forage, no antibiotics, no hormones, no vaccines, no chemical-laden feed. Just healthy animals living happily and humanely, as they were designed to live. The rancher films every step, posts daily test results online, invites customers to see the operation with their own eyes. Transparency elevated to art.

Now imagine that same rancher being prohibited from selling you a single steak.

That’s our reality.

How we got here

It started with good intentions. In 1906, appalled by the filth exposed in The Jungle, Congress passed the Federal Meat Inspection Act to keep diseased meat out of the national supply. Inspectors verified that slaughterhouses were clean and animals were healthy. Compliance for small plants was simple: running water, soap, sharp knives, and the occasional visit from a local official. Meat was mostly local, served by thousands of small slaughterhouses.

Half a century later, the Poultry Products Inspection Act (1957) applied similar oversight to chickens and turkeys. Again, the rules were modest, local, and outcome-based: no visible filth, spoilage, or disease.

As the nation industrialized, rules once defined as “clean” became tools of corporate gatekeeping. Small operations were forced to install costly USDA-approved equipment and maintain continuous federal inspection. Compliance could cost millions—trivial for national giants, impossible for local butchers. HACCP in 1996 and FSMA in 2011 piled layers of complex bureaucratic hoops—testing, monitoring, and endless paperwork—that small farms could never realistically manage, while giants absorbed the costs effortlessly. A dozen cattle a day could no longer compete with a plant killing thousands, and only the largest players survived.

In 1977, the four largest beef packers controlled only about 25 % of cattle slaughter; today, they process roughly 85 % of all U.S. beef. That shift from a competitive industry to an oligopoly is the bottleneck locking out smaller operators and concentrating power in just a handful of corporations.

What began as a public safety law had become structural gatekeeping. Compliance shifted from biological reality to bureaucratic affordability. The bigger the plant, the cheaper each rule per pound. Food safety, once a civic duty, has become a corporate franchise—locking out small, honest farmers and leaving a handful of giants to dominate American meat.

Where clean meat goes to die

Imagine a small rancher who somehow beats the odds: his animals are happy and healthy, grazing on real grass, never given any pharmaceutical drugs, and he manages to secure a slot at a USDA-approved slaughterhouse.

Then the system takes over.

The trip to the plant can stretch hundreds of miles because most local processors are gone. Calm, pasture-raised cattle are crammed into trailers, driven for hours at a time without water, and unloaded into concrete pens reeking of ammonia and fear.

Inside, scale rules everything. Small farmers deliver on the same schedule as industrial feedlot trucks. Animals are tagged, queued, and slaughtered on a kill floor designed to process ten thousand animals a day. Compliance leaves no room for separation—every carcass merges into the same stream.

After slaughter, the meat is typically doused with harsh chemical rinses—chlorine, lactic acid, and peracetic acid—that impose industrial uniformity on every animal, turning carefully raised pasture beef into commodity protein. Even the cleanest, most humanely raised animal is chemically baptized.

The system doesn’t regulate contamination—it institutionalizes it.

By the time the rancher pays for processing, inspectors, packaging, labeling, and compliance paperwork, his small batch has cost him thousands—erasing his profit and undermining his independence. Compliance has undone the very reason he farmed clean in the first place.

The market is rigged

At the center of the system sits a simple legal choke point: in America, you cannot legally sell meat unless the animal was slaughtered in a USDA-inspected facility. Building one costs millions, and the few that exist are dominated by the same corporations that control the meat industry—creating a bottleneck that is scarce, prohibitively expensive, and effectively closed to small farmers.

Regulatory overhead compounds the problem. HACCP rules and the 2011 Food Safety Modernization Act demand the same paperwork, testing, and hazard mapping from a five‑acre farm as a plant producing hundreds of tons a day. These “safety” requirements don’t protect consumers—they protect incumbents by keeping competitors out.

Federal subsidies tilt the scales further. Corn and soy—the main ingredients in the chemically loaded, industrial feed that sustains factory farming—are heavily subsidized at the taxpayer’s expense. Grass-fed and regenerative farms receive next to nothing. Pasture-raised meat becomes artificially expensive, while corn-finished meat dominates the market.

Liability frameworks, insurance policies, and biosecurity mandates often force small farms to vaccinate and medicate animals—even when no law explicitly requires it—creating de facto standardization across the industry.

The result: the market is rigged. Even the most conscientious, transparent, humane farmer must submit to corporate plants and bureaucratic hoops—or vanish entirely.

This is corruption, not consumer protection

Defenders of the status quo may appeal to calls for safety.

“You can’t just let anyone sell meat—it goes into people’s bodies!”

Safety’s important, but there’s a world of difference between protecting consumers from fraud and protecting corporations from competition. Anti-fraud enforcement punishes deception after the fact. What we have now isn’t enforcement—it’s licensing. The USDA regime doesn’t verify truth; it grants permission. “Safety” has become the language used to criminalize independence.

If consumers like the USDA label, fine—they can buy inside that system. But others should have the right to purchase from producers without being blocked by corporate choke points or onerous regulatory hurdles.

Consider the alternatives America already tolerates:

• Dietary supplements, including vitamins, minerals, and herbal products.

