Not glamorous work. Compare capacities, cross-check clearances, verify what the marketing copy says against what the engineering documents actually say.</p><p>One retailer described a 42-inch-wide bed frame as a &#8220;bariatric option.&#8221; The weight capacity was 450 pounds &#8212; identical to the narrower frame. Nothing about the bed changed except the width. Somewhere between the manufacturer&#8217;s spec sheet and the retail listing, &#8220;wider&#8221; quietly became &#8220;bariatric,&#8221; a clinical term with a specific meaning that has nothing to do with frame width. Nobody who understood what that word means to a clinician was in the room when that copy got written.</p><p></p>

Not analyzing data someone handed her. Building it, from raw open sources, herself.</p><p>In the comments, another nurse named the real divide: most of us are trained to work with data that&#8217;s already been packaged. Premade cake mix. Add a couple wet ingredients, pour, bake. The actual dry mix &#8212; the pile of raw data before anyone cleaned it, structured it, decided what it meant &#8212; that part eludes us. We&#8217;re never taught to build the mix. Only to use it.</p><p>I&#8217;ve spent the last year and a half learning to build the mix.</p><p>Every course I write, every article I fact-check, every claim I put my name behind &#8212; the standard I hold myself to is the same one that post described. Don&#8217;t repeat what&#8217;s already circulating. Go back to the primary source. CMS regulatory text, not the summary of the summary. Peer-reviewed literature, not the blog post that cited it secondhand. Verify the number before it goes in front of a reader.</p>

Not wandering. Terrified. Going door to door, banging on the houses of people he had lived beside for years, asking them to call the police.</span></p><p><em><span>She&#8217;s trying to kill me. Help. Help.</span></em></p><p><span>She was on the porch. Watching her neighbors look at her.</span></p><p><span>Fifty years married. He had Parkinson&#8217;s first, then the dementia came, the way it so often does. Lewy body. She had learned to say it. She could spell it by then.</span></p><p><span>She was seventy and she was doing all of it. Every medication. Every appointment. Every night.</span></p><p><span>And she had not asked anyone for help until that afternoon.</span></p><p><span>Here is the part that matters. Nothing had gone wrong that day. He was not overmedicated. He had not missed a dose. He had not fallen.</span></p><p><span>What happened was Lewy body dementia doing exactly what Lewy body dementia does.</span></p><p><span>Hallucinations and delusions are not complications of this disease. They are features of it. Written into the diagnostic criteria. There is even a name for the specific thing that happened to him &#8212; the belief that the person you love has been replaced by an impostor who means you harm. It has been described in the literature for decades.</span></p><p><span>Somebody had diagnosed him. Somebody had prescribed for him. Somebody had scheduled the follow-ups.</span></p><p><span>Nobody had told her this was coming.</span></p><p><span>I went to the house.</span></p><p><span>There were no assistive devices. No hospital bed. No lift. Nothing that would have told you a person with advancing Parkinson&#8217;s and dementia lived there.</span></p><p><span>There were medications. A lot of them. And a calendar with appointments written in on almost every week.</span></p><p><span>And there were bed rails.</span></p><p><span>Not the kind that gets ordered, or measured, or fitted to a frame. The kind you buy yourself and slide under the mattress and hope holds.</span></p><p><span>She had bought them. Because he was falling out of bed, and nobody had come to the house to look at the bed, and she had done what any reasonable person does now. She searched for the problem she had, and she bought what came up.</span></p><p><span>Those rails have a history. There are documented injuries and deaths from adult portable bed rails, and the mechanism is the gap &#8212; the space that opens between the rail and the mattress when a body presses into it. A person who is confused, or agitated, or hallucinating is the person most likely to end up in that gap.</span></p><p><span>Which is to say: the population these get bought for is the population they are most dangerous to.</span></p><p><span>She did not know that. How would she?</span></p><p><span>She was not being careless. She was being resourceful, alone, with no one to ask.</span></p><p><span>I have been a nurse for forty years. ICU, PACU, endoscopy, and years of VA case management where I sat in people&#8217;s living rooms and looked at how care was actually happening. This is the thing I saw more than anything else, and it is the thing I still cannot get past.</span></p><p><span>We send families home to do clinical work.</span></p><p><span>Not emotional work. Not companionship. Clinical work. Repositioning a body that cannot reposition itself. Watching skin for the first sign of breakdown. Managing a medication list that would make a floor nurse slow down and read it twice. Deciding, at two in the morning, alone, whether this is the night that warrants a call.</span></p><p><span>That is nursing. We just stopped calling it that when we stopped paying for it.</span></p><p><span>She looked at me in that kitchen and said the only thing left to say.</span></p><p><em><span>I don&#8217;t know what to do anymore.</span></em></p>

You learn, in advance, what to say when a colleague humiliates you. You practice the words until they&#8217;re available to you under stress, because under stress most people can&#8217;t find words at all &#8212; acute stress degrades working memory, and the part of you that composes sentences goes offline exactly when you need it. </span></p><p><span>So you pre-load the response while you&#8217;re calm and retrieve it later as a unit. </span><em><span>I want to talk about what just happened.</span></em><span> </span><em><span>When you said that in front of the patient, it undermined me.</span></em><span> </span><em><span>I need you to answer my question.</span></em></p><p><span>The military does a version of this. Battle drills, immediate action drills, repetition until the response survives the loss of conscious thought. Same mechanism, different setting.</span></p><p><span>It works. The research is real and it goes back  decades. Nurses who rehearse respond more often instead of freezing.</span></p><p><span>I&#8217;ve taught it. I&#8217;d fight to keep it in the curriculum.</span></p><p><span>And it fails, predictably and completely, in most of the situations nurses actually face.</span></p><div><hr></div><p><span>In forty years &#8212; ICU, PACU, Endoscopy, VA case management &#8212; I have watched bullying arrive from different directions, and they are not the same problem.</span></p><p><strong><span>Nurse to nurse.</span></strong><span> This is the one with the name. </span><strong><span>Horizontal violence</span></strong><span>. The seasoned nurse who decides the new one hasn&#8217;t earned help. The eye-roll at the desk. The information withheld and then held against you. This is the situation cognitive rehearsal was actually designed for, and here it has a real chance, because the authority is roughly matched. You can say the sentence and survive saying it.</span></p><p><strong><span>Physician to nurse.</span></strong><span> </span><strong><span>Vertical violence</span></strong><span>. The tone in front of a patient. The question asked in a way that&#8217;s meant to establish that you shouldn&#8217;t have asked it. The behavior everyone has agreed to work around for eleven years because of what he generates for the organization.</span></p><p><strong><span>Supervisor to nurse.</span></strong><span> </span><strong><span>Vertical violence</span></strong><span> &#8212; the quietest one, and the worst. Not raised voices &#8212; changed schedules. Difficult assignments. The performance note that appears in your file after you raise a concern. The reference you&#8217;re going to need.</span></p><div><hr></div><p><span>Years ago at the VA, I came around a corner and found one of our medical assistants standing in the hallway, crying. Her supervisor was standing over her with his finger in her face, shouting. What he was accusing her of doesn&#8217;t matter. How he was doing it is the entire point.