But without a laced shoe locking your foot in place, your foot works overtime to stabilize itself with every step. Do that for years and you&#8217;ve trained instability into your gait without knowing it.</p><p><strong>2. You sleep with your toes pointed down.</strong> Side and stomach sleepers do this automatically. Hours a night, every night, your plantar fascia sits shortened. It&#8217;s one of the most common hidden contributors to heel pain I see.</p><p><strong>3. You can&#8217;t isolate your big toe.</strong> Try lifting just your big toe while the other four stay flat on the floor. Most people can&#8217;t do it cleanly. Your habits are causing the injury for the other 23.</p><p>I see this every week. Someone walks in with heel pain, convinced something happened. A run. A new pair of shoes. A bad step off a curb. They want a moment to point to.</p><p>There isn&#8217;t one.</p><p>Plantar fasciitis is rarely an event. It&#8217;s a pattern. And the pattern is happening while you sleep, sit, and walk around all day without thinking about your feet at all.</p><p>Here&#8217;s what I actually find when I dig into a patient&#8217;s 23 hours:</p><p><strong>They sleep with their toes pointed down.</strong> Stomach sleepers and side sleepers do this by default. All night, every night, the plantar fascia sits in a shortened position. Multiply that by thousands of hours over years.</p><p><strong>Their sheets are tucked in.</strong> Sounds harmless. It&#8217;s not. A tight top sheet forces the foot into plantar flexion for eight hours. Same mechanism as pointing your toes on purpose, except you&#8217;re unconscious for it.</p><p><strong>They mouth breathe at night.</strong> This one surprises people. Mouth breathing changes sleep posture and muscle tone throughout the body, foot included. It&#8217;s not the direct cause, but it&#8217;s part of the pattern I look for.</p><p><strong>Their heels never touch the ground during the day.</strong> Heels, barstools, standing desks with a rail, cars. Modern life is built to keep your heel elevated. Your fascia adapts to whatever position you give it most.</p><p><strong>Their shoes don&#8217;t tie, or they don&#8217;t tie them.</strong> Slip-ons feel efficient. They also let your foot move inside the shoe with zero stability, so your fascia does stabilization work it was never built to do alone.</p><p><strong>They can&#8217;t dissociate their big toe from the other four.</strong> Try it right now. Lift just your big toe, keep the other four flat. If you can&#8217;t, you&#8217;ve lost a control pattern that matters for how load moves through your foot with every step.</p><p><strong>They can&#8217;t balance on one leg for 30 seconds.</strong> This is my go-to test. It&#8217;s not about your foot. It&#8217;s about whether your nervous system has the proprioceptive control to protect your foot under load. Most people fail this and have no idea.</p><p>None of this is random. Every one of these is a 23-hour habit hiding behind a 1-hour symptom.</p><p>This is the whole idea behind the 23-Hour Advantage&#8482;. The hour you spend with a clinician matters. Layer on some stress, a constant stream of Slack notifications, and an overflowing inbox, and by 2:00 PM, your nervous system finally sends a notification: <em>&#8220;Hey. We&#8217;ve been running the exact same loop for six hours straight.&#8221;</em></p><h3>The Optimization Paradox</h3><p>I see this paradox most often with founders, executives, and high-performers. They optimize absolutely everything in their lives: their AI stack, their supplement routines, their morning habits, their sleep trackers, and their calendar systems.</p><p>Then they sit across from me and say, <em>&#8220;My neck has been killing me for two years.&#8221;</em></p><p>It&#8217;s fascinating because they have meticulously optimized every single system <em>around</em> themselves, except for the one running all of it. But you can only ignore operating system updates for so long before things lag, glitch, and eventually crash.</p><p>When that crash happens, we tend to look in the wrong place for answers. Symptoms are terrible detectives. They show up late to the crime scene, they blame the wrong hour, and they point fingers at standard scapegoats like &#8220;stress&#8221; or &#8220;bad posture.&#8221;</p><p>In reality, the root cause is almost never a random malfunction. It&#8217;s usually hiding somewhere in the inputs we never bother to audit.