• Cottage-food products, including baked goods, jams, preserves, and fermented foods.

• Craft beverages, including kombucha, cider, mead, and more.

All of these go into people’s bodies, yet none require the onerous USDA inspection, regulatory hoops, or expensive compliance that have cartelized the meat industry.

A sane regulatory approach would focus on truth, not permission: punishing fraud rather than locking small producers out with rules and costs they can’t possibly meet.

Americans don’t need corporate babysitters—they need honesty, transparency, and the ability to make informed decisions.

What we have now is corruption masquerading as safety.

If the system was fair, prices would fall and quality would rise

If honest farmers could legally process and sell their own meat, “premium” wouldn’t be a luxury label. Clean, grass-fed, toxin-free meat could become the default.

Most of the price consumers pay has nothing to do with raising animals. It comes from corporate slaughterhouses charging hundreds of dollars per head, long-distance cold storage and trucking, supermarket markups, and compliance with onerous regulations. Allow farmers to process on-site or in small regional co-ops, and many of those costs and middlemen vanish. Meat travels ten miles instead of a thousand. Consumer prices would fall significantly, and the farmer would keep a much larger share of revenue.

A liberated market would deliver better quality, lower prices, higher farmer income, and regional resilience. In that world, clean beef wouldn’t be a boutique product—it would be ordinary food.

Conclusion

America lost food freedom not because people stopped wanting clean meat, but because early‑twentieth‑century laws and many subsequent layers of costly regulation made voluntary exchange of such meat effectively illegal.

The USDA inspection system now functions as a de‑facto cartel licensing scheme. Small, honest farmers cannot legally sell meat outside this rigged system without going through cumbersome loopholes.

Many might like to think of the United States as the “land of the free”—but how free is a country where buying meat without federal and corporate intermediaries is a crime?

If a farmer raises animals on clean pasture, feeds them real food, and treats them humanely—and you want that meat—the moment money changes hands, the farmer is breaking the law, even if you know exactly what you’re buying.

These are not natural market forces—it’s a system of corporate-captured regulations intentionally designed to destroy small farmers, enrich mega corporations, and saddle consumers with overpriced, lower-quality meat.

Note: Some legal loopholes exist—CSA programs, custom-exempt slaughter, state-inspected facilities, and poultry exemptions for small producers. They don’t overturn the rigged system, but they do offer a path to clean, local meat. “If that were true, my doctor would tell me.”

“If it were a real problem, we’d see it on the news.”

These responses aren’t really about the drug or guideline being discussed. They reflect something much deeper: trust in the system.

Trust in the system

Most people assume modern medicine functions as a largely self-correcting enterprise—independent scientists generate evidence, neutral regulators evaluate it, professional bodies translate it into guidelines, and doctors pass on those conclusions to patients in good faith. If something were seriously wrong, the system would detect and correct it.

That assumption is a major reason why simply citing studies rarely changes anyone’s mind.

Debates devolve into citation tennis—he said, she said—and few people shift their position. Not because people cannot understand evidence, but because the real disagreement is rarely about the information. It is about the system that produced the information.

There is value in scrutinizing individual studies—their methods, assumptions, limitations. But the more illuminating question is often upstream:

How did we get here?

What incentives determine which narratives dominate medical education, clinical guidelines, and media coverage?

Incentives shape outcomes

Outcomes in large systems tend to reflect incentives more than intentions.

Modern medicine is not just a body of knowledge. It is an ecosystem: pharmaceutical companies, research institutions, regulatory agencies, academic journals, professional societies, insurers, media outlets, and practicing clinicians.

When you recognize that incentives shape behavior—even when intentions are good—the conversation changes.

Instead of asking, “Why would my doctor lie to me?” you begin asking, “What information environment is my doctor operating inside? Who shaped that environment? What incentives shaped which evidence was highlighted, which risks were emphasized, and which uncertainties were minimized?”

Medical consensus emerges from institutions shaped by incentives—and those incentives often do not align with patient health.

The aim of this Substack

My goal with this Substack is not to present stacks of papers and expect you to simply accept a contrarian conclusion.

What I believe is more persuasive is understanding the architecture of modern medicine—the funding structures, regulatory dynamics, professional hierarchies, media incentives, and legal frameworks that shape what becomes accepted as settled knowledge.

But statins are an insidious scam. They poison your body, degrade your mind, and cause a cascade of health problems while delivering zero real benefit. The narrative we’ve been told about fat and cholesterol? Entirely false. Cholesterol is essential, not harmful. And the doctors prescribing these drugs? They’re either clueless or corrupt, acting as hired guns for the pharmaceutical industry. Following a statin prescription from a doctor is like following financial advice from a loan shark. The statin-cholesterol paradigm is a gigantic, diabolical fraud.

Behind the curtain is one of the most profitable enterprises in human history: a vertically integrated system built on eternal “risk management.” In practice, that means: lie to you, trick you into harmful treatments, create new medical problems, sell more interventions, and drain your bank account—until you die early, mind and body degraded.

From blood tests and “preventative” pills to low-fat snack aisles and seed oils, fear of cholesterol drives a multi-trillion-dollar industry. Do you think it will ever tell the truth if doing so threatens the cash cow?