</span></p><p><span>They both stopped when they saw me. I stopped too.</span></p><p><span>And then my mouth opened before I had decided anything. I felt my spine straighten. I looked at him and said, &#8220;I don&#8217;t know what&#8217;s going on here, but what I see is her crying and you raising your voice.&#8221; Then, to her, in the strongest voice I had: </span><em><span>Go to the break room. Now.</span></em></p><p><span>He turned to me. &#8220;You may be an RN, but I&#8217;m the supervisor of this department.&#8221;</span></p><p><span>&#8220;I don&#8217;t care. I can write you up like anyone else, and I intend to.&#8221;</span></p><p><span>I did write him up. She wrote him up too.</span></p><div><hr></div><p><span>There were two of us in that hallway, but only one could afford to speak.</span></p><p><span>I didn&#8217;t report to him. My evaluation didn&#8217;t cross his desk. My schedule wasn&#8217;t his to write. When I said </span><em><span>I can write you up like anyone else</span></em><span>, I was spending something I could spare &#8212; and I want to be honest that this is the only reason the words arrived as easily as they did. Courage is cheaper when the bill is small.</span></p><p><span>She reported to him directly. Every sentence available to me carried a different price for her, and she knew exactly what it would cost, down to the dollar. That is why she cried instead of speaking up. Her job, her family, her income, that was what was on the line for her. A much larger bill.</span></p><p><span>And that is what the training never accounts for. When a nurse says nothing to a physician or a supervisor, she is almost never frozen for lack of a sentence. She has the sentence. What she&#8217;s doing in that silence is arithmetic, the same as that medical assistant standing in the hallway. The arithmetic is about her evaluation, her ability to earn more, her assignment next month, whether the person she&#8217;d report this to plays golf with him, the mortgage, how many years until she can leave.</span></p><p><span>Handing her a better-rehearsed sentence does not change a single term in that equation.</span></p><p><span>Sideways, the rehearsal works &#8212; nurse to nurse, peer to peer &#8212; because the cost of using it is a frosty week at the desk. Upward, vertically it doesn&#8217;t, because the cost is your livelihood.</span></p><p><span>Cognitive rehearsal doesn&#8217;t fail because it&#8217;s shallow. It fails </span><strong><span>wherever there is a power differential</span></strong><span>, which is most of it. The tool was built for peers, but we keep deploying it against a hierarchy.</span></p><div><hr></div><p><span>Here&#8217;s the part I&#8217;ve stopped being polite about.</span></p><p><span>Organizations know this. Not in a conspiratorial way &#8212; in the ordinary way institutions know inconvenient things and route around them. Everyone in that building understands the difference between confronting a peer and confronting the person who signs your evaluation. Nobody needs it explained.</span></p><p><span>Which is precisely what makes the training so appealing as a response. It is the one intervention that costs nothing and touches no one with power.</span></p><p><span>Something is wrong on a unit. The organization becomes aware. And the organization responds by </span><em><span>educating the workforce</span></em><span> &#8212; a module, assigned to everyone, completed on a Tuesday, documented. Not by removing the person doing it. Not by asking why he was protected. Not by asking what it means that three experienced nurses left that floor in eighteen months and every exit interview said the same thing.</span></p><p><span>The metric reported afterward is completion rate: a number measuring how many people sat through a video, and nothing whatsoever about whether anyone is still being humiliated at work.</span></p><p><span>The file is now clean.</span></p>

tool last month.</span></p><p><span>Quietly.</span></p><p><span>Most patients have no idea it&#8217;s happening, and honestly, most nurses aren&#8217;t talking about it either &#8212; because we&#8217;re too busy watching AI reshape three other corners of our world at the same time, and nobody&#8217;s connected the dots yet.</span></p><p><span>Here&#8217;s the pattern, and once you see it, you can&#8217;t unsee it.</span></p><p><strong><span>In nursing school</span></strong><span>, students are using AI to study for the NCLEX, and 2026&#8217;s pass rates are falling. Not because students are less capable &#8212; because the cohort testing right now came through nursing school during COVID, when clinical rotations got replaced by simulations and rushed practicums.</span></p><p><span> The Next Gen NCLEX was built specifically to test clinical judgment &#8212; the ability to read a real situation and prioritize in real time.</span></p><p><span> That&#8217;s exactly the muscle that got the least exercise during their training. AI can quiz you on content. It cannot hand you the hundred small unscripted moments at a real bedside that build the instinct the exam is now testing for.</span></p><p><strong><span>In CE content</span></strong><span> &#8212; my own work, so I&#8217;ll say this plainly &#8212; AI can draft a paragraph explaining a clinical protocol in seconds.</span></p><p><span>It cannot tell you that the citation behind that protocol went stale four years ago and nobody caught it, because catching that requires a working nurse who still knows what the bedside actually looks like this year, not what a dataset said it looked like whenever it was trained. AI drafts content. It does not carry clinical currency.</span></p><p><span>Those are not the same skill but continuing education that treats them as interchangeable is how courses go wrong while looking perfectly fine.</span></p><p><strong><span>In everyday practice</span></strong><span>, two-thirds of physicians are leaning on an AI tool mid-encounter, and calling it &#8220;borders on miraculous.&#8221;</span></p><p><span>Meanwhile, down the hall, a nurse doing a routine respiratory assessment feels crepitus under her hands &#8212; that fine, crackling give beneath the skin, the one every nurse learns to recognize as the &#8220;Rice Krispies&#8221; sound and feel &#8212; hours before it shows up as a number anyone would flag. That&#8217;s a negotiated term, reviewed by someone whose job is to protect the hospital&#8217;s spending. Legal read it. Finance approved it. Somebody signed it.</span></p><p><span>The same hospital that couldn&#8217;t find the money to fix New York&#8217;s nearly two hundred documented safe-staffing violations &#8212; the ones on record </span><em><span>before</span></em><span> a single nurse voted to strike &#8212; found the money for that clause without blinking.</span></p><p><span>That&#8217;s not a budget problem. That&#8217;s a priority problem, and the paperwork proves it.</span></p><p><span>Do the math with me for a second.</span></p><p><span>A guaranteed five-day contract for over a thousand replacement nurses, at premium travel rates, doesn&#8217;t come cheap &#8212; we&#8217;re talking well into seven figures for a single one-day strike response. That number got approved, funded, and paid without a second vote.</span></p><p><span>Nearly two hundred documented safe-staffing violations sat on the books in New York before any of this started. Every one of those violations is a version of the same event repeating on a smaller scale, every shift, without a headline: too few nurses, too many patients, too little margin for error.</span></p>