</p><h3>Moving Beyond Factory Settings</h3><p>I don&#8217;t believe most people have broken bodies. And yet chronic pain is at an all-time high.</p><p>Nobody is asking the obvious question: what are you doing with the other 23 hours?</p><p>Because here&#8217;s the math that nobody in medicine wants to talk about. You spend one hour in therapy. Maybe two. Then you go home and you spend the next 23 hours undoing every single thing we just did. The way you sleep. The way you sit. The way you collapse onto the couch after work. The way you rest, move, commute, and wind down. That&#8217;s not background noise. That&#8217;s the treatment. And most people are failing it without knowing it.</p><p>This is why people don&#8217;t get better. Not because medicine has failed them. Because they went home and kept doing what they were doing.</p><p></p><p><strong>Your couch is not your friend.</strong></p><p>I say this with love: the couch is one of the most dangerous pieces of furniture in your home. It is specifically designed for comfort and specifically terrible for your body. The cushions are too soft, the back too reclined, the armrest too high or too low for your neck. People spend hours there &#8212; half-sitting, half-lying, twisted toward a screen &#8212; and then wonder why their back hurts in the morning.</p><p>The bed isn&#8217;t much better if you&#8217;re using it wrong. And most people are.</p><p></p><p><strong>We&#8217;re blaming our genes when we should be looking in the mirror.</strong></p><p>Here&#8217;s something I&#8217;ve observed after a decade of practice that nobody wants to hear: we don&#8217;t inherit our pain. We inherit our habits. You sleep the way your parents slept. You sit the way they sat. You rest the way they rested. And when you develop the same back problems, the same migraines, the same neck issues they had &#8212; we call it genetics.</p><p>It&#8217;s not genetics. It&#8217;s a learned position, repeated ten thousand times, finally reaching its tipping point.</p><p>Your foundation has been cracked for years. Every bad night of sleep, every hour on the wrong couch, every drive without lumbar support &#8212; that&#8217;s not neutral. That&#8217;s one step closer to the edge. Pain doesn&#8217;t arrive randomly. It arrives when the accumulation finally wins.</p><p></p><p><strong>Everyone is obsessed with quantity of sleep. Quality is what actually matters.</strong></p><p>Eight hours in a broken position isn&#8217;t rest. It&#8217;s an eight-hour passive injury. And I can tell you exactly what those injuries look like, because I see them walk into my clinic every week.</p><p>The person with chronic migraines and tension headaches who has tried everything? They&#8217;re sleeping on two pillows. The head propped forward all night, compressing the base of the skull, triggering the same headache cycle &#8212; every single night &#8212; that no medication has been able to break.</p><p>The person going to the ER for neck pain? Hunching forward all day, then going home and sleeping with their cervical spine in the same flexed position for eight hours. I could never fully fix a stomach sleeper. I cannot overcome eight hours of cervical compression caused by double-pillow stacking. These aren&#8217;t limitations of treatment. They&#8217;re physics.</p><p></p><p><strong>Try the 4-pillow method tonight.</strong></p><p>If you&#8217;re a side sleeper and most people are this could change your life by this time tomorrow.</p><p>One pillow under your head: just one, filling the space between your ear and the mattress. One pillow hugged against your chest to support the top shoulder and stop it from rolling forward. One long pillow between your knees running all the way down to your ankles (not just the knees, the full leg) keeping your pelvis neutral. And one pillow at your back for support if you need it.</p><p>That&#8217;s it. No appointment. No prescription. No specialist referral. Or a tight lower back. Or that dull ache between your shoulder blades that takes until noon to shake.</p><p>You assumed it was stress. Or aging. Or just&#8230; how mornings feel now.</p><p>But here&#8217;s what I need you to hear:</p><p><strong>Your body just spent 7&#8211;9 hours loaded into one position. Whatever that position was &#8212; it was either healing you or hurting you. There is no neutral.</strong></p><p>This is one of the highest-leverage 23-hour habits I address with clients. And almost no one talks about it.</p><p>Let&#8217;s fix that.