How we got here

The food industry and medical system have formed a deadly alliance, profiting off your suffering while poisoning you. The cholesterol myth and statin scam are just a couple of chapters in a long history of fraud.

After World War II, ultra-processed foods surged in popularity. For the first time in history at such a massive scale, people started eating highly processed foods instead of whole, minimally processed ones. This was no accident. Processed foods are much more profitable. They turn cheap commodities into branded, shelf-stable, high-markup products with extended shelf life and massive scale advantages.

As heart disease emerged as an epidemic in the mid-20th century, the food industry needed a scapegoat. They couldn’t point the finger at the ultra-processed junk they were making. So, they turned to cholesterol. Enter Ancel Keys.

In the late 1950s, Ancel Keys conducted the Seven Countries Study, which became one of the most influential pieces of propaganda in the cholesterol myth. His research selectively cherry-picked countries that supported his hypothesis that fat was the cause of heart disease. Meanwhile, he ignored countries that didn’t fit the narrative, and ignored competing hypotheses, including sugar.

By the late 1940s, the American Heart Association (AHA)’s rise coincided with funding linked to Procter & Gamble—a company whose major products included the hydrogenated vegetable oil Crisco. Soon after, the AHA promoted dietary advice favoring vegetable oils and discouraging saturated fat, adopting Keys’ theory while other factors—like ultra-processed foods—were swept under the rug. The result was a widespread belief that dietary fat—especially saturated fat—raised cholesterol levels, which were assumed to cause heart disease.

Fast forward to the 1970s-80s: government dietary guidelines were solidified, pushing a low-fat, high-carb agenda. The result? A massive rise in obesity, diabetes, and cardiovascular disease.

Then in 1987, statins hit the market as prescription drugs, and the marketing blitz began. Cholesterol was cemented as the ultimate dietary villain in the public mind, and by continuously lowering the “recommended” levels of cholesterol, statin prescriptions skyrocketed. The statin market was born. And it continues to grow, with billions in revenue year after year.

Through all of this, voices warning against this system were ignored, drowned out, or outright censored by powerful commercial interests. The Atkins Diet, which went against Keys’ low-fat, high-carb dogma, was widely demonized in the media. Those who spoke out against the cholesterol myth were silenced, and the narrative became medical doctrine.

Some basic cholesterol facts

Despite what you’ve heard in the media, cholesterol is not harmful—it’s essential. Your body manufactures it deliberately. About 75% of the cholesterol in your body is made by your own system.

Cholesterol levels fluctuate based on many factors—they vary seasonally, typically being lower in the summer and higher in the winter. Stress, diet, exercise, and even the weather can all influence cholesterol levels, meaning it’s not a simple, static figure.

Cholesterol is crucial for brain function. While your brain makes up only around 2% of your body weight, it contains about 25% of your total cholesterol.

Do you really want to take a drug that actively reduces the cholesterol in your brain? Is it any wonder statin users experience cognitive issues—aptly referred to as “statin brain”?

The cholesterol myth

For over fifty years, we’ve been conditioned to fear cholesterol—despite it being a vital substance our bodies literally can’t live without.

The original theory, popularized in the 1950s, blamed cholesterol as a whole. The claim was that eating saturated fat—primarily from foods like red meat, butter, eggs, and full-fat dairy—raised blood cholesterol, which in turn caused heart disease.

When this story failed to hold up under scrutiny, the goalposts were moved. Total cholesterol quietly faded from focus. Cholesterol was split into two different types: “good” (HDL) and “bad” (LDL), and LDL cholesterol alone was rebranded as the villain. From that point on, medical intervention targeted LDL cholesterol specifically—pushed ever lower with each new set of guidelines.

We’re told to imagine LDL cholesterol as greasy sludge clogging the pipes of our arteries, but human arteries aren’t kitchen drains, and cholesterol doesn’t behave like sludge. Fats in the body are transported in liquid form, carried by lipoproteins—more like courier trucks than clots blocking flow. And autopsy data show that most heart attacks occur at sites where blood flow was perfectly normal before the event.

There is no shortage of evidence undermining the broader cholesterol hypothesis—total cholesterol, ratios, triglycerides, and more. But today, the statin narrative hinges on the claim that high levels of LDL cholesterol cause heart disease.

If high LDL cholesterol were truly the cause of heart disease, patients with the highest LDL cholesterol would suffer the most events. They don’t.

What follows isn’t an exhaustive critique of cholesterol theory—it’s a focused debunking of the LDL cholesterol justification for statin use.

Clinical observations

• Hospital data: Up to 70% of patients admitted for heart attacks had “normal” LDL cholesterol levels.

• Large registry analysis (late 2000s): Among nearly 140,000 hospitalized heart‑attack patients, the average LDL cholesterol was lower than in the general population.

Population data

• Framingham Heart Study (begun 1948) and a BMJ Open review (2016): Older adults with higher LDL cholesterol had no increased risk of death, and in some analyses, even lower risk.

• The Nurses’ Health Study (begun 1976): Tracking over 120,000 women for decades, researchers found that five lifestyle factors—smoking, inactivity, obesity, poor diet quality, and heavy drinking—explained almost all coronary events, none of which had anything to do with lowering LDL cholesterol.