Different hospitals, different cities, same sentence in every statement: <em>not enough of us to keep patients safe.</em></p><p><strong>Here&#8217;s exactly when it happened.</strong></p><p>July 3 &#8212; 178 nurses at MyMichigan Medical Center Alma walked out for three days, ending July 6.</p><p>July 6 &#8212; nurses at Ascension Saint Agnes in Baltimore struck for one day. The same day, about 65 hospice nurses in Minneapolis walked out.</p><p>July 8 &#8212; the big one. Over 4,000 nurses at Brigham and Women&#8217;s Hospital in Boston began what was supposed to be a one-day strike.</p><p>July 13 &#8212; St. Charles Hospital in Port Jefferson, NY.</p><p>I want to slow down on two of these, because what happened <em>after</em> the walkout tells you more than the walkout itself.</p><p><strong>Brigham and Women&#8217;s: the one-day strike that became five.</strong></p><p>Four thousand nurses struck for one day, July 8. The hospital had already flown in nearly 1,300 replacement nurses &#8212; and those replacements were contractually guaranteed five days of work. So the hospital chose to lock its own nurses out until July 13, rather than send the replacements home early.</p><p>Four extra days. Not because the nurses wanted to stay out. Because paying strangers $100 an hour, flights and hotels included, was apparently easier than letting your own staff back in the building.</p><p>When nurses finally walked back in Monday morning, July 13 &#8212; with music playing and bubbles in the air, by every account a genuinely joyful moment &#8212; they still didn&#8217;t have a contract. Eight months of bargaining. I think most of us do.</span></p><p><span>We&#8217;ve been trained &#8212; not gently, not accidentally, but systematically &#8212; to look inward first when something falls apart. Did I chart it wrong. Did I miss something. Did I not advocate hard enough, push hard enough, stay late enough, know enough. We audit ourselves before we ever think to audit the structure we&#8217;re standing inside of.</span></p><p><span>Here&#8217;s what nobody tells you early enough in this career: sometimes the structure is the problem. Sometimes you did everything right and it still didn&#8217;t work, because the door was never actually open &#8212; it just looked like one.</span></p><p><span>I think about the number of nurses I&#8217;ve talked to this year alone who are quietly building something on the side. A course. A consulting practice. A better way to teach the next generation what nobody taught us. Most of them are doing it in stolen hours, unpaid, uncredentialed by the system that trained them, because that system has no lane for a nurse who wants to lead differently.</span></p><p><span>And when it stalls &#8212; when the door that looked open turns out to be painted on &#8212; the instinct is always the same. It must be me.</span></p><p><span>It usually isn&#8217;t.</span></p><p></p>

The new grad asks about the door-to-needle window for tPA. Not by a little &#8212; enough that a patient triaged against the old number could miss a window that the current evidence says is still open.</span></p><p><span>Here&#8217;s the part that should bother you as much as it bothered me: the new grad wasn&#8217;t the sharper clinician. She was the more </span><em><span>recently</span></em><span> certified one. Her knowledge wasn&#8217;t better. It was just newer.</span></p><p><span>That&#8217;s not a competence problem. That&#8217;s a systems problem. And it&#8217;s the same one I keep circling back to, because once you see it, you can&#8217;t stop seeing it.</span></p>

Their case wasn&#8217;t only clinical. It was operational, and it was delivered with total confidence.</span></p><p><span>Here is the thing I want you to sit with: they were not careless nurses. They were good ones. Careful, experienced, the kind you want at the bedside when it goes bad.</span></p><p><span>They were also completely wrong.</span></p><p></p>

Not enough nurses. Not enough pipeline. Not enough graduates coming through. Schools at capacity. An aging workforce retiring faster than new graduates can replace them.</p><p>It&#8217;s a compelling story. It&#8217;s also incomplete.</p><p>The nurse vacancy rate across U.S. hospitals sits at 9.6%, with more than 40% of hospitals reporting vacancy rates above 10%. Recruiting an experienced RN takes an average of 83 days &#8212; nearly three months of operating short-staffed, paying overtime, and watching burnout compound among the nurses who stayed. <a href="https://mtnviewsurgical.com/chemo-port-vs-picc-line/">Mtnviewsurgical</a><a href="https://www.mdanderson.org/cancerwise/should-you-get-a-central-line-for-chemotherapy-.h00-159224934.html">MD Anderson Cancer Center</a></p><p>Meanwhile, as of August 2024, the National Council of State Boards of Nursing reported 5,762,433 active registered nurse licenses in the United States. <a href="https://www.mskcc.org/cancer-care/patient-education/hand-foot-syndrome-skin-reaction">Memorial Sloan Kettering Cancer Center</a></p><p>Read that number again. Nearly 5.8 million active nursing licenses. And hospitals cannot fill their units.</p><p>This is not a supply problem. It is a systems problem. These systems collect, organize, and filter large volumes of applicants, screening for keywords, qualifications, and criteria defined by the job posting. 98.5% of Fortune 500 companies and 75% of all recruiters use one. Every major health system in the country runs applications through one of these platforms before a recruiter ever opens a file. <a href="https://clinicaltrials.gov/study/NCT06733545">clinicaltrials</a><a href="https://www.mypcnow.org/fast-fact/hand-foot-syndrome-palmar-plantar-erythrodysesthesia/">MyPCNow</a></p><p>The system works like this: a nurse applies online. The ATS parses her resume, scores it against the job description&#8217;s keyword requirements, and ranks it against every other applicant in the pool. Most systems score and weight items such as qualifications, education, and job titles, comparing the resume to the job description. Some employers use knockout questions &#8212; &#8220;do you have at least five years of experience in this specific unit type?&#8221; &#8212; and once you answer no, you are out of consideration entirely. <a href="https://www.cancer.org/cancer/managing-cancer/side-effects/hair-skin-nails/hand-foot-syndrome.html">American Cancer Society</a></p><p>According to a Harvard Business School and Accenture report, 88% of employers acknowledged that qualified, high-skilled candidates were rejected outright from the hiring process because they did not exactly match the hiring criteria. 94% said the same. <a href="https://www.elitelearning.com/resource-center/rehabilitation-therapy/massage-therapy/massage-therapists/chemotherapy-radiation-and-massage-therapy/">Elite Learning</a></p><p>Not underqualified. Not poor performers. Qualified candidates &#8212; rejected by an algorithm before a human saw their name.</p><div><hr></div><p><strong>Where Experienced Nurses Disappear</strong></p><p>Here is where the system specifically fails the nurses hospitals say they need most.</p><p>ATS platforms prioritize keyword matching, formatting, and predictive ranking. Candidates who don&#8217;t speak the algorithm&#8217;s language are less likely to move forward &#8212; even if they are clinically excellent. A nurse with 15 years of ICU experience who writes &#8220;monitored hemodynamic status&#8221; instead of &#8220;hemodynamic monitoring&#8221; may score lower than a newer graduate whose resume was optimized for the system&#8217;s parsing logic. <a href="https://dermavitality.com/blogs/wellness-blog/hand-foot-syndrome-related-to-chemotherapy">Dermavitality</a></p><p>The experienced nurse relocating to a new city faces compounding disadvantages. She may have worked at a system that used different unit naming conventions. Her certifications may be formatted differently than the system expects. Her career gap &#8212; even a three-month gap to move, settle her parents into care, and get her bearings &#8212; flags as a risk variable in predictive ranking models.</p><p>64% of U.S. workers aged 50 and older have experienced age discrimination in hiring, and 74% believe their age will be a barrier when applying &#8212; partly because ATS filtering criteria can disadvantage older workers by screening on graduation dates. A nurse who graduated from a diploma program in 1988 and has spent four decades perfecting her clinical judgment may be algorithmically deprioritized before her first sentence is read. <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6775515/">nih</a></p><p>In 2024, Workday &#8212; one of the largest HR software providers in the country &#8212; faced a class-action lawsuit alleging its automated screening systems discriminate based on age, race, gender, and disability status. One plaintiff reported being rejected more than 100 times by companies using Workday&#8217;s platform. y2="14"></line></svg></button></div></div></div></a></figure></div><p>How the hospitality model broke nursing &#8212; and why the data proves it.</p><p><em>The nursing shortage isn&#8217;t a pipeline problem. It isn&#8217;t a pay problem &#8212; though pay matters. It&#8217;s what happens when the people who were called to this work are managed by CEO&#8217;s, who act like they&#8217;re running a Marriott.