</p><h2>The three sleep positions and what they&#8217;re actually doing</h2><h3>&#128308; Stomach sleeping: the one I wish I could ban</h3><p>I know. You love it. It feels cozy. I&#8217;m sorry.</p><p>Sleeping on your stomach forces your neck into full rotation for hours at a time &#8212; typically 4+ hours on the same side. That&#8217;s a sustained end-range stretch on your cervical joints, muscles, and nerves.</p><p>If you wake up with neck pain, headaches, or numbness/tingling into your arms &#8212; stomach sleeping is a prime suspect.</p><p>It also puts your lumbar spine into extension and compresses the small joints in your lower back all night long. For anyone with disc issues, stenosis, or chronic low back pain, this is gasoline on a fire.</p><p><strong>The fix:</strong> This is the hardest habit to break because it&#8217;s deeply ingrained. Start by placing a pillow under one hip and one side of your chest &#8212; it makes full stomach sleeping less comfortable and begins training your body toward side lying. It takes weeks, not days. Be patient.</p><h3>&#128993; Back sleeping: great in theory, complicated in practice</h3><p>Back sleeping is biomechanically sound <em>if</em> your pillow and mattress are doing their job.</p><p>The goal is neutral spine &#8212; meaning your head isn&#8217;t pushed too far forward by a thick pillow, and your lower back isn&#8217;t arching excessively off the mattress.</p><p><strong>Where it goes wrong:</strong></p><ul><li><p>Pillow too thick &#8594; your chin juts toward your chest &#8594; sustained cervical flexion all night &#8594; neck pain, upper trap tension, headaches</p></li><li><p>No support under the knees &#8594; your hip flexors stay in mild stretch all night &#8594; morning low back stiffness</p></li><li><p>Mattress too soft &#8594; your hips sink, your lumbar spine loses support &#8594; chronic low back pain</p></li></ul><p><strong>The fix:</strong> One pillow, not two. Thin to medium loft. A pillow or rolled towel under your knees changes everything for low back pain sufferers &#8212; try it tonight.</p><h3>&#128994; Side sleeping: the best option, still easy to mess up</h3><p>Side sleeping is generally the most supportive position for your spine &#8212; <em>when done right.</em></p><p><strong>Where it goes wrong:</strong></p><ul><li><p>Pillow too thin &#8594; your top shoulder collapses inward, your neck bends laterally all night &#8594; neck and shoulder pain</p></li><li><p>No pillow between knees &#8594; your top hip internally rotates, pulling on your SI joint and piriformis &#8594; hip and low back pain</p></li><li><p>Fetal position (knees pulled tight to chest) &#8594; sustained lumbar flexion &#8594; morning stiffness, disc irritation over time</p></li><li><p>Sleeping on the same side every night &#8594; asymmetrical loading, rib tightness, shoulder compression</p></li></ul><p><strong>The fix:</strong> Your pillow should fill the full space between your ear and the mattress &#8212; enough to keep your head neutral, not tilted. Add a pillow between your knees (it doesn&#8217;t have to be fancy &#8212; a regular pillow works). need the most expensive mattress. You need one that:</p><ul><li><p>Keeps your spine in neutral when side lying (not sinking, not propped up)</p></li><li><p>Doesn&#8217;t create pressure points at your hip and shoulder</p></li><li><p>Isn&#8217;t so firm that your shoulder has nowhere to go when you&#8217;re on your side</p></li></ul><p>General rule: if your mattress is over 7&#8211;8 years old and you have chronic pain, it has earned a serious look. Most people hold onto mattresses far too long because they don&#8217;t connect the dots between their bed and their body.</p><h2>The pillow problem</h2><p>Your pillow should match your sleep position.</p><ul><li><p><strong>Back sleepers:</strong> low to medium loft, soft to medium fill (maybe no pillow)</p></li><li><p><strong>Side sleepers:</strong> medium to high loft, firmer fill to support the head-to-shoulder gap</p></li><li><p><strong>Stomach sleepers:</strong> the thinnest pillow possible &#8212; or none &#8212; to minimize neck rotation</p></li></ul><p>Most people are using the wrong pillow for their position. This alone can account for chronic neck pain, morning headaches, and upper back tightness that no amount of stretching touches.</p><h2>What to do tonight</h2><p>Pick one thing from this list and try it:</p><ol><li><p>If you&#8217;re a back sleeper &#8212; place a pillow under your knees before you fall asleep</p></li><li><p>If you&#8217;re a side sleeper &#8212; add a pillow between your knees (FULL LENGTH FROM THIGHS TO TOES)</p></li><li><p>If you&#8217;re a stomach sleeper &#8212; put a pillow under one leg as a starting point </p></li><li><p>Check your pillow loft: does your head look neutral in a mirror, or is it tilted toward or away from the mattress?