• Nomura cohort study (2002–2014): Older adults with higher LDL cholesterol had a significantly lower risk of death from any cause compared with those with lower LDL levels.

Global comparisons

• Across countries, higher average LDL cholesterol may coincide with lower rates of death from heart disease. Japan, France, and Iceland are notable examples: high LDL cholesterol, low heart disease. Conversely, some nations with lower average LDL—often those with more aggressive cholesterol-lowering policies—do not necessarily have lower rates of death from heart disease.

Dietary intervention trials

• Minnesota Coronary Experiment (1968–1973): Participants who replaced saturated-fat-rich foods—such as butter, meat, and whole milk—with vegetable oils and margarine lowered their LDL cholesterol, yet heart-disease deaths did not fall. Those whose cholesterol dropped the most were actually more likely to die. The findings were suppressed for over a decade.

• Lyon Diet Heart Study (1980s): Participants who followed a Mediterranean-style, whole-food diet—including olive oil, fish, and nuts—saw heart disease deaths drop by roughly 75%, even though LDL cholesterol barely changed. The results were so dramatic the study was ended early for ethical reasons: continuing the low-fat, cholesterol-lowering diet was considered too harmful.

No real benefits from statins

The previous section showed that high LDL cholesterol does not cause heart disease. Statins are designed to lower your LDL cholesterol, but do they actually prevent heart attacks or extend your life? (Spoiler alert: no.)

Even if we take the industry’s own optimistic (and deceptive) numbers at face value, the benefits are virtually non-existent.

Researchers consistently found that lowering LDL cholesterol had no significant effect on total mortality. In some trials, more people actually died in the statin group.

• People without heart disease: Fewer than 1 in 100 users avoid a non-fatal heart attack or stroke over five years, with no meaningful extension of life expectancy (on average, only a few days over several years of statin use).

• People with existing heart disease: Taking a statin for five years prevents a non-fatal heart attack in roughly 1 in 40 patients and has a 1-in-83 chance of delaying death.

• Older adults (≥75 years): Roughly 1 in 400 patients without existing heart disease taking statins over several years will avoid a first non‑fatal heart attack or stroke.

• Average life extension: Statins postponed death by only a few days on average (about 3–4 days over 5 years of use).

Even these tiny numbers are massively inflated by the pharmaceutical industry through selective reporting and deceptive, industry-funded research. The reality: hundreds medicate, one might benefit, and the supposed benefit isn’t even due to the cholesterol-lowering properties of statins. Any improvement likely comes from mild anti-inflammatory or anti-clotting effects—benefits that can be achieved much more effectively and safely with diet, exercise, and lifestyle.

The minuscule (and illusory) benefits might be forgivable if statins weren’t extremely harmful, which they are.

The harm caused by statins

Statins don’t just fail to extend your life—they destroy your quality of life.

With most medicines, negative side effects show up pretty quickly—for example, nausea, vomiting, rash, headache, or dizziness. Statins are different. Their side effects creep in gradually—months or years after starting. By then, the link is obscured, and patients are often told they’ve developed a new condition requiring another prescription.

Below are some of the side effects statins cause. Most are even acknowledged on the official package inserts, though severity and prevalence of these harms are drastically downplayed.

• Muscle pain and weakness: officially labeled as “rare,” yet it’s the #1 reason people quit statins, with most stopping within two years of starting. Real-world surveys show it affects over 20% of users.

• Persistent fatigue and low energy: commonly reported by 10–20% of users, even though clinical trials often dismiss it as “nonspecific.”

• Increases risk of developing diabetes and weight gain: acknowledged on FDA labels but downplayed as “small”; occurs in about 1 in 10 long‑term users.

• Cognitive problems: forgetfulness, brain fog, and memory lapses (aptly named “statin brain”) affect roughly 5–10% of users. Long-term statin use has been linked in some studies to increased risk of serious cognitive decline, including dementia and Alzheimer’s.

• Hormonal disruption: low libido, erectile dysfunction, and reduced testosterone or estrogen levels; affects several percent of men and women on long-term statins, though rarely acknowledged in guidelines.

• Liver strain and abnormal enzymes: acknowledged by mainstream medicine; occurs in 1–3% of users and can sometimes progress to serious liver damage.

• Possible increase in long‑term cancer risk: not listed on labels, but independent studies in populations with very low cholesterol have reported associations with higher cancer incidence.

And that’s just what’s been reported. The true scope of statin harm is far greater.

Most side effects never make it into official records. Only 1–5% of adverse reactions are formally documented, meaning the real impact on patients’ health is dramatically underestimated. The reality is orders of magnitude worse.

How pharma deceives you

Almost every major statin trial was funded by the very companies that stood to profit from positive results.

They control the data, the publication rights, and the press releases. When a negative result threatens profits, it quietly disappears.

That isn’t science—it’s marketing with footnotes.

I’ve documented the broader corruption of medical journals and published research elsewhere. What follows are the tactics most relevant to how the pharmaceutical industry manufactures the illusion that statins are safe and effective.

Here’s how they do it.

Rigging the study participants in advance

Before a clinical trial even begins, researchers “pre-screen” participants. On paper, it sounds like routine housekeeping; in practice, it filters out anyone likely to report problems later.