</em></p><p>In 2012, the Affordable Care Act tied Medicare reimbursement to patient satisfaction scores for the first time. Hospitals that scored well on the HCAHPS survey &#8212; the Hospital Consumer Assessment of Healthcare Providers and Systems &#8212; would be rewarded. Those that scored poorly would be penalized up to 2% of their Medicare payments.</p><p>Today, patient experience accounts for 25% of a hospital&#8217;s Hospital Value-Based Purchasing score. A single percentage point improvement in HCAHPS scores can translate to hundreds of thousands of dollars in additional reimbursement. A low score can cost a hospital system millions.</p><p>That is the moment American healthcare stopped being a clinical enterprise and became a hospitality business. And nurses &#8212; who didn&#8217;t get a vote &#8212; became the front desk staff.</p><p>The HCAHPS survey asks patients whether nurses treated them with courtesy and respect. Whether nurses listened carefully. Whether nurses explained things in a way they could understand. These are reasonable things to measure. But when the financial survival of a health system is indexed to how patients felt about the experience &#8212; not whether they got better, not whether the clinical decision was correct, not whether the nurse caught the early sign of sepsis &#8212; something fundamental shifts.</p><p>The nurse is no longer primarily a clinician. She is a service provider. And her hospital is her employer and her regulator, simultaneously, in a system designed to extract satisfaction data from patients who don&#8217;t always understand what good care looks like &#8212; but who absolutely know whether the room was quiet.</p><h2><strong>The category error nobody names</strong></h2><p>Healthcare CEOs keep making the same mistake. They look at nursing and see a hospitality problem. Clean rooms. Hot meals. Friendly staff. Satisfaction scores. So they run nurses through customer service training. They hire consultants. They implement scripted patient interactions. And they wonder why it doesn&#8217;t stick.</p><p>Here&#8217;s what they don&#8217;t understand: caring isn&#8217;t a skill you train into someone. It isn&#8217;t a script. It isn&#8217;t a behavior checklist you review at your annual evaluation. You either have it or you don&#8217;t.</p><p>The nurse who sits with a dying patient at 3am isn&#8217;t doing it because HR told her to. The nurse who notices the family hasn&#8217;t eaten and quietly brings crackers from the break room isn&#8217;t executing a customer experience protocol &#8212; and it most likely won&#8217;t appear in any Daisy Award nomination or employee review.</p><p>She&#8217;s doing it because that&#8217;s who she is.</p><p>What will appear is the patient complaint &#8212; because you couldn&#8217;t reach a physician for a diet order after surgery was cancelled, and the patient remained unable to eat. That&#8217;s what gets documented. That&#8217;s what follows you.</p><p>Nurses are not waitresses. We are not maids. We are not hotel concierges with stethoscopes. We hold the line between life and death. We absorb trauma so patients don&#8217;t have to carry it alone. We make clinical decisions in seconds that determine whether someone goes home or doesn&#8217;t.</p><p>And then we get a patient satisfaction survey asking if the room was quiet enough.</p><h2><strong>What the data actually shows</strong></h2><p>Press Ganey &#8212; the company that built much of the infrastructure of patient satisfaction measurement in American hospitals &#8212; published its own data last year. They analyzed nearly 115,000 registered nurses and found a 19% nursing turnover rate. They found that 29% of clinical RNs say their organization doesn&#8217;t consistently show them respect. They found that feeling respected at work is the number one driver of nurse engagement.</p><p>Respect. Not satisfaction scores. Not scripted interactions. Respect.</p><p>The same report found that hospitals with the most positive work environments achieve 7.4% higher patient ratings and a 6.7% increase in likelihood-to-recommend scores. In other words, the thing that actually drives patient satisfaction isn&#8217;t customer service training. It&#8217;s treating nurses like professionals.</p><p>Leadership has had this data for years. They built the measurement system that generated it. A convenient one.</p><p>Two years after COVID broke the profession, the data started to improve. Burnout numbers dropped. Job satisfaction climbed. Healthcare leadership exhaled. The crisis, they decided, had passed.</p><p>It hadn&#8217;t. They just stopped looking.</p><p>The 2026 State of Nursing data landed last week. After three consecutive years of improvement, job satisfaction dropped 8 points. The number of nurses likely to leave the profession entirely jumped nearly 50% in a single year.</p><p>One year.</p><p>They declared victory in the middle of a war.</p><p>Here&#8217;s how suppression works in healthcare.</p><p>It doesn&#8217;t require a memo. It doesn&#8217;t require intent.</p><p>The American Journal of Nursing publishes the findings. AONL releases the longitudinal study. The data is technically available. Accessible. Public.</p><p>To whom?</p><p>Not the nurse who hasn&#8217;t taken a lunch break since Tuesday. Not the night shift charge nurse managing four call-outs. Not the bedside RN who clocks out late, goes home exhausted, and starts again tomorrow.</p><p>The data lives where nurses aren&#8217;t. Leadership reads it, exhales, and moves on.</p><p>The headlines wrote themselves. &#8220;Nursing is recovering.&#8221; &#8220;Healthcare workforce stabilizing.&#8221; Nobody with a corner office and a budget had any reason to correct them. The story was convenient. The story was cheap. And the story let leadership off the hook for another fiscal year.</p><p>And while the journals celebrated progress, the body count kept climbing.</p><p>In 2026, 52% of nurses reported verbal threats or aggressive language. 27% reported physical assault. At work. From patients. From families. Sometimes from colleagues.</p><p>Not last decade. This year.</p><p>These aren&#8217;t near misses. These are assaults that go into incident reports that go into files that go nowhere. Because the nurse who just got shoved against a medication cart still has six patients to medicate before the shift ends. Because reporting takes time nobody has. Because nothing changes when you do.</p><p>Leadership calls it a workload problem.</p><p>It isn&#8217;t. Not entirely.</p><p>Workload is the safe diagnosis. The workload has a number. You can put it in a budget proposal, attach a staffing ratio, and show the board a slide. Workload doesn&#8217;t require anyone in the C-suite to look in a mirror.</p><p>Poor management does.</p><p>When experienced nurses leave and take forty years of clinical judgment with them &#8212; when the nurse who knew which patient was spiraling downward is replaced by someone still learning where the central line kit is in the supply room &#8212; that&#8217;s not a staffing ratio failure.</p><p>That&#8217;s a leadership failure dressed up in workforce language.</p><p>And the nurses knew it. Twelve nurses listed in black marker. Green tape separating the columns &#8212; bed numbers, patient names, the notes that told you everything you needed to know before you even found your assignment.</p><p>Big red stars for the ones who needed extra attention. Blood orders. Pre-op prep. IV push medications that the LPN beside you was not permitted to give under Florida law &#8212; so those fell to you too, on top of everything else.</p><p>I found my name. Twelve patients.</p><p>Not eleven. Not ten. Twelve.</p><p>No one had called out. The census was not unusually high. This was just Tuesday.</p><p>I had twelve patients to assess and document before noon. Surgical preps that could not wait. Blood hanging that could not be delegated. IV medications that were mine and mine alone because the law said so. And an LPN partner doing everything she legally could while I carried what she could not.</p><p>The board never lied. It just showed you exactly what the system had decided your shift was worth.</p><p>What I did not know that Tuesday &#8212; what none of us knew &#8212; was that the understaffing was not an accident. One that had been sitting inside hospital budget formulas for over forty years, in textbooks and professional organization workshops and leadership training materials across seven countries, quietly guaranteeing that every shift would start short before a single nurse walked through the door.