</p></li><li><p>Notice what side you wake up on &#8212; that&#8217;s likely your dominant side. Start consciously alternating.</p></li></ol><p>You spend roughly <strong>a third of your life</strong> in your sleep position.</p><p>More time than you spend at your desk. More time than you spend exercising. More time than almost any other single posture in your day.</p><p>It is not a small variable.</p><p><strong>#NothingIsRandom</strong> &#8212; not your morning stiffness, not your neck pain, not the ache that never quite goes away. has to lose.</p><p>If your hands are in a good position, the screen is too low.</p><p>If the screen is high enough, your hands are too high.</p><p>So most people spend hours every day in what I call gremlin mode:</p><p>Head forward.</p><p>Shoulders rounded.</p><p>Elbows floating.</p><p>Wrists bent.</p><p>Spine curved.</p><p>And then one day:</p><p>&#8220;My neck always hurts.&#8221;</p><p>&#8220;My shoulders are so tight.&#8221;</p><p>&#8220;I think I slept wrong.&#8221;</p><p>You probably did not sleep wrong.</p><p>You worked wrong.</p><p>For six hours.</p><p>With your body pretending it was okay.</p><p>The body is incredibly good at adapting.</p><p>That is actually the problem.</p><p>It adapts until it cannot anymore.</p><p>Then it sends pain.</p><p>Not because it is broken.</p><p>Because it is tired of compensating.</p><p>The fix is simple.</p><p>Raise the screen.</p><p>Separate the keyboard.</p><p>Let your elbows drop naturally by your sides.</p><p>Get the top of the screen slightly above eye level.</p><p>Because your body should not have to collapse just to answer emails.</p><p>I have seen people completely change their neck pain, headaches, and shoulder tension just by changing their laptop setup.</p><p>Not because they got stronger.</p><p>Because they stopped practicing the position causing the problem.</p><p>Your body becomes whatever position it repeats most.</p><p>And if you spend six hours a day folded over a laptop...</p><p>Eventually your body starts to think folded is normal.</p><p>Because pain is not stopped golfing for three months because he was convinced he had finally &#8220;thrown his back out.&#8221;</p><p>Nothing changed.</p><p>His pain was always the same.</p><p>Low back pain on the right side.</p><p>Tight hip.</p><p>Stiffness getting out of the car.</p><p>Sciatica that came and went.</p><p>He walked into my office and sat down.</p><p>Or at least he tried to.</p><p>Because before he clue.</p><p>Because pain is not random.</p><p>It is patterns.</p><p>And sometimes the thing making you crooked is sitting in your pocket right the time, it does not run in the family.</p><p>It runs in the couch.</p><p>I went to a client&#8217;s home last year.</p><p>Marcus had been dealing with back pain for six years.</p><p>Before I could ask much, he shrugged.</p><p>&#8220;It runs in the family. My dad has it. My grandfather had it.&#8221;</p><p>I asked if he grew up in the same house as his dad.</p><p>He laughed.</p><p>&#8220;Same house. Same couch actually.&#8221;</p><p>I asked him to sit the way he normally would.</p><p>He sank into the left corner.</p><p>Right leg crossed over left.</p><p>Left hip dropped.</p><p>Body rotated toward the TV.</p><p>One elbow on the armrest.</p><p>&#8220;Is that how your dad sits?&#8221;</p><p>Long pause.</p><p>&#8220;Yeah. Nothing happened. No heavy lifting. No awkward movement. No incident you can point to.</p><p>So you blamed the mattress. Or you figured it was stress. Or you decided your back just &#8220;does this&#8221; sometimes, for no reason.</p><p>Here&#8217;s what actually happened.</p><p>Your back pain didn&#8217;t start Monday morning. It started Saturday afternoon on your couch. Your body just didn&#8217;t send you the invoice until two days later.</p><p>This is the thing nobody tells you about pain. And once you understand it, you will never think about your body the same way again.</p><h2>Pain is a lagging indicator</h2><p>Think about the last time you had a really hard workout after a long break.</p><p>Did you feel it immediately?</p><p>No. You felt fine walking out of the gym. Maybe a little pumped. You went home, ate dinner, went to bed feeling pretty good about yourself.</p><p>Then you woke up the next morning and your legs wouldn&#8217;t work properly going down the stairs.