A Disease Older Than History Books

Let’s go back to around 3000 BCE in the Edwin Smith Papyrus, an ancient Egyptian medical text. It described hard masses in the breast and ended with a heartbreaking statement. The conclusion was simple and tragic:

«There is no treatment.»

Centuries later, the Greek physician Hippocrates, often called the Father of Medicine, observed tumors with swollen blood vessels extending outward. To him, they resembled a crab, so he called them karkinos, the Greek word for “crab.”

During the Roman era, the physician Aulus Cornelius Celsus, used the Latin term cancer for these tumors, which also means “crab.” The name has remained with us for nearly two thousand years.

Centuries later, physicians expanded the understanding of this disease.

Al-Zahrawi (Abulcasis) described surgical techniques for removing these masses and advocated complete removal whenever possible.

Ibn Sina (Avicenna), in The Canon of Medicine, stressed that the disease should be treated as early as possible and observed that completely removing the affected tissue offered the best chance of success.

During the nineteenth and twentieth centuries, scientists began to understand the true nature of the disease. Through advances in pathology, microscopy, and later genetics, physicians transformed the way they diagnosed, studied, and eventually treated it.

Although cancer is often considered a modern disease, it is one of the oldest diseases ever described. But discovering its name answered only one question. A far more fascinating one remained

The Birth of a Rebel Cell

Your body contains trillions of cells.

Every second, millions of them divide, replace damaged tissue, heal wounds, and keep your organs functioning. This process is tightly regulated by genes that tell cells when to grow, when to stop dividing, and when to die

Sometimes, however, mistakes occur.

Multiple factors and conditions can damage the DNA. Most of this damage is repaired immediately. If the damage is too severe or occurs repeatedly, and cannot be properly repaired, the cell usually destroys itself through a process called apoptosis, or programmed cell death.

Cancer begins when these protective systems fail.

Mutations accumulate in genes that normally control cell growth. However, it is important to understand that cancer is rarely the result of a single mutation or a single risk factor. Instead, it usually develops after multiple genetic alterations accumulate over time.

The affected cell ignores signals telling it to stop dividing. It survives when it should die, continues multiplying while acquiring additional genetic mutations, and can eventually form a mass of abnormal cells called a tumor.

Not all tumors are cancerous. Benign tumors remain confined to one location and do not invade nearby tissues. Malignant tumors, however, can invade surrounding structures and spread to distant parts of the body.

That is cancer.

How Does Cancer Develop?

If cancer begins with DNA damage, why does that damage happen in the first place?

After heart disease, cancer is the second leading cause of death worldwide According to the WHO.

However, cancer is rarely caused by a single factor.

The answer begins with something happening inside our cells all the time.

Every day, our DNA is exposed to damage. Most of the time, this isn’t a problem. Our cells have sophisticated repair systems that detect the damage and fix it, while other protective mechanisms can stop severely damaged cells from surviving.

Cancer can develop when genetic changes accumulate in critical genes that control cell growth, DNA repair, and cell survival.

We should ask again If cancer begins with DNA damage, why does that damage happen in the first place?

1. Things that can damage or alter our DNA:

Tobacco: The King of Cancer Risk Factors

If there is one major culprit, it is tobacco.

According to the World Health Organization (WHO), tobacco use is actually responsible for 25% (or 1 in 4) of all cancer deaths globally.

Tobacco smoke contains at least 80 carcinogens that have been classified by the International Agency for Research on Cancer (IARC). These include substances classified as Group 1, Group 2A, or Group 2B carcinogens.

These chemicals do not attack the body in the same way. Each damages DNA through different biological mechanisms, increasing the likelihood of harmful mutations.

So why does smoking-related cancer take so long to appear?

Smoking-related cancer often takes years or decades to develop because the damage accumulates gradually.

Years of exposure can leave many cells in a tissue carrying different genetic changes. Eventually, one cell may acquire the particular combination that allows it to escape normal controls.

This is why one cigarette does not cause cancer. The real danger is becoming addicted and exposing your cells to repeated DNA damage over many years.

Alcohol

Alcohol increases the risk of cancers of the: Mouth, Throat, Esophagus, Liver, Breast, Colon and rectum

Alcohol itself can contribute to cancer risk, and one important reason is its breakdown into acetaldehyde, a toxic compound that can damage DNA.