• Placebo run-in: Participants take a dummy pill for weeks. Those who report problems are removed. The result is a population more likely to tolerate the drug and less likely to complain, even about real harms.

• Active-drug run-in: Participants take the actual statin for a few weeks before the trial officially starts. Anyone who develops muscle pain, fatigue, memory issues, or other statin side effects is quietly removed for “intolerance.”

By the time the trial officially begins, the population has been hand-picked to include only the most compliant and tolerant participants. Every person most at risk of harm has already been edited out. It’s an effective way to make a toxic drug look harmless.

Advertising “relative risk” to make tiny differences seem huge

One of the oldest tricks in the pharmaceutical playbook is the relative vs. absolute risk deception.

If a drug reduces heart-attack risk from 2% to 1%, the real-world benefit is 1 percentage point.

But that same result is advertised as a “50% reduction in risk.”

The math is technically correct—and profoundly misleading. When you delete the denominator, tiny effects suddenly look dramatic.

That single statistical maneuver turns a marginal difference into a media headline and justifies a lifetime prescription.

Patients end up sacrificing their health for a ratio that exists only on paper.

Silencing dissent

Doctors and researchers who question the statin narrative are branded “anti-science,” while industry-aligned voices receive front-page coverage.

Medical journals—typically financially dependent on pharmaceutical advertising and reprint sales—decide which studies are “not of sufficient public interest.”

The media then cites those same journals to label alternative analyses as “misinformation.”

The feedback loop is airtight: fund the research, filter the participants, spin the numbers, and suppress dissent.

Every step leads to the same outcome—a scientific record in which statins win, and the people harmed by them effectively disappear.

Conclusion

This was never an honest mistake.

The flaws in the cholesterol hypothesis have been known for decades. The harms of statins have been documented since the 1990s. The role of ultra-processed, sugary foods in chronic disease has been obvious to anyone willing to look. Yet the same institutions—medical journals, guideline committees, regulatory boards—continue to act as if none of this exists.

Why? Because too much money, status, and legal liability are tied to maintaining the illusion.

A cardiologist who challenges the guidelines risks losing grants, hospital privileges, and even their medical license—facing professional censure, legal threats, or career-ending retaliation. A journal that publishes inconvenient data risks losing the revenue from the pharmaceutical industry that keeps it afloat. So the narrative survives—not because it’s true, but because it’s enforced.

Despite overwhelming evidence of harm, cholesterol targets are continuously lowered. Statins are now promoted for lifelong use, for children, and even floated as candidates for mass medication. Many doctors go so far as to suggest adding statins to the water supply—a testament not to science, but to institutional momentum and groupthink.

Statins aren’t about health—they’re a profit engine. A drug that fails to extend life, causes widespread harm, and still dominates global guidelines tells you everything about the system that protects it.

Doctors repeat the same talking points, but the narrative collapses when patients stop taking statins, clean up their diets, and feel better.

The statin story isn’t just about one drug. It’s a case study in modern medicine itself. Replace “cholesterol” with almost any other manufactured health crisis and you’ll see the same machinery at work: exaggerated fear, captured science, compliant media, and silenced dissent.

I thought it would be nice for my first Substack Post of 2026 to be geared toward community building.

Substack has become one of the most important platforms for long-form writing on health and medicine—particularly for those willing to challenge institutional narratives.

But anyone who’s watched the last decade of online publishing knows how this usually ends: platforms tolerate controversial ideas—until they don’t.

The pharmaceutical industry is a criminal enterprise generating trillions of dollars in revenue. It will not take sustained, organized criticism lying down—and it will use every tool at its disposal to suppress voices that threaten its interests.

I expect Substacks covering controversial topics will eventually get nuked. But the fact that you can take your email list with you is a game-changer. On every other major platform, you’re operating on rented land. When the rug gets pulled, there’s no recourse.

On that note, a friendly reminder to fellow Substack writers: regularly download and back up your email list. Audience ownership only matters if you actually safeguard it. If this platform turns hostile, those lists will be the lifeboat that allows communities to regroup elsewhere and keep the work alive.

Why we need more voices

We didn’t end up where we are because of one corrupt doctor or scientist lying in isolation. Massive institutions—and the legions of people on their payrolls—repeated the same narratives in unison, manufacturing a social consensus about what is “true,” even when it isn’t.

That’s why we need as many voices as possible amplifying more honest perspectives on health, in every way they can. People wake up differently: for some, it’s a long-form article; for others, it’s a meme, a short post, or a concise summary that finally cuts through. Most people just follow the herd, so volume and repetition are what break the spell.

The purpose of this directory is to help you discover health-focused Substacks that challenge mainstream narratives. There are many ways to present information effectively; my own publishing style just happens to lean toward executive-summary articles—synthesizing and distilling the most important, compelling information as concisely as possible for maximum impact—because most people don’t have the time, interest, or attention span to dig through everything themselves.

The directory

Consider this a very rough first draft, not an exhaustive or definitive list. I drew some arbitrary boundaries to keep it manageable for now; all criteria are subject to change as the directory evolves.

• Health-focused Substacks. Only writers primarily covering health are included.