</p><p><strong>The Error Nobody Fixed</strong></p><p>Robert Wingo, BSN, RN &#8212; a nursing economics researcher and Nursing Economics Fellow &#8212; has spent years trying to get someone in healthcare leadership to look at the numbers.</p><p>Not the staffing numbers. The budget formula itself.</p><p>What he found is not complicated. It is, as he describes it, a middle school math error. A calculation mistake baked so deeply into how hospitals budget for nursing staff that it has become invisible &#8212; accepted as truth, taught as standard, defended with the words that end every accountability conversation in healthcare:</p><p>&#8220;We&#8217;ve always done it this way.&#8221;</p><p>The result of that error is not theoretical. Because of the formula.</p><p>When Robert brought this finding to healthcare leaders &#8212; CNOs, CFOs, the people with the authority to fix it &#8212; the responses he received told the whole story.</p><p>&#8220;We&#8217;ve always done it this way.&#8221;</p><p>And then, the one that should end careers:</p><p>&#8220;Fixing the error would mean we&#8217;d have to hire more nurses. Finance wouldn&#8217;t approve it.&#8221;</p><p>They knew. And they chose the spreadsheet anyway.</p><p><strong>What Starting Behind Actually Feels Like</strong></p><p>It feels heavy and defeating &#8212; and at the exact same time your mind is already running the list. Do one. Do two. Do three. Priorities sorted before you have touched a single patient, because that is what survival on that unit required.</p><p>It feels like knowing before your shift starts that you will not take a lunch break. That your pee breaks better be quick. That you are going to miss something &#8212; and you spend the whole shift hoping it is a pitcher of water that did not get refilled and not a patient who needed pain medication an hour ago.</p><p>That you will chart after everyone else has gone home. That you will spend the drive home replaying the moments where you almost caught everything and the ones where you did not quite get there in time.</p><p>It feels like a moral injury. Not burnout &#8212; that word has been so overused it has lost its weight. Moral injury. The specific damage that comes from being repeatedly placed in situations where you cannot meet your own standard of care, not because you are not good enough, but because the system was never designed to let you be.</p><p>Nurses have been told for decades that the staffing crisis is a supply problem. Not enough nurses. Not enough schools. Not enough pipeline.</p><p>But the nurses did not disappear. Hundreds of thousands of them left the bedside &#8212; not because they stopped caring, but because they did the math and realized the system had never intended to staff adequately. The board told them every morning. The formula guaranteed it every fiscal year.</p><p><strong>The Nurse Who Said No</strong></p><p>A nurse left a comment on my post this week that I have not stopped thinking about.</p><p>She said her preceptor, early in her career, told her not to pick up extra shifts unless she absolutely had to. To protect her energy. When her manager asked if she wanted to be on the unit call list &#8212; the list that generates the 5am phone calls, the apologies and pleas, the guilt that gets weaponized every time a shift goes uncovered &#8212; she said no.</p><p>The manager looked perplexed. And moved on.</p><p>She was never burdened with those calls. She never had to fight the guilt that the system had worked so hard to install in her. One conversation, early enough, changed the entire trajectory of how that nurse was used &#8212; and how she perceived herself as a nurse.</p><p>That is not a staffing solution. One nurse protecting herself does not fix the formula. But it is what self preservation looks like inside a system that was never going to protect her first.</p><p>The 5am call works because nurses answer it. Because somewhere along the way we were taught that saying no to an extra shift means we do not care about our patients. That protecting our own bodies and minds is a betrayal of the profession.</p><p>It is not a betrayal. It is a rational response to a system that starts every shift in deficit and counts on your conscience to make up the difference.</p><p><strong>Who Fixes This</strong></p><p>Robert Wingo said something in the comments of his post that I want to close with because it is the most honest thing I have read about this problem in years.</p><p>&#8220;The leaders who got us into this mess will not be the ones to get us out of it.&#8221;</p><p>He is right. The CNOs and CFOs who looked at the mathematical proof and said finance would not approve the fix are not coming to save the nurses standing in front of that whiteboard at 6:58am calculating whether they can physically do what the board is asking them to do today.</p><p>The fix will come from nurses who understand the numbers well enough to name the problem in the language finance speaks. From researchers like Robert who refuse to stop pushing. The woman who walked the wards at night with a lamp, who turned nursing from domestic service into a profession built on science, accountability, and the radical idea that patients deserved competent care.</p><p>The Department of Justice chose her name deliberately.</p><p>On January 25, 2023, the U.S. Department of Health and Human Services Office of Inspector General and the FBI launched a coordinated multi-state enforcement action against three South Florida nursing schools. Twenty-five defendants charged. Search warrants executed in Florida, Delaware, New York, New Jersey, and Texas.</p><p>The scheme was straightforward and devastating. Between 2016 and 2021 three nursing schools &#8212; Palm Beach School of Nursing in West Palm Beach, Siena College of Health in Lauderhill, and Sacred Heart International Institute in Fort Lauderdale &#8212; sold fake diplomas and transcripts to aspiring nurses who never sat in a classroom. Never completed a clinical rotation. Never touched a patient.</p><p>$10,000 for an LPN diploma. $17,000 for an RN diploma.</p><p>The buyers used those documents to qualify for the NCLEX. Roughly one third of them passed. They obtained licenses. They got jobs. They went to work in hospitals, home health agencies, and long term care facilities across the country &#8212; in New York, New Jersey, Texas, Maryland, Georgia, Ohio, Massachusetts, and Florida.</p><p>More than 7,600 fake credentials. A $114 million scheme.</p><p>The U.S. Attorney for the Southern District of Florida called it what it was: a public safety crisis that eroded trust in the entire healthcare system.</p><p>But here is what the press releases did not say loudly enough.</p><p>This did not start in 2023.</p><p><strong>Drive Straight Across on I-80</strong></p><p>Take I-80 across the state of Florida. Palm Beach County on the east coast. Lee County on the west. Two coasts. Two nursing scandals. One straight line connecting them.</p><p>In 2011 the faculty senate at Edison State College in Fort Myers approved a vote of no confidence in the college president and senior vice president. The allegations were serious &#8212; employment discrimination, inflated administrative salaries, and problems with the accreditation of the nursing program. The college president, Kenneth Walker, who had led the institution for over twenty years, announced plans to resign. The Board of Trustees fired him in January 2012.</p><p>In June 2012 the accrediting agency placed Edison State College on probation. The citation was specific. The institution had demonstrated an inability to show integrity. It lacked adequate full-time faculty. Nursing students had been misled about their program&#8217;s accreditation status. Students had been allowed to substitute electives for core classes and graduate without completing their actual degree requirements.</p><p>Read that again. Nursing students were misled about their program&#8217;s accreditation status.</p><p>Not a minor paperwork issue. Students completed coursework &#8212; paid tuition, showed up, did the work &#8212; toward a degree that was not what they had been promised. Credits that would not transfer. Futures interrupted before they began.</p><p>Probation was lifted in June 2013 after new leadership corrected fourteen cited problems and restored full accreditation.