Ultraviolet (UV) Radiation

UV radiation is the leading environmental cause of skin cancer, including:

- Melanoma

- Basal cell carcinoma

- Squamous cell carcinoma

It damages the DNA of skin cells. If this damage is not properly repaired, mutations accumulate and may eventually lead to cancer.

Types of UV Radiation

UVA (315–400 nm)

- Penetrates deeply into the skin.

- Produces reactive oxygen species (ROS).

- Causes indirect DNA damage.

- Contributes to skin aging and skin cancer.

UVB (280–315 nm)

- The main cause of sunburn.

- Directly damages DNA by forming cyclobutane pyrimidine dimers (CPDs) and 6-4 photoproducts.

- If these lesions are not repaired by the nucleotide excision repair (NER) pathway, mutations can develop in genes such as TP53, one of the body’s most important tumor suppressor genes.

UVC (100–280 nm)

- Extremely damaging.

- Fortunately, almost completely absorbed by Earth’s ozone layer.

- Mainly encountered in laboratory or industrial settings.

Some chronic infections can also contribute to cancer.

According to the World Health Organization, approximately 10% of cancers diagnosed worldwide in 2022 were attributable to carcinogenic infections.

The burden is much higher in low- and lower-middle-income countries, where cancer-causing infections account for approximately 30% of cancer cases.

Examples include:

- HPV → Cervical, anal, penile, vulvar, vaginal, and oropharyngeal cancers

- Hepatitis B and C viruses → Liver cancer

- Helicobacter pylori → Gastric cancer and gastric MALT lymphoma

- Epstein–Barr virus (EBV) → Nasopharyngeal carcinoma and several lymphomas.

Diet, Obesity & Physical Inactivity

Processed meat is classified as a Group 1 carcinogen. A high intake of red meat, diets low in fiber, and excess body fat are also associated with an increased risk of several cancers.

You should know that risk increases with higher consumption, and that classification as Group 1 means there is strong evidence of carcinogenicity.

Obesity contributes by promoting Chronic inflammation, Higher insulin and IGF-1 levels, Increased estrogen production from fat tissue

These changes create an environment that favors cancer development.

A sedentary lifestyle is closely linked to this problem. Regular physical activity lowers the risk of several cancers, including colorectal and breast cancer.

The goal is not to completely eliminate these foods, but rather to maintain a balanced diet and healthy lifestyle.

2.Things that change our vulnerability

Hormonal & Reproductive Factors

Hormones influence the growth of certain tissues. Therefore, long-term exposure to estrogen can increase the risk of hormone-sensitive cancers.

Examples include:

Certain forms of long-term menopausal hormone therapy can alter the risk of hormone-sensitive cancers, depending on the type and duration of treatment.

Early onset of menstruation

Late menopause

These factors increase lifetime estrogen exposure, raising the risk of breast and endometrial cancers.

Inherited Genetic Mutations (Non-Modifiable)

Some people are born with inherited mutations that greatly increase their risk of developing cancer at a younger age.

BRCA1 & BRCA2

These are tumor suppressor genes.

Normally, they:

- Repair damaged DNA.

- Prevent mutations from accumulating.

- Help stop cells from becoming cancerous.

If someone inherits a harmful mutation in BRCA1 or BRCA2, DNA repair becomes much less efficient. Over time, mutations accumulate, increasing the risk of:

- Breast cancer

- Ovarian cancer

- Prostate cancer

- Pancreatic cancer

Inherited BRCA mutation ≠ inherited cancer. A person inherits a predisposition, not the cancer itself.

Lynch Syndrome is another inherited condition in which DNA mismatch repair is impaired, increasing the risk of colorectal and endometrial cancers.

- MLH1, MSH2, MSH6, PMS2

It is mainly associated with Colorectal cancer and Endometrial cancer

According to the World Health Organization, approximately inherited mutations explain only about 5–10% of all cancers.

The remaining 90–95% develop because mutations accumulate during life through aging, environmental exposures, lifestyle factors, infections, and random DNA errors.

3.And finally: Random DNA Errors

The “Bad Luck” Factor (Random DNA Errors)

Even under ideal conditions, cells occasionally make mistakes while copying their DNA during normal cell division.

The more often a tissue renews itself—such as the colon, skin, or bone marrow—the more opportunities there are for copying errors.

These unavoidable copying errors explain why cancer can sometimes develop in people who have never smoked, maintained a healthy lifestyle, and had no obvious risk factors.

Importantly, having one or even several risk factors does not guarantee that a person will develop cancer. Likewise, some individuals with no obvious risk factors may still be diagnosed with the disease.

