They have done more for obesity in three years than we managed in thirty, and the benefits run well past the scale.</p><p>Which is why what happened on July 1 deserved more attention. Medicare <a href="https://www.cms.gov/newsroom/press-releases/coming-soon-cms-provide-50-monthly-access-glp-1-medications-medicare-beneficiaries">started covering them for weight loss</a> for the first time, putting them within reach of millions of older adults for $50 a month.</p><p>Read the coverage criteria, and there is something easy to miss. To qualify, the drug has to be prescribed &#8220;<a href="https://www.ncpa.co/pdf/2026/advocacy/medicare-glp-1-bridge-faqs.pdf">in combination with current and ongoing lifestyle modification,</a> including structured nutrition and physical activity.&#8221; Medicare did not forget about exercise. It made exercise a condition of coverage.</p><p>What it did not do is define what that means, say who provides it, check whether it happened, or pay anyone to deliver it. The requirement exists. The thing that would satisfy it does not.</p><h2>The math is different at 72</h2><p>About a quarter of the weight lost on these drugs is <a href="https://dom-pubs.onlinelibrary.wiley.com/doi/10.1111/dom.16275">lean mass rather than fat</a>. That is a real cost at any age. At 72 it is a different kind of cost, because you are spending down a reserve that was already thin, and muscle does not come back on its own.</p><p>Bone goes with it, for a simple reason. Bone maintains itself in response to load, and body weight is load. Take the weight off and the skeleton remodels to the lighter demand. Muscle is the other thing putting load through bone, which is why losing it compounds the problem instead of sitting beside it. One trial set out to test whether semaglutide would build bone. After 52 weeks, it <a href="https://pmc.ncbi.nlm.nih.gov/articles/PMC11087719/">found the opposite</a>: bone breakdown had increased, bone formation had not, and density was lower at both the hip and the spine.</p><p>Some clinicians would argue that lean mass is not the same as muscle, that some of the loss is proportional to the weight coming off, and that the fracture data points both ways. That is all fair, and the honest answer is that nobody yet knows what this costs an older adult over ten years. But the argument does not rest on knowing. Whether the risk turns out to be serious or modest, the protection against it is the same, and in one trial the people who took the drug and did the exercise program <a href="https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2820308">lost no bone density at all</a>.</p><h2>The advice is here, and it&#8217;s the wrong shape</h2><p>The muscle conversation has finally gotten loud, and the advice is everywhere now. Eat more protein. Lift twice a week. Get a baseline DEXA. Watch your calcium and vitamin D. It is in the clinic handouts, the newsletters, and the comment section under every post about these drugs.</p><p>All of it assumes the constraint is knowing, which is seldom the case in American healthcare. The hard part is acting on the knowledge and sticking with it. People do start when their doctor tells them to. Most of them are not still going three months later. Among adults 65 and older, <a href="https://www.cdc.gov/mmwr/volumes/71/wr/mm7118a6.htm">22% of men and 17% of women</a> meet the federal muscle-strengthening guideline, and only <a href="https://www.cdc.gov/nchs/data/nhsr/nhsr215.pdf">13.9% meet both the aerobic and strength guidelines</a>. By 85, that falls to 6.2%.</p><h2>Why &#8220;just lift weights&#8221; fails</h2><p>My mom is 71, motivated, and knows exactly what she is supposed to do. She stopped playing tennis because of tendonitis in her elbow and stopped picking up her grandkids because of the arthritis in her hands. She does not need to be told. She needs help putting what she knows into action, and navigating the pain and discomfort that gets in the way.</p><p>I have spent the last fifteen years designing for behavior change, much of which has been with adults over 50 managing arthritis, osteoporosis, and chronic pain. Three things go wrong, over and over, and <strong>none of them are willpower</strong>.</p><p><strong>The instruction has no first step.</strong> &#8220;Lift twice a week&#8221; means something to a person who can already get down to the floor and back up. To someone whose knees hurt on stairs, it is an aspiration.</p><p><strong>Everything else in their care has taught them to be careful.</strong> An osteoporosis diagnosis arrives with a scan, a script, and a warning. We identify a problem of weakness, counsel caution, and then a few years later ask the same person to load their spine.