• Substack-native. Writers publishing elsewhere are excluded unless they also post on Substack.

• Active on Substack. Must have posted within the past year.

• Audience size. Rough cutoff of ~3,000 subscribers to keep the list manageable.

• Vaccine stance as a baseline filter. While there is significant disagreement among people on this list across many topics, the minimum requirement for inclusion is that the author does not recommend vaccines. People may disagree on why—but recommending vaccination is a dealbreaker for this directory.

This current draft reflects my own reading habits and blind spots, so I’ve undoubtedly missed many worthy Substacks—my sincere apologies. Consider this a living document that will evolve over time. I’ve sorted the list alphabetically for now, but in the future I may group Substacks by category. If I’ve mischaracterized a writer’s work, overlooked a Substack that should be here, or made any other mistakes, please let me know in the comments. I welcome all suggestions and feedback.

I want to highlight a recent video compilation by Checkur6 that documents over 1,000 unusual injuries in professional sports since the Covid shot, including 320 deaths, from March 2021 through September 30, 2025.

These are elite athletes we’re talking about—among the healthiest people in the world. And these are only the most high-profile cases. The video would be many times longer if it included deaths and collapses from non-professional sports as well.

Gaslighting you

You’ll find a number of papers and “fact-check” articles online claiming that there has been no increase in sudden deaths or health issues among athletes since the rollout of the Covid shot.

I’m still trying to enjoy the holiday spirit, so I’m not particularly interested in going into exhaustive detail about every method they use to gaslight you. But broadly speaking, the tactics include blended timelines, where the so-called “pandemic period” begins in early 2020—months before a single dose even existed—thereby diluting the very window being examined.

They also ignore exposure normalization. Entire seasons were canceled. Training facilities were closed. Travel was restricted. Competitive minutes collapsed in 2020–2021. Comparing raw event counts without adjusting for play time is misleading.

Basic logic says otherwise

More importantly, basic logic dictates that if you accept that the Covid shot has caused heart issues for some people—which at this point everyone admits, including the CDC, FDA, EMA, WHO, etc.—and given that athletes are among the people who took the Covid shot, then there would necessarily be an increase in heart issues among athletes.

Given the now-unanimous recognition that the Covid vaccine can cause heart issues, the most generous counterargument a true believer can offer goes something like this: “Yes, the shot can cause heart issues, but the effect is so vanishingly rare and so mild that it would be virtually undetectable at the population level.”

If you want to believe that, I’ve got some magic beans you’ll love.

At that point, congratulations—you’ve joined the Ian Weissman School of Medicine, whose members apparently believe the Covid shot is the elixir of life.

Somewhere between then and now, the story radically changed.

Today, that same rash and fever is framed as a crisis, prompting an endless stream of alarmist media coverage, including emergency alerts, public scolding, and moral lectures about “anti-vaxxers endangering the community.”

But measles didn’t change—only how it was marketed to the public.

Simply put: measles isn’t a big deal, yet the media tries to gaslight us into believing it is. Why? To promote vaccines.

Measles in the 1950s–1970s

Measles is viewed as so mild—and even beneficial—that parents deliberately expose their child to it.

A child writes a letter explaining they missed a Christmas concert because of measles, treating it as an inconvenience—not as a serious or frightening illness.

Measles is framed as a mild inconvenience that keeps children from attending a football game, not as a serious health threat.

1952

Measles is treated as a benchmark of harmlessness. Polio is described in comparison, with scientists hoping it might become “as harmless as measles.”

1953

A family is thrilled to attend a “measles party,” revealing how unthreatening the illness is.

“We all whooped and hollered when she invited us to a measles party.”

1954

Measles is explicitly grouped with the “usual diseases of youth,” framed as a normal and expected part of childhood.

1954

Measles is mentioned casually in dialogue between two characters in the film Young at Heart, treated as a shared childhood experience and laughed about, not as a serious illness.

A children’s Christmas song treats measles as a minor inconvenience that might complicate Santa’s visit, rather than something dangerous or frightening.

A county health commissioner recommends hosting measles parties to “get it over with,” highlighting the illness as mild and manageable.

This week, I want to switch gears and share something for those who do want to go deeper: a fantastic new documentary called An Inconvenient Study, produced by ICAN and Del Bigtree.

What it covers

The film unpacks a bombshell Henry Ford Health System study comparing health outcomes between vaccinated and unvaccinated children.

The results? Strong, undeniable associations between vaccination and chronic disease among American kids.

But the bigger story is what happened next—the researchers’ panic about publishing their own results, and the institutional pressure that buried the evidence.

The concern wasn’t scientific accuracy. It was career survival in the shadow of a multi-billion-dollar industry.

Short version

In it, Dr. Marcus Zervos—the study’s lead author—is caught on hidden camera admitting that the data is sound, but publishing the study would be career suicide.

And this wasn’t some junior scientist. Zervos is the Division Head of Infectious Diseases and the Medical Director of Infection Control at Henry Ford Health System. A senior physician. Financially secure. Near retirement. A man who promised to publish the study no matter what it showed.

Still, he folded.

If someone with his freedom couldn’t speak the truth, what hope is there for younger doctors with mortgages, student loans, and careers to protect?