How Cancer Outsmarts the Body?

One of the most fascinating aspects of cancer is its ability to survive despite the body’s defenses.

Normally, the immune system can recognize and eliminate many abnormal cells before they become clinically significant.

Cancer cells, however, can evolve strategies to escape immune detection.

As the tumor grows, it also stimulates the formation of new blood vessels (a process called angiogenesis) to supply itself with oxygen and nutrients.

Eventually, some cancer cells can detach from the primary tumor, invade surrounding tissues, enter the bloodstream or lymphatic system, and establish new tumors in distant organs.

This process is known as metastasis, and metastatic disease is responsible for the vast majority of cancer-related deaths.

In advanced disease, treatment is often aimed at controlling the cancer, prolonging survival, and relieving symptoms rather than achieving a cure—although some metastatic cancers can still be treated with curative intent.

The Silent Nature of Cancer

One of the major challenges of cancer is that it can develop silently, particularly in its early stages.

In their earliest stages, many cancers cause few or no symptoms.

This is why screening for cancers such as breast, cervical, and colorectal cancer is so important.

Depending on the type and location of the cancer, possible symptoms may include:

- Unexplained weight loss.

- Persistent fatigue.

- A lump or swelling.

- Unusual bleeding.

- A persistent cough.

- Changes in bowel or bladder habits.

- Difficulty swallowing.

How Medicine Fights Cancer

The treatment of cancer has changed dramatically over the past century.

• Chemotherapy uses drugs that interfere with cancer-cell growth and division and can circulate throughout the body.

• Targeted therapies attack specific molecular abnormalities found in cancer cells while sparing many healthy tissues.

• Immunotherapy helps the immune system recognize and eliminate cancer cells, transforming the treatment of diseases such as melanoma and certain lung cancers.

• In selected patients, advanced treatments such as CAR-T cell therapy have produced remarkable results by genetically modifying a patient’s T cells to recognize and attack cancer cells.

• Modern oncology is increasingly moving toward precision medicine, where treatment is tailored to the genetic characteristics of both the patient and the tumor.

Can Cancer Be Prevented?

Although not every cancer can be prevented, experts estimate that a substantial proportion of cases could be avoided by reducing exposure to known risk factors.

Prevention includes:

• Avoiding tobacco in all forms.

• Receiving HPV and hepatitis B vaccinations.

• Maintaining a healthy body weight and avoiding excess weight gain.

• Exercising regularly.

• Following a healthy dietary pattern rich in whole grains, vegetables, fruits, and legumes.

• Avoiding alcohol or limiting its consumption.

• Protecting the skin from excessive ultraviolet radiation.

• For many cancers, detecting the disease at an early stage can significantly improve treatment options and survival.

• Looking Toward the Future

Cancer research is advancing at an unprecedented pace.

Scientists are developing blood tests capable of detecting cancers before symptoms appear, known as liquid biopsies.

Artificial intelligence is improving the interpretation of medical images,pathology, and other clinical data. while precision medicine is allowing treatments to be tailored to each patient’s unique tumor.

Researchers are also exploring therapeutic cancer vaccines, improved immunotherapies, and gene-editing technologies that may one day transform cancer care even further.

Although cancer remains one of humanity’s greatest medical challenges, our understanding and ability to treat it are vastly different from those of ancient Egypt. Many cancers that once had very limited treatment options can now be prevented, detected earlier, effectively controlled, or even cured.

The story of cancer spans more than five thousand years, from ancient Egyptian physicians who could only observe and describe its effects to modern scientists decoding the genetic alterations that drive cancer.

Every discovery has brought us closer to understanding this complex disease. While the fight is far from over, continued advances in prevention, early detection, and innovative treatments are giving millions of people around the world new reasons for hope.

The story of cancer is ultimately not only a story of disease—but also one of human curiosity, scientific progress, and resilience.

Night is fascinating, but the greatest mystery begins after we close our eyes.

1. Circadian Rhythm and Sleep Cycle:

First, let’s take a closer look at sleep and what happens while we are sleeping.

The Circadian Rhythm:

Sleep is a normal, periodic physiological state characterized by a temporary loss of consciousness, reduced responsiveness to external stimuli, muscular relaxation, slower heart rate and breathing, and periods of dream activity.

This circadian sleep-wake rhythm, which spans approximately 24 hours, is the result of dual regulation: an internal genetic biological clock and external synchronizers. Deep within the brain, the master clock resides in the suprachiasmatic nuclei (SCN) of the hypothalamus, while peripheral clocks exist in every cell of the body.