</p><p>Or worse &#8212; you woke up feeling fine on day one, and then day two hit you like a truck.</p><p>That&#8217;s called delayed onset muscle soreness. Everyone knows about it in the context of exercise.</p><p>Nobody applies the same logic to pain.</p><p>But the same principle is at work. The body accumulates stress, adapts, compensates &#8212; and then, when it finally runs out of compensation room, it sends the signal. Usually one to three days after the thing that actually caused it.</p><p>By then, you&#8217;ve completely forgotten what you were doing one to three days ago.</p><p>So you blame Monday.</p><p>Monday is innocent.</p><h2>The boy who tracked his pain to the rain</h2><p>I work with a lot of kids through my PainProof practice. Children are actually some of the clearest cases I see &#8212; they haven&#8217;t had decades of chronic pain layered on top of each other, so the patterns are easier to trace.</p><p>Ryan was 13. He had back pain for a few days every month, reliably, predictably.</p><p>He started tracking it.</p><p>&#8220;When it hurt, it was usually a Tuesday or a Wednesday.&#8221;</p><p>When I sat down with him, I didn&#8217;t ask where it hurt. I asked something else.</p><p>&#8220;What did you do over the weekend?&#8221;</p><p>He thought for a second.</p><p>&#8220;Rainy weekend,&#8221; he said. &#8220;Baseball got canceled. I sat on the couch all day.&#8221;</p><p>There it was.</p><p>The clue everyone misses.</p><p>Ryan&#8217;s rainy Saturdays and his Tuesday back pain were the same event &#8212; separated by 48 to 72 hours. The couch marathon would poke the bear on Saturday and Sunday. The bear would wake up angry on Tuesday.</p><p>&#8220;But I&#8217;ve had rainy weekends before,&#8221; Ryan said. &#8220;They never hurt.&#8221;</p><p>That&#8217;s the part that makes the delayed pain theory complicated. It&#8217;s not just about what you do &#8212; it&#8217;s about whether your body currently has enough reserve to absorb it.</p><p>Before, the bear was asleep. Now the bear was already grumpy from other habits. y2="14"></line></svg></button></div></div></div></a></figure></div><p></p><h2>Why you can&#8217;t connect the dots</h2><p>The gap between cause and pain is the whole reason chronic pain is so confusing and so persistent.</p><p>If touching a hot stove caused immediate, localized pain, you would never touch a hot stove again. The feedback loop is instant and obvious. You learn.</p><p>But if sitting on your couch in a collapsed position for three hours on Saturday causes back pain on Tuesday, you never make the connection. The feedback loop is broken. You don&#8217;t learn. You keep doing the thing. The pain keeps coming back. And everyone &#8212; including your doctor &#8212; focuses on Tuesday instead of Saturday.</p><p>I&#8217;ve watched this play out in my practice hundreds of times.</p><p>A client comes to me with &#8220;mystery&#8221; back pain that appears every few weeks for no apparent reason. When we sit down and map the timeline carefully, the pattern emerges. The pain clusters around Tuesdays and Wednesdays. The weekends before those Tuesdays are almost always the same: more screen time, more couch time, less movement, longer periods in compromised positions.</p><p>Not because the weekend caused some catastrophic injury.</p><p>Because the weekend was one too many straws on the camel&#8217;s back.</p><h2>What you&#8217;re actually doing when you rest</h2><p>Here&#8217;s where it gets uncomfortable.</p><p>When pain arrives, the natural instinct is to rest. And rest sounds passive, neutral, healing.</p><p>It isn&#8217;t.</p><p>Rest means something. Whatever position you rest in, your body is practicing that position. Whatever load you put through your joints while resting, your body is responding to that load.</p><p>Most people rest on the couch. Spine curled into a C. Head dropped forward. Hips loaded asymmetrically. For two, three, four hours.</p><p>Then they go to bed. Stomach sleeping with the neck rotated 90 degrees. Or side sleeping with the top hip dropping forward all night without a pillow between the knees.</p><p>They wake up on Monday and their body has spent the entire weekend practicing the exact problem.</p><p>Then they call Monday&#8217;s pain a mystery.</p><p>I keep a mental note of what happens to my patient schedule when it rains on a weekend.</p><p>By Tuesday, Wednesday, Thursday &#8212; I&#8217;m fully booked.</p><p>Not because rain hurts people.