</p><p><strong>The first three weeks decide it, and nobody is there.</strong> Something will hurt or feel awkward early on. With someone they trust to help them follow through and troubleshoot, they adjust and keep going. Without that, they stop, blame themselves, and do not come back. And there is usually no referral at all. Physical therapy is built around acute episodes, one complaint at a time, and it is not what anyone gets handed when they start a weight loss drug.</p><h2>What has to exist</h2><p>None of this needs new science. It needs someone to be responsible.</p><p><strong>1. Someone has to own muscle.</strong> Right now nobody does. The prescriber owns the prescription and watches the weight come down, which reads as success on every number on the chart.</p><p><strong>2. Someone has to measure capability, not weight.</strong> Grip strength, sit-to-stand, gait speed, balance. All cheap to measure, all better predictors of whether a person keeps living in their own house, and almost nobody starting a GLP-1 has a baseline for any of them.</p><p><strong>3. Someone has to get paid for it.</strong> Remote therapeutic monitoring codes already cover a version of this and mostly go unused. A program that writes physical activity into its coverage criteria should probably also pay for the thing that delivers it, given that <a href="https://www.ajmc.com/view/economic-burden-of-osteoporotic-fractures-in-us-managed-care-enrollees">a hip fracture runs $34,000 to $40,000 in the first year alone</a>.</p><p>We are building a version of this, so I am not a neutral party. What bothers me more is that the GLP-1 companion programs that already exist are sold to employers, which structurally excludes the 72-year-old on Original Medicare who just became eligible in July.</p><h2>Building the other half</h2><p>We spent twenty years trying to get people to lose weight, and we finally have drugs that do it. That was supposed to be the hard part.</p><p>What is left is a problem we already know how to solve. Muscle responds to load at 75 the same way it does at 35, a little slower and just as reliably, and the people who do the work get to keep carrying their own groceries and their own grandkids. The science is not missing. The programs work.</p><p>Medicare wrote physical activity into the coverage criteria in July. That's the opening. Our ability to continue doing the things we love&#8212;whether it's playing with grandchildren, walking with friends, traveling, or exploring new hobbies&#8212;is intrinsically linked to our physical capabilities.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-1" href="#footnote-1" target="_self">1</a></p><p>However, a staggering number of adults over 50 are living with physical health challenges that threaten their ability to stay active. Consider a few sobering statistics:</p><ul><li><p>Over 54 million Americans experience joint pain, a major cause of inactivity. 43.5% of these cases result in arthritis-attributable activity limitations.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-2" href="#footnote-2" target="_self">2</a></p></li><li><p>Muscle mass decreases 3&#8211;8% per decade after age 30 (and more rapidly after 60), yet only 27% of people offset this loss with strength training.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-3" href="#footnote-3" target="_self">3</a></p></li><li><p>53 million Americans have low bone density, significantly increasing their risk of fractures.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-4" href="#footnote-4" target="_self">4</a></p></li><li><p>1 in 3 older adults fall each year, and falls are a leading cause of injury for this age group.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-5" href="#footnote-5" target="_self">5</a></p></li></ul><p>There&#8217;s a vicious cycle at play y2="14"></line></svg></button></div></div></div></a></figure></div><p>This cycle results in an inactive older adult population with a heightened risk of falls, chronic disease, sarcopenia, and difficulty living independently.</p><p>My mom is 69 and very active for her age. But over the last couple of years, I&#8217;ve seen her &#8220;retire&#8221; from activities that historically brought her joy and fulfillment. She stopped playing tennis due to tendonitis in her elbow. She hasn&#8217;t been able to pick up her grandkids due to arthritis in her hands. Neuropathy in her feet has made going on walks increasingly difficult.</p><p>While she&#8217;s motivated and knows she needs to work on her strength, balance, and mobility, she&#8217;s struggled to find ways to do so without causing discomfort. Learning how to support her in her journey has illuminated both (1) how fragmented and unsupportive our health system is and (2) how common this cycle is.