</p><p>Edison went on to rename itself Florida SouthWestern State College in 2014 and rebuild its reputation.</p><p>The nursing students who were caught in the middle? They are still out there somewhere. Carrying credits that may or may not have transferred. Building careers on a foundation that was shaken before it was finished.</p><p>Now cross the state on I-80.</p><p><strong>Palm Beach County. 2017.</strong></p><p>The Florida Board of Nursing revoked the license of Palm Beach School of Nursing. The reason was straightforward &#8212; failing NCLEX pass rates. Standards not met. The school was not producing nurses who could demonstrate minimum competency on the national licensing exam.</p><p>The license was revoked in 2017.</p><p>The school stayed open.</p><p>For six more years.</p><p>During those six years &#8212; from 2016 through 2021 &#8212; the school&#8217;s owner, Johanah Napoleon, and her employees were selling fake diplomas and transcripts to people who wanted to be nurses without going through the process of becoming one. School employees Gail Russ, Cheryl Stanley, Krystal Lopez, and Ricky Riley were all part of the operation. Recruiters brought aspiring nurses from New York and New Jersey. One undercover FBI agent met with a Fort Lauderdale businesswoman in 2021 who offered a Palm Beach School of Nursing degree and NCLEX prep training for $16,000.</p><p>The Florida Board of Nursing had revoked the license four years earlier.</p><p>Nobody shut the door.</p><p><strong>What the FBI Found. What the Courts Did.</strong></p><p>When Operation Nightingale was announced in January 2023 the numbers were staggering. More than 7,600 fake diplomas across three schools. Twenty-five defendants charged in Phase I alone.</p><p>The convictions followed.</p><p>Johanah Napoleon &#8212; owner and president of Palm Beach School of Nursing &#8212; pleaded guilty to conspiracy to commit healthcare fraud and wire fraud. She cooperated with prosecutors and testified against her own employees. She was sentenced to 21 months in federal prison and ordered to forfeit $3.2 million.</p><p>Gail Russ &#8212; the registrar of Palm Beach School of Nursing &#8212; the person whose job it was to maintain the academic records, to issue the diplomas, to certify that students had completed what the documents said they completed &#8212; was convicted at trial. She was sentenced to 78 months in federal prison. Six and a half years. She was ordered to forfeit $861,672.</p><p>Cassandre Jean, who ran a nursing review operation in Brooklyn and recruited students into the scheme, was sentenced to 36 months in prison and ordered to forfeit $4.7 million.</p><p>Vilaire Duroseau, who operated out of New Jersey, received 33 months and forfeited $1.4 million.</p><p>School employees Cheryl Stanley, Krystal Lopez, and Ricky Riley &#8212; all convicted.</p><p>Let those numbers sink in.</p><p>$4.7 million. $1.4 million. $861,672.</p><p>That is not pocket change. That is not a side hustle that got out of hand.</p><p>That is life-altering money. Generational money. Not to the one who drove home at 7am replaying everything she almost caught in time. Not to the one working a second job because $30 an hour does not pay rent in South Florida.</p><p>That money went to people who sold nursing&#8217;s most sacred credential like a product on a shelf.</p><p>In total 27 defendants were charged and convicted in 2023 in Phase I of Operation Nightingale.</p><p>In September 2025 the DOJ announced Phase II. Twelve more defendants charged. More schools implicated. More fake credentials. The scheme, it turned out, extended well beyond the original three schools. Investigators identified diploma mills operating out of Boca Raton, Delray Beach, Plantation, Miami, North Miami, and Orlando.</p><p>The scheme is still unwinding.</p><p><strong>Who Paid the Real Price</strong></p><p>The defendants will serve their sentences. The money will be forfeited. The schools are closed.</p><p>But the story of who actually paid the price for what happened in Florida is more complicated than the criminal docket suggests.</p><p>Many of the people who purchased fake diplomas were not sophisticated fraudsters. They were women &#8212; disproportionately women of color, many of them single mothers &#8212; who were aggressively recruited by schools that marketed a faster path to a nursing career. They were told the school was accredited. They believed it. Some had completed legitimate LPN training and wanted to advance to RN. Some had attended classes, done real work, and trusted that the institution they were paying was real.</p><p>When Operation Nightingale broke, their licenses were flagged in the national nursing database. State boards began investigating. Some received letters demanding they surrender their licenses immediately. Others lost jobs before any determination of individual culpability was made.</p><p>Legitimate nurses were being swept up alongside fraudulent ones. The process of separating them &#8212; with schools closed and records inaccessible &#8212; was and remains extraordinarily difficult.</p><p>A former Palm Beach School of Nursing student told investigators: they are monsters because you have to have clinical experience to give medications, to start an IV &#8212; you can kill somebody.</p><p>She was right. And she had been defrauded by the same institution she was condemning.</p><p>The nurses who spent years earning real credentials &#8212; who did the clinicals, passed the coursework, sat for boards legitimately &#8212; watched their profession&#8217;s public trust erode because a school in West Palm Beach treated their license as a product to be sold.</p><p><strong>The Question Nobody Is Asking Loudly Enough</strong></p><p>Florida declared a nursing shortage.</p><p>Florida responded by building more schools. Loosening oversight of private nursing education. Creating faster pathways to licensure. Expanding capacity.</p><p>The stated goal was to fill the pipeline.</p><p>The actual result &#8212; at least in part &#8212; was a system so focused on enrollment numbers and revenue generation that schools with revoked licenses stayed open for years, that nursing students at a public college were misled about their accreditation status, that 7,600 fake diplomas found their way into the hands of people who then sat for NCLEX and went to work in hospitals.</p><p>The Florida Nurses Association&#8217;s Willa Hill said it plainly after Phase II was announced in September 2025: she is pushing for tougher laws requiring more stringent guidelines for private nursing schools to operate. The frontline of defense, she said, needs to be highly educated and highly qualified.</p><p>She is right. And she is saying it in 2025 about a problem that was visible in Lee County in 2011 and in Palm Beach County in 2017 and in federal court in 2023.</p><p>The neon lights were flashing for over a decade.</p><p>Two counties. One highway connecting them. And a system that kept looking the other way until the FBI stopped looking away.</p><p>We did not have a nursing shortage.</p><p>We had a shortage of people willing to ask who was getting rich while we wrung our hands about the pipeline.</p><p>When did nursing become something you could sell?</p><p><em>If this is the conversation nursing has been waiting for &#8212; share it with every nurse you know. Right?</p><p>That is the story we told ourselves. And for a while it looked like it was working. More programs. More seats. More graduates entering the pipeline.</p><p>But a pipeline is only as good as what comes out the other end.</p><p><strong>The shortage is real. The average registered nurse in the United States is now 46 years old. Over one million RNs are projected to retire by 2030. We are not just short on nurses today &#8212; we are watching the workforce that trained the current generation walk out the door, taking decades of clinical knowledge with them.</p><p>Nearly 40% of the nurses still working intend to leave the profession by 2029. The average hospital turnover rate is 16.4% &#8212; costing facilities between $3.9 million and $5.7 million every year in recruitment, orientation, and lost institutional knowledge. The same institutions that will not fund retention programs will spend millions replacing the nurses they lost.</p><p>And we are turning away over 80,000 qualified nursing school applicants annually. Not because they aren&#8217;t good enough. Because we don&#8217;t have enough faculty to teach them.