</p><p>Because rain keeps people inside, on their couches, in their beds, on their phones &#8212; in positions their bodies weren&#8217;t designed to hold for that many hours. And two to three days later, they feel it.</p><p>The rain didn&#8217;t hurt them.</p><p>The rain changed their habits.</p><p>And the habits did the rest.</p><div immediately.</p><p>Imagine a healthy tree. Strong. Deeply rooted.</p><p>Now imagine someone walks up and swings an axe into the exact same spot every single day.</p><p>Day one &#8212; barely a mark. Day ten &#8212; a visible groove. Day thirty &#8212; the tree starts to lean. Day ninety &#8212; it falls.</p><p>Now imagine instead they swung the axe a little higher each day. Or a little lower. Varying the spot each time.</p><p>The tree would survive indefinitely. The same total force, distributed differently, allows for repair.</p><p>Your spine works exactly like this.</p><p>The problem is never usually that you sit. It&#8217;s that you sit in the exact same position for too many hours, hitting the same spot over and over, without enough variation for your body to repair between sessions.</p><p>Standing desks that people stand at all day are the same problem in a different direction. Same spot. Just vertical.</p><p>The body doesn&#8217;t need a perfect position. It needs variety. Movement. Change. Different spots on the tree.</p><p>&#8220;Your best posture is always your next posture.&#8221;</p><p>This is why I tell people that the goal isn&#8217;t to sit correctly. The goal is to sit differently &#8212; often.</p><h2>The flight that explains everything</h2><p>On a recent flight, I couldn&#8217;t stop watching the people across the aisle from me.</p><p>From the moment we boarded, they were hunched over their phones. Candy cane posture. Head dropped, neck loaded, shoulders rounded forward.</p><p>Three hours later, when we landed, they were in the exact same position.</p><p>They felt fine walking off the plane.</p><p>I knew they wouldn&#8217;t on Wednesday.</p><p>Because that&#8217;s not how pain works. It doesn&#8217;t show up when the damage is happening. It shows up when the compensation runs out.</p><p>Three hours of sustained forward head posture doesn&#8217;t hurt in the moment. But multiply it by the neck being already loaded from the desk, and the night of sleeping in the wrong position before the flight, and the two hours of scrolling at the gate &#8212; and suddenly Wednesday&#8217;s &#8220;mysterious&#8221; headache has a very specific origin story.</p><p>The cause was Tuesday&#8217;s flight.</p><p>The symptom arrived on Wednesday.</p><p>The person blamed Wednesday.</p><h2>How to use this knowledge</h2><p>Once you understand that pain is a lagging indicator, everything shifts.</p><p>You stop asking &#8220;what did I do today?&#8221; and start asking &#8220;what did I do two or three days ago?&#8221;</p><p>You stop treating Monday like the problem and start auditing the weekend.</p><p>You stop resting in positions that compound the problem and start asking what your body is actually practicing while you recover.</p><p>Here are the questions worth sitting with:</p><p><strong>When does your pain tend to show up?</strong> Monday? Tuesday? After a long weekend? After a travel day? After a particularly screen-heavy stretch?</p><p><strong>What happened two to three days before?</strong> Be specific. What surface were you on? For how long? What position? How much did you move versus stay still?</p><p><strong>What does your rest look like?</strong> When you&#8217;re tired or in pain, where do you go? The couch? The bed? Your phone? What position does your body actually spend recovery time in?</p><p><strong>Where are you hitting the same spot on the tree?</strong> What&#8217;s the one position you default to, over and over, for hours at a time? That&#8217;s almost always where to start.</p><h2>The uncomfortable truth</h2><p>Pain has a cause.</p><p>Almost always.</p><p>It&#8217;s just that the cause and the symptom are rarely in the same place at the same time, which makes them almost impossible to connect without someone helping you look backward.</p><p>That&#8217;s the job I love most.</p><p>Not treating Tuesday&#8217;s pain.</p><p>Finding Saturday&#8217;s habit.</p><p>Because once you find it &#8212; really find it &#8212; the pain doesn&#8217;t come back. Not because you got better treatment. But stay with me.</p><p>In the last five years, I&#8217;ve done something most physical therapists never do. Instead of seeing patients in a clinic, I go to them. Their homes. Their bedrooms. Their offices. Their bathrooms.