Although this internal machinery is autonomous, it needs to be reset daily by external signals, the most powerful of which is the day-night cycle. Specifically, the light perceived by the retina is transmitted via the retinoptohypothalamic tract to the SCN; during the day, this light signal blocks the secretion of melatonin by the pineal gland, keeping the body in a state of wakefulness.

Conversely, when darkness falls and this inhibition is lifted, the pineal gland begins to release large amounts of this “time-giving hormone,” which acts as a chemical signal for evening, preparing the body for sleep.

Alongside this light-based mechanism, the transition between wakefulness and sleep depends on a set of neurological inhibitions: during wakefulness, the excitatory nuclei of the brainstem (ascending activating system) keep the cortex alert thanks to external sensory stimulation, as well as internal signals.

However, when light dims and external stimuli become scarce, the anterior hypothalamus takes over and actively inhibits these brainstem nuclei; deprived of its activators, the cortex then passively synchronizes with the slow rhythmic discharges of the thalamus (the well-known 3 Hz delta rhythm), plunging the individual to fall asleep.

This process does not stop there: it is coupled with homeostatic regulation, because during wakefulness, brain activity generates a progressive accumulation of hypnogenic (sleep-promoting) substances in the brain, and it is only when their concentration crosses a certain threshold that the onset of sleep becomes inevitable, thus completing the loop of this daily cycle.

The Sleep Cycle:

The sleep cycle is the rhythmic and repetitive organization of sleep during a single night, alternating strictly between slow-wave sleep and REM sleep. Specifically, a complete cycle lasts between 1.5 and 2 hours (approximately 90 minutes on average), and a normal night’s sleep consists of 4 to 5 successive cycles.

Each cycle invariably begins with slow-wave sleep (non-REM), which gradually deepens: we move from stage 1 (light sleepiness) to stage 2, then we plunge into stages 3 and 4, which correspond to deep slow-wave sleep (the “core” of sleep, with low brain activity).

After this gradual descent, the cycle ends with a REM sleep phase, characterized by highly active brain activity (close to wakefulness) and complete muscle atonia; this is the period when dreams occur. It is, in fact, at the end of each cycle, just after this REM phase, that we can naturally wake up for a few moments before moving on to the next cycle.

If we look at the hypnogram (the sleep curve), we observe a very precise distribution: deep slow-wave sleep is heavily concentrated during the first 4 to 5 hours of the night, making it the body’s absolute priority for recovery; conversely, REM sleep, very short at the beginning of the night, sees its phases gradually lengthen over the course of the cycles, becoming increasingly longer and more intense towards the end of the night, towards morning.

Ultimately, over the course of the night, the overall distribution is approximately 75% slow-wave sleep (ensuring physical recovery) and 25% REM sleep (ensuring cognitive functions, memory consolidation, and synaptic reorganization), thus forming this cyclical and dynamic structure that governs our nights.

2. Brain While Sleeping:

During sleep, the brain doesn’t shut down completely; on the contrary, it enters highly organized states that vary radically depending on the stage. This shift to these phases is triggered by a homeostatic mechanism: during wakefulness, hypnotic substances (promoting sleep) gradually accumulate in the brain, and when their concentration crosses a certain threshold, they force the body to enter this complex process of brain restoration and reorganization.

Dreams primarily emerge during REM sleep, a phase of intense neurological activity: characterized by rapid eye movements and temporary muscle paralysis wich prevents us from physically acting out our dreams.

Brain scans reveal that the limbic system is highly active; the amygdala (the emotional center) and the hippocampus (the memory center) become hyperactive; while the prefrontal cortex, the seat of logic, reasoning, and self-control, is strongly inhibited. This imbalance helps explain why our dreams are so emotionally charged, chaotic, illogical, or surreal. But how are these dream scenarios actually constructed?

According to the activation-synthesis theory, it all begins with random neuronal activity generated by the brainstem. The cerebral cortex, like a skilled storyteller, receives these disparate signals and desperately tries to integrate them into a coherent story, thus crafting a narrative from this background noise.

However, this process is not merely meaningless noise: during this process, the brain takes the opportunity to replay fragments of our day, sorting through useful information, discarding the superfluous, and reinforcing important memories.