</p><p>A nursing administrator I know in New Hampshire told me their program has a long waiting list. Not enough instructors to open more seats. And the students who do get in must maintain a B+ average or they are automatically dropped from the program.</p><p>B+ or you&#8217;re out.</p><p>Meanwhile hospitals are posting thousands of unfilled positions and administrators are mandating overtime on nurses who are already running on empty.</p><p>We have a supply problem and a quality control problem happening at the same time. And we responded to both by making it easier to open nursing schools.</p><p><strong>The for-profit model.</strong></p><p>When the nursing shortage became impossible to ignore the for-profit education sector saw an opportunity.</p><p>More seats. Flexible schedules. Faster paths to licensure. Lower barriers to entry.</p><p>On paper it looked like a solution. In practice it created a new problem.</p><p>Private for-profit nursing schools consistently produce lower first-time NCLEX pass rates than public universities and community colleges. A peer-reviewed study published in the Journal of Nursing Regulation in 2025 confirmed what many in the profession already suspected &#8212; ownership status significantly predicts NCLEX outcomes across every program level.</p><p>For-profit schools have more seats, more flexible schedules, and cost two to five times more than community colleges. They are more expensive and less likely to get you licensed.</p><p>The schools with the most seats are producing the fewest licensed nurses.</p><p>Which brings us to the question nobody in a position of power is asking out loud.</p><p>Are we solving the nursing shortage &#8212; or are we selling desperate people an expensive degree that is less likely to get them licensed?</p><p><strong>The transparency problem.</strong></p><p>NCLEX pass rates are publicly available. The National Council of State Boards of Nursing publishes them. Some states like California legally require schools to disclose them. Most states do not.</p><p>For-profit schools are not required to prominently advertise their pass rates. They advertise flexible schedules, more seats, and faster paths to licensure. The data exists &#8212; buried in state board dashboards and PDF reports that most 22-year-olds choosing a nursing school have never heard of.</p><p>Nobody hands a prospective nursing student a sheet that says &#8212; before you sign anything, look up this school&#8217;s NCLEX pass rate.</p><p>Nobody tells them to look.</p><p class="button-wrapper" data-attrs="{&quot;url&quot;:&quot;https://monicawoodwardluberto.substack.com/subscribe?&quot;,&quot;text&quot;:&quot;Subscribe now&quot;,&quot;action&quot;:null,&quot;class&quot;:null}" data-component-name="ButtonCreateButton"><a class="button primary" href="/__u/monicawoodwardluberto.substack.com/subscribe"><span>Subscribe now</span></a></p><p>So they don&#8217;t.</p><p><strong>Florida is the case study.</strong></p><p>In 2009 Florida eased regulations to invite new nursing programs into the market. Within five years the number of programs more than doubled. Most of the new entrants were for-profit institutions.</p><p>Florida now ranks 52nd out of 55 jurisdictions in first-time NCLEX pass rates. The national average is 91.2%. Florida sits at 84.9%. More than 70 programs across the state posted pass rates 10 or more points below the national average in 2025.</p><p>One school in suburban Miami produced over 2,300 graduates in a decade. Only 13% passed their boards on the first attempt. That school charges $27,000 a year.</p><p>Those graduates have debt. They have degrees. They cannot legally practice nursing.</p><p>To retake the boards they must wait 45 days minimum and pay $200 each attempt. Fail three times in Florida and the state requires a mandatory remedial course &#8212; at their own expense &#8212; before they can try again.</p><p>The school already has their money.</p><p>The Florida Nurses Association executive director said it plainly. Predatory operators realized that a lot of people want to be nurses.</p><p>&#8220;It&#8217;s a cash cow.&#8221;</p><p>Legislation to fix it passed the Florida House nearly unanimously in January 2026. It died in the Senate. This was the third year in a row reform efforts failed to cross the finish line.</p><p>Florida still needs 60,000 more nurses by 2035.</p><p><strong>The question nobody is asking.</strong></p><p class="button-wrapper" data-attrs="{&quot;url&quot;:&quot;https://monicawoodwardluberto.substack.com/subscribe?&quot;,&quot;text&quot;:&quot;Subscribe now&quot;,&quot;action&quot;:null,&quot;class&quot;:null}" data-component-name="ButtonCreateButton"><a class="button primary" href="/__u/monicawoodwardluberto.substack.com/subscribe"><span>Subscribe now</span></a></p><p>We talk about the nursing shortage as though it is a supply problem. Not enough people entering the pipeline.</p><p>But what if the pipeline itself is the problem?</p><p>What if we are not short on people who want to be nurses &#8212; we are short on systems that actually get them licensed, supported, and retained once they are?</p><p>We are turning away qualified applicants because we don&#8217;t have enough faculty.</p><p>We are dropping enrolled students who fall below a B+ average.</p><p>We are graduating students from programs that leave 87% of them unable to pass their boards.</p><p>And we are mandating overtime on the nurses who survived all of it &#8212; burning through the workforce we do have while pretending the problem is just a numbers game.</p><p>This is not a staffing problem.</p><p>It is a values problem.</p><p>And until the people making decisions about nursing education and nursing retention have actually stood at a bedside &#8212; the shortage will keep getting worse no matter how many new programs we approve.</p><div><hr></div><p><em>The Clinical Edge Brief publishes every Monday and Wednesday. Thousand-bed hospital. Day shift was never guaranteed &#8212; management could pull you to nights any time census and staffing demanded it.</p><p>After orientation I could have fought for days.</p><p>I didn&#8217;t, I stayed.</p><p>Not because of the differential. Because of the team. Because at 3am nobody rounds. Nobody schedules a family meeting at midnight. Nobody stops by to manage your documentation.</p><p>At 3am you are just a nurse doing the job.</p><p>And for a lot of us that was the only shift where that was still true.</p><p>That is not a staffing preference. Corporate standard. The customer who has a minor complaint but says nothing. Then another disappointment. Then another. By the third or fourth they quietly stop returning. No complaint filed. No feedback given. They simply disappear.</p><p>The entire session was built around that logic.</p><p>Keep the customer satisfied so they choose you again next time.</p><p>It is a model that has been quietly overtaking healthcare for years.</p><p>Because somewhere between the mission statement and the board meeting, the patient became a revenue unit. The nurse became a service rep. And care became a transaction rated on a five-point scale.</p><p>I sat there looking around the room at nurses who had spent the night before managing septic patients, running codes, holding the hands of people who were dying alone.</p><p>And I kept thinking about that title: &#8220;The customer who doesn&#8217;t come back.&#8221;</p><p>In a hospital that customer didn&#8217;t have a bad experience.</p><p>That customer died.</p><p>Or they went home with a diagnosis that changed everything.</p><p>Nurses are not servants. They are not service staff. They are highly trained, highly skilled clinicians.</p><p>Press Ganey doesn&#8217;t measure care. It measures satisfaction. Satisfaction and safety and healthcare are not the same thing.</p><p>Patients are not customers. They are not guests.</p><p>They are the whole reason nurses are there. Behind every course you complete for relicensure is a writer &#8212; someone who researched the topic, organized the material, and produced the clinical content you are required to know.</p><p>Here is what the industry does not advertise.</p><p>A significant portion of that content is being written by nurses who no longer hold active licenses. Some stepped away from the bedside years ago. Some allowed their licenses to lapse entirely. Others and worse, are non-clinical writers with no real-world patient care experience &#8212; people who can research a topic and produce grammatically correct sentences but have never stood at a bedside, never run a code, never made a split-second clinical decision with a patient&#8217;s life in the balance.