</p><p>And what I find, over and over, is the same thing.</p><p>The answer was hiding somewhere nobody thought to look.</p><p>Here are the five places I find it most often &#8212; and what to do about each one.</p><h2>1. Your bedroom</h2><p>A client messaged me the morning after our session.</p><p>&#8220;I didn&#8217;t wake up in pain for the first time in 15 years.&#8221;</p><p>15 years.</p><p>A week later: &#8220;Day 7. Still no pain. Scared it&#8217;s going to come back.&#8221;</p><p>It didn&#8217;t.</p><p>Here&#8217;s what we changed.</p><p>Her pillow was slightly tilted toward the headboard. Every night. For 15 years. Six to eight hours of that micro-angle loading the same structures, night after night.</p><p>She&#8217;d seen doctors. Tried medications. Changed her mattress twice.</p><p>Nobody had looked at her bedroom. Nobody had asked about her pillow.</p><p>The fix took thirty seconds.</p><p>What to check tonight:</p><p>Look at your pillow. Is it pressing against the headboard or another pillow in a way that could be tilting your head even slightly? If it is, reposition it so it sits flat and nothing nearby can push it out of alignment. One pillow only under your head &#8212; never stacked.</p><p>If you sleep on your side and don&#8217;t have a pillow between your knees, that&#8217;s the second thing to fix. Your top hip drops forward all night without it. I&#8217;ve seen that single change cancel scheduled surgeries.</p><p>Anna was already booked for a hip replacement. She&#8217;d had months of consultations, X-rays, specialist visits. When I came to her home and asked her to show me how she slept, the problem was immediate. Her top hip was dropping forward every night, loading the joint for hours while she slept.</p><p>One pillow between her knees. Two weeks later she called me.</p><p>&#8220;I canceled the surgery. The hip pain is almost gone. I can&#8217;t believe it was my sleeping position all this time.&#8221;</p><p>The fix cost $0. Your back pocket</h2><p>Try something.</p><p>Put a credit card under your foot and stand on it.</p><p>You feel it immediately, right?</p><p>And that&#8217;s with all the muscles between your foot and the ground absorbing some of the unevenness.</p><p>Now imagine sitting on something that thick, directly under your pelvis, for eight hours a day. No muscles to absorb it. Just a constant, subtle tilt feeding directly into your spine.</p><p>That&#8217;s what a wallet in your back pocket does.</p><p>I&#8217;ve treated Uber drivers in San Francisco who had already had spine surgery. When I sat in their cars and watched them drive, the pattern was identical every time. Wallet in the back pocket. Leaning slightly to one side. No pillow between the knees at night.</p><p>The surgery hadn&#8217;t fixed the pain because the habits were still there.</p><p>What to do right now:</p><p>Empty both back pockets before you sit down. Every time. This costs nothing and takes three seconds. Not by what it intends. Not by what it exercises. By what it practices most, hour after hour, year after year.</p><p>Pain is almost always the body&#8217;s way of telling you that something it&#8217;s been practicing has finally crossed a threshold.</p><p>The question to ask yourself:</p><p>What position does your body practice the most each day?</p><p>Not at the gym.</p><p>Not during your workout.</p><p>During the other 23 hours.</p><p>How do you hold your phone?</p><p>How do you sit on the couch?</p><p>What does your body look like at hour seven of a workday?</p><p>What position do you hold for eight hours every night?</p><p>That&#8217;s where the answer usually is.</p><h2>What to do tonight</h2><p>You don&#8217;t need a new chair.</p><p>You don&#8217;t need a new mattress.</p><p>You don&#8217;t need more physical therapy.</p><p>Start with these five things:</p><ol><li><p>Check your pillow. One pillow under your head only. Nothing tilting it.</p></li><li><p>Add a pillow between your knees if you sleep on your side.</p></li><li><p>Empty your back pockets before you sit down.</p></li><li><p>Center your monitor directly in front of you.</p></li><li><p>Notice what your pinky is doing the next time you pick up your phone.</p></li></ol><p>None of these cost money.</p><p>All of them take under two minutes.</p><p>And any one of them might be the thing that&#8217;s been practicing your pain for years without you knowing it.</p><p>I&#8217;m Dr. Ariana Fontenot, DPT &#8212; known as the Rogue PT.