Final thought

Whether you agree with its conclusions or not, watch the film—and then ask yourself:

Why must findings like the Zervos study surface through a documentary instead of medical journals?

That, perhaps, is the most inconvenient truth of all.

But most people don’t have time for any of that.

And even if they do, almost nobody wants to spend it digging into the problems with vaccines.

Like a mantra, we’re told that “vaccines have saved more human lives than any other medical invention in history”—an unassailable truth, reinforced by every authority imaginable.

And barely anyone checks whether it’s actually true.

Spoiler: it’s not.

Death rates collapsed before vaccines arrived

Thanks to better sanitation and rising living standards, death rates for virtually every major disease of the 20th century—measles, diphtheria, pertussis, tuberculosis, scarlet fever, and more—had already fallen by 90–100% before their respective vaccines were introduced. Some of these diseases never had a vaccine at all.

In other words, vaccines can’t take credit for improvements that already happened.

Measles

Diphtheria

Pertussis (whooping cough)

Tuberculosis

Scarlet fever

The same pattern appears wherever we have data—not just in the U.S., but also in England and Wales, Scotland, Ireland, Spain, France, Belgium, Sweden, Norway, Australia, and New Zealand.

The idea that vaccines “saved millions of lives” in the U.S. falls apart once you look at the actual death counts. The table below shows deaths in the year before each vaccine was licensed. Numbers were already small and trending lower. There simply weren’t millions of lives left to save.

Moving the goalposts

Faced with the reality that vaccines don’t live up to the hype, vaccine zealots will move the goalposts.

Okay, fine—death rates were already dropping before vaccines… but look at the number of cases!

Don’t let incidence numbers fool you

Given that vaccines were clearly not responsible for the massive declines in mortality, defenders pivot to case counts or morbidity as evidence of vaccine success. Yet historical case reporting was inconsistent at best. Deaths, by contrast, are concrete events, legally recorded, and therefore far more reliable.

But even if we take incidence numbers at face value, the metric becomes largely meaningless once mortality has already collapsed. By the 1960s, measles deaths had fallen by 99% without a vaccine, and severe polio cases were rare long before mass vaccination.

The diagnostic shell game

Disease definitions often change after a vaccine’s introduction, creating the illusion that the vaccine eliminated the disease—when in reality, only the diagnostic criteria shifted.

Consider polio. Before the vaccine, temporary muscle weakness sufficed for a polio diagnosis. After the introduction of the vaccine in 1955, only cases with paralysis lasting 60+ days counted. Those cases that would have previously been called polio got relabeled as viral meningitis, transverse myelitis, Guillain-Barré, and more.

Conclusion

Death rates for virtually every major disease fell nearly 100% before their respective vaccines existed. Vaccines didn’t cause the decline—they arrived after it had already happened and then claimed the credit.

Proponents try to distract from this reality by pointing to falling case counts as proof vaccines “worked”, but these drops usually reflect narrower diagnostic criteria, not a genuine reduction in illness. Once a vaccine exists, doctors are far less likely to diagnose a vaccinated patient with the very disease the shot was meant to prevent, so they typically call it something else.

Bottom line: death rates fell before the shots even existed. The rest is marketing.

What passes for safety testing in the vaccine world isn’t science—it’s theater. No placebo controls. Absurdly short follow-up periods. Underpowered studies.

To any reasonable person, this would be considered fraud. But when it comes to vaccines, it’s standard practice.

No placebo controls

To determine whether a new drug is safe, you test it against a placebo.

A placebo is something inert, meaning it produces no effect on the human body.

Without this neutral baseline, there’s no way to know for sure what a product does.

It’s what allows you to tell whether the outcomes you see are caused by the product you’re testing—or not.

Yet not a single routine childhood vaccine in the United States was licensed based on a placebo-controlled clinical trial. Not one.

The FDA confirms this in the package inserts for each vaccine, available on their website.

That alone should stop the conversation.

Because without a placebo control, claims about vaccine safety rest on faith, not fact.

What they use instead

Instead of a placebo, vaccines are tested against other vaccines, or injections containing active ingredients like aluminum adjuvants.

If both test and control groups show similar rates of adverse effects, the manufacturer can claim, “See? No difference—the vaccine is safe.”

It’s like comparing whiskey to vodka and claiming whiskey has no effect because both get you just as drunk.

The FDA allows this practice. The CDC defends it. The media ignores it.

Here’s a snapshot showing what (if any) controls were relied upon to license every routine childhood vaccine in the United States:

Vaccine trial pyramid scheme

You might think that, even though none of the vaccines are tested against a placebo, surely they’re at least compared to something that was. They’re not.

Newer vaccines are tested against older ones—but those older vaccines were often tested against yet another vaccine instead of a placebo.

And that one? Same story.

Safety claims for each vaccine rest on the assumption that the vaccine it was compared to was properly tested—but peel back the layers, and there’s no placebo control anywhere along the chain.

None of the routine childhood vaccines were licensed based on trials that used a placebo—or even a control that was.

Absurdly short safety review periods

Even if we ignore the lack of placebo controls, the safety review periods for vaccines are absurdly short—sometimes only three days.