</p><p>They know what the textbook says.</p><p>They do not know what actually happens at 3am when the textbook does not apply.</p><div><hr></div><h2><strong>Why This Matters More Than You Think</strong></h2><p>Continuing education is not trivia. It is not a box to check.</p><p>It is the mechanism by which your clinical knowledge is supposed to stay current. The 2025 AHA resuscitation guidelines. Updated pharmacology protocols. Evolving best practices in wound care, behavioral health, geriatric management. You are expected to learn this material, retain it, and apply it at the bedside.</p><p>When the person who wrote that content has not been at a bedside in ten years &#8212; or ever &#8212; the gap between what is written and what is real can be significant.</p><p>Not just inaccurate. Clinically significant.</p><p>The nurse reading that content does not know the difference. There is no requirement for real-world clinical experience. No verification process. No accountability to a board.</p><p>The accreditation process evaluates the content for educational structure, learning objectives, and reference citations.</p><p>It does not ask who wrote it or whether they have ever used what they are teaching.</p><p><em><strong>That is the gap.</strong></em></p><div><hr></div><h2><strong>What Good Looks Like</strong></h2><p>The fix is not complicated.</p><p>Vetted nurses. Active licenses. Real-world clinical experience in the specialty they are writing about.</p><p>Not retired. Not lapsed. Not adjacent.</p><p>Active. Licensed. Currently practicing or recently separated from practice with verifiable credentials.</p><p>That standard exists in other high-stakes industries. A pilot cannot train other pilots without current flight hours. A surgeon cannot teach surgical technique without current OR privileges.</p><p>Nursing continuing education deserves the same standard.</p><p>The content that nurses stake their licenses on &#8212; and their patients&#8217; safety on &#8212; should be written by nurses who are still in it. Who know what current practice actually looks like. Who would recognize an error because they have lived the alternative.</p><div><hr></div><h2><strong>What Clinical Verification Actually Means</strong></h2><p>I developed a clinical verification network for exactly this reason.</p><p>Every piece of content I produce is reviewed by actively practicing clinicians before submission &#8212; an ER/trauma RN, a hospice RN, a VA healthcare RN, an EMT, a crisis intervention specialist. Not former practitioners. Not consultants. People who clocked in last week.</p><p>The result is a 90% first-submission approval rate with the CEU companies I work with.</p><p>That number is not about my writing ability.</p><p>It is about clinical accuracy. This is documented.</p><p>According to AARP&#8217;s most recent research, 46% of adults between the ages of 45 and 59 report feeling lonely. Nearly half. And the biggest predictors are not what most people expect. It is not age. It is not illness. That is a description of a nurse on her day off.</p><p>The loneliness that so many nurses carry is not a personal failure. It is the predictable outcome of a profession that selects for givers, rewards self-sacrifice, and never once asks &#8212; who is taking care of you?</p><p><strong>Why Nursing Makes It Worse</strong></p><p>Every profession has stress. Every career asks something of you.</p><p>But nursing asks for something different. Nursing asks for you.</p><p>From the first day of clinical rotation you were taught to put the patient first. To set aside your own discomfort, your own fear, your own needs. Your own faith. Your own political beliefs. Your own opinions about how things should be done. Professionalism demanded it. The code of ethics required it. And if you forgot &#8212; there was always someone ready to remind you that your personal views had no place at the bedside.</p><p>But nobody told you what happens after 20 or 30 or 40 years of practicing that skill outside the hospital too.</p><p>You bring it home. You bring it to your marriage, your children, your aging parents, your neighbors, your church, your community. You become the person everyone calls because you always show up. You always know what to do. You never fall apart.</p><p>And somewhere in the middle of all of that giving &#8212; the friendships that require reciprocity, the relationships that need you to show up as a whole person and not just a caregiver &#8212; those quietly disappear.</p><p>Not because you are a bad friend.</p><p>Because you were trained to be something other than a friend. You were trained to be a provider. And providers do not ask for help. They give it.</p><p>Until there is nothing left to give.</p><p><strong>When Tuesday Looks Like a Disaster</strong></p><p>I still remember the first week of September 2004. Hurricane Frances had just hit Florida and Hurricane Jeanne was three weeks behind it.</p><p>The entire hospital ran on generator power for five days. No air conditioning. No guaranteed water. Power was rationed to keep the critically ill alive.</p><p>I cared for my patients in T-shirts, shorts and sneakers in South Florida heat. While I was at the bedside, my husband stood in line in 90 plus degree temperatures filling gas cans to keep our home running.</p><p>Nobody asked us to stop. So we didn&#8217;t. We just kept lifting others, holding up everyone else.</p><p>That was nursing. I am talking about something specific and real that plays out in nursing careers every single day.</p><p>I have watched nurses retire with a cake in the break room and nobody to call the next day.</p><p>I have watched nurses work night shifts in their 70s with adult children at home and no social safety net.</p><p>I have watched nurses spend every holiday at the bedside of someone else&#8217;s family while their own sat at a table with an empty chair.</p><p>I have watched nurses give decades of compassion to strangers and have nobody show up when it was their turn to need it.</p><p>Coworkers always remained coworkers. Never deep lasting friends.</p><p>Because the relationships formed under fluorescent lights at 3am during a code are intense and real &#8212; but they are built around crisis not connection. When the shift ends you go home. When one of you leaves the unit the relationship fades. There is no foundation underneath it built from choosing each other outside of obligation.</p><p>We are so good at taking care of everyone else. Patients. Husbands. Parents. Children.</p><p>But we are terrible at letting anyone take care of us.</p><p>And we are even worse at admitting we are lonely.</p><p><strong>You Cannot Pour From an Empty Cup &#8212; But You Already Knew That</strong></p><p>We say it to patients. We say it to new nurses. We put it on motivational posters in break rooms across every hospital in America.</p><p>And then we go back to ignoring it completely.</p><p>So let me say something different. Something more honest.</p><p>Building friendship after decades of giving everything away is hard. It does not happen by accident at this stage of life. It requires the same intentionality you brought to your clinical skills &#8212; deliberate, practiced, and repeated until it becomes second nature.</p><p>Here is what that actually looks like:</p><p>It means saying no to one more obligation so you have energy left for something that feeds you. It means remembering that &#8220;No&#8221; is a complete sentence.</p><p>It requires no explanation, no apology, and no follow up.</p><p>It means finding people who understand what you have lived &#8212; not just colleagues but community.</p><p>Other nurses who have walked away from the bedside.</p><p>Other women in their 50s and 60s who are building something new.</p><p>It means being willing to need something from another person and saying so out loud. That last part is the hardest for us. We were trained out of it.</p><p>It means showing up for yourself the way you showed up for every patient, every family member, every coworker who needed one more thing before the shift ended.</p><p>You were always that someone for everyone else.</p><p>It is time to be that someone for yourself.</p><p>The loneliness that so many nurses carry is not a life sentence.