Simultaneously, dream activity may serves as an emotional reset, offering us a safe space to relive, come to terms with, and process our fears, unresolved conflicts, or traumas, which may help explain why people with PTSD often relive tramatic experiences in their dreams.

Furthermore, by freeing our thinking from the constraints of logic, dreaming may allow us to form new connections and explore original associations of ideas, which has given rise to masterpieces such as Mary Shelley’s Frankenstein or Mendeleev’s discovery of the periodic table.

Some evolutionary psychologists add that these scenarios, even distressing ones (like being chased or trapped), may help prepare us to face danger by allowing us to rehearse our survival strategies in a completely safe virtual environment. Thus, dreaming is not simply a passive byproduct of sleep: it is an active and multifunctional process where the brain, from a random spark, weaves a personal narrative that simultaneously helps us to memorize, heal our emotions, invent, and prepare for the challenges of the outside world.

3. Dreams Through Different Lenses:

While neuroscientists agree on the biological mechanisms of REM sleep, the question of dream meaning deeply divides specialists. Sigmund Freud, the father of psychoanalysis, proposed a radically different interpretation: according to him, dreams represent the “royal road” to the unconscious, a disguised outlet for repressed desires, primarily sexual or aggressive, which the ego’s censorship forbids in the waking state.

For Freud, the manifest content (the bizarre scenario that one recounts upon waking) is merely a decoy; only the latent content (the hidden meaning) truly matters, and it is up to the psychoanalyst to decipher the symbols, displacements, and condensations to reveal the unconscious wish buried beneath the nocturnal narrative.

Carl Jung, a former disciple of Freud, moved away from this exclusively sexual view to develop a much broader and more spiritual approach. Jung believed that dreams are not “deceptions” intended to conceal drives, but authentic and spontaneous messages from the unconscious, that seek to compensate for imbalances in the conscious personality and guide the individual toward the process of individuation (self-realization).

In contrast to symbolic approaches, a decidedly materialistic view is often taken, where dreams are regarded as little more than an epiphenomenon—a by-product—of brain activity. Other models, however, allow for the possibility that dreams might serve psychological or adaptive functions.

Under frameworks like activation-synthesis, for instance, dreams can be interpreted as the cortex’s attempt to make sense of random neural discharges, an approach that inherently excludes any universal symbolism or latent desire from the equation.

Other models, which emphasize memory consolidation, acknowledge that dream content may occasionally reflect daily concerns or anxieties, yet this perspective generally sets aside Freudian decoding or Jungian archetypes as scientifically unverifiable.

It is also possible to consider that these two interpretive levels are not mutually exclusive. In this combined view, biological mechanisms could account for the “container” of the dream—the neuronal theater and REM sleep processes—while psychology and psychoanalysis might illuminate the “content,” or the way personal history, culture, and emotions dress up the underlying neural noise and give it an appearance of meaning.

In the end, the dream emerges as a boundary phenomenon, where biology meets subjectivity, and where different schools of thought grasp only one facet of an experience that remains, in large part, still deeply mysterious.

4. Why we forget our dreams :

When we wake up after a long night’s sleep, we forget about 95% of our dreams, and this is largely explained by a very specific neurochemical phenomenon. During REM sleep, neurotransmitters essential for memory, such as norepinephrine, are at extremely low levels in the brain, which prevents the efficient transfer of dream content from short-term to long-term memory. Adding to this is the reduced activity of the prefrontal cortex (the seat of logic and organization) during REM sleep: since it is not fully operational, the brain struggles to structure and “label” dream memories coherently. As soon as we wake up, if we don’t mentally replay the scenario or write it down immediately, these neural traces fade very quickly, because our brain considers this information non-essential to our waking functions.

So, the next time you have a dream, you’ll have a better understanding of how it happened—even if its true meaning remains a mystery.

If we check Wikipedia, we find that yawning is that involuntary “big inhale, stretch, exhale” thing your body does when you’re tired, bored, or just reading an article on yawning. In technical terms, it’s an involuntary wide opening of the mouth with a deep inhalation, followed by a slow exhale. A typical yawn lasts about 4–7 seconds and often comes with eye-closing or a full-body stretch. It’s mostly a reflex, you can’t really force a yawn, even if you try (though weirdly, you can “fake” one by inhaling slowly with your tongue curled back).

• Evolutionarily, this social yawning may help groups stay alert or connected. Studies even show wolves, dogs, chimpanzees, and other species yawn contagiously, often aligned with social bonds, further proving yawning’s role in emotional contagion.

o   Ampullary tumor