Childhood vaccines typically get just a few days or weeks of monitoring—never more than six months—yet developmental, neurological, and immune disorders (such as autism, ADHD, allergies, etc.) often don’t appear until years later.

In other words, these trials cannot capture harms that emerge years after vaccination.

By contrast, while routine childhood vaccines are monitored for only a brief period, other widely used drugs undergo multi-year trials before approval.

Such brief follow-ups for vaccines cannot capture long-term risks.

Underpowered studies

Even if there were placebo-controlled trials—which there aren’t—and even if the follow-up periods were long enough—which they aren’t—many vaccine studies would still fail to reveal real harms because they are severely underpowered (meaning they include too few participants to detect rare or serious adverse effects).

Many routine childhood vaccine trials enroll only a few hundred or a few thousand children. If a vaccine causes a chronic health condition in 1 in 1,000 children, a trial of 500 would likely miss it.

Post-licensure monitoring

Some may claim that VAERS (Vaccine Adverse Event Reporting System) monitors vaccines post-licensure to ensure their long-term safety.

In reality, VAERS is a passive system that captures “fewer than 1% of vaccine adverse events”—meaning most vaccine injuries are never recorded.

Conclusion

None of the routine childhood vaccines were ever licensed based on long-term, placebo-controlled trials.

When injuries occur, agencies like the CDC and FDA dismiss them with the standard line: “No causal link has been established.” They never explain why no causal link has been established—because without placebo-controlled trials, there is no way to prove it. The system is structured to avoid detecting the very harms it is supposed to protect the public against.

The failure to conduct long-term, placebo-controlled trials for the licensure of routine childhood vaccines is scientifically and morally unjustifiable.

And it remains standard practice.

No other product works like this. Cars, appliances, even hot coffee—if they harm someone, the company can be sued on the grounds that it could have made the product safer. Safety matters because ignoring it is costly.

Vaccines are the only products completely shielded from liability. Neither the manufacturer, the doctor, the hospital, nor anyone else involved in the administration of vaccines can be held legally responsible for harm caused.

For most products, risk to consumers is a company’s risk too—lawsuits, lost revenue, damaged reputation. That risk drives safety. Vaccines reverse this logic: complete legal immunity removes the consequences, making safety an unnecessary cost. Vaccine manufacturers are therefore disincentivized from investigating or improving the safety of their products.

How we got here

By the 1980s, vaccines like DTP, MMR, and polio were more widespread. But as use increased, so did reports of harm: chronic illness, developmental delays, even death. Parents sued in growing numbers.

Manufacturers panicked. Liabilities were massive—one DTP maker faced claims 200 times its annual sales. Companies exited the market, leaving few remaining.

Congress stepped in with the National Childhood Vaccine Injury Act of 1986. In plain terms:

“You can sue McDonald’s if your coffee is too hot. But you cannot sue Pfizer if their vaccine harms your child.”

Vaccine court

Since 1986, families have been barred from suing vaccine makers directly. All claims must instead go through the Vaccine Injury Compensation Program (VICP), also known as “Vaccine Court.”

Here, victims don’t sue the manufacturer; they must sue the government—an opponent with effectively unlimited resources.

VICP was sold as a fast, fair way to help families injured by vaccines. In reality, most petitions are denied. Cases drag on for years, hidden behind opaque procedures, with little chance of meaningful justice.

Even when payouts occur, they’re funded by taxpayers—not by the corporations responsible.

The result: zero accountability, zero incentive for safety. Vaccine manufacturers reap the rewards while taxpayers cover the costs. The government acts as both mediator and shield. On paper, victims are compensated; in practice, the system protects the perpetrators.

Both regulator and defendant

The Department of Health and Human Services (HHS) both approves and promotes vaccines, and also defends them in Vaccine Court when they cause harm—a glaring conflict of interest.

The result is predictable: systemic bias, cover-ups, and deliberate ignorance. The department meant to enforce safety instead colludes with the industry it’s supposed to regulate.

The explosion in the vaccine schedule

Once liability was removed, the financial risk of unsafe vaccines vanished—removing the biggest check on growth.

The childhood vaccine schedule subsequently ballooned from a handful of recommended shots in 1986 to more than 70 doses by age 18 under today’s guidelines.

The vaccine industry went from being on the brink of ruin in 1986 to a $92 billion global market today, projected to reach $161 billion by 2034.

If they’re safe, why legal immunity?

Vaccine makers insist their products are “safe and effective.” So why do they need blanket immunity?

The only other products with partial protection are guns—but even then, only when used illegally. Gun companies can still be sued for design flaws, and nobody is required to buy one.

Pharma already has a captive, mandated market with free promotion from the government, yet it still demands immunity. If liability were restored, companies would only need to make vaccines as safe as possible and disclose their risks—the same standard that applies to every other product.

The need for blanket protection exposes just how unusual the system is.

Conclusion

No one would buy a car (or drive their family in one) if the manufacturer wasn’t liable for safety—but we accept mysterious injections from pharma giants with a decades-long record of criminal wrongdoing, who face zero consequences for the harm their products cause.

Even without looking at any clinical trials, this legal setup alone should raise red flags.

For many parents, vaccine makers’ blanket immunity for injuries or deaths is reason enough to refuse them.

And it only gets worse from here.