</p><p>Avoiding activity and learning to live with discomfort is often the worst thing you can do. It amplifies the risk of bigger complications and risks down the road. While early detection and support to navigate this cycle is foundational, our care ecosystem doesn&#8217;t make it easy.</p><p></p><h2><strong>The healthcare conundrum</strong></h2><p>The unfortunate reality is that a lot of low-grade pain goes untreated.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-6" href="#footnote-6" target="_self">6</a>  Because our health system has historically been structured around reactive care, people often don&#8217;t think of the healthcare system for issues they can live with, even when issues affect their activities and lifestyles.</p><p>Gone are the days when most Americans had an intimate, high-touch relationship with a primary care doctor that they knew and trusted. As the system has evolved to a more splintered network of specialized practices with a convoluted system of reimbursement codes, we&#8217;ve moved further and further from the centralized, whole-person care that has the greatest potential to support our aging population.</p><p>Physical therapy, often the best solution to address and remedy the cycle mentioned above, has it&#8217;s own set of systemic challenges. While it is designed to apply evidence-based methods to improve strength, mobility, and balance, it is woefully underutilized and hard to access.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-7" href="#footnote-7" target="_self">7</a>  When it is used, the approach to reimbursement often results in a limited number of visits that end up focusing on acute scenarios and injuries, missing the opportunity for holistic preventative care.</p><p></p><h2><strong>The false narrative of aging</strong></h2><p>As a society, we are prone to buying into a false and problematic narrative: that older adults are unavoidably weaker, less capable, and destined for an increasingly inactive life because &#8220;that&#8217;s just what happens when you get older.&#8221;</p><p>As Joseph Coughlin articulates in his book <a href="https://agelab.mit.edu/books/the-longevity-economy/">The Longevity Economy</a>: </p><blockquote><p>&#8220;We&#8217;re stuck with a notion of oldness that is so utterly at odds with reality that it has become dangerous. It constrains what we can do as we age, which is deeply troubling, considering that the future of our older world will naturally hinge on the actions of the older people in it.&#8221;</p></blockquote><p>We need to rebrand our cultural perception of aging from this monolithic and misinformed presumption into what it is: an opportunity for continued vitality and fulfillment.</p><p>The good news is that much of the physiological (and mental) decline can be slowed or even prevented. Consider some uplifting data&#8230;</p><ul><li><p>Science-based exercises for arthritis can reduce pain and increase mobility, function, and quality of life by <a href="https://www.cdc.gov/arthritis/prevention/">up to 40%</a>.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-8" href="#footnote-8" target="_self">8</a></p></li><li><p>All adults, even those with osteoporosis, can maintain (and even <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6323511/">improve</a>) bone density and muscle mass as they age.</p></li><li><p>Studies have shown that regular exercise can reduce the risk and rate of falls amongst older adults by 20-40%.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-9" href="#footnote-9" target="_self">9</a></p></li><li><p>It doesn&#8217;t take much; Studies show that as little as 11 minutes of activity a day can significantly reduce the risk of early death and prevent chronic diseases, stroke, and cancer.<a class="footnote-anchor" data-component-name="FootnoteAnchorToDOM" id="footnote-anchor-10" href="#footnote-10" target="_self">10</a></p></li><li><p>Finding movement that fits in your life has also been scientifically proven to improve <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7377236/#:~:text=Regular%20exercise%20is%20a%20formidable,bout%20and%20through%20chronic%20adaptations.">sleep</a>, <a href="https://www.health.harvard.edu/mind-and-mood/exercise-can-boost-your-memory-and-thinking-skills">brain health</a>, <a href="https://www.hopkinsmedicine.org/health/wellness-and-prevention/exercise-and-the-heart">cardiovascular health</a>, <a href="https://www.nia.nih.gov/news/maintaining-mobility-and-preventing-disability-are-key-living-independently-we-age">flexibility</a>, <a adults aspiring to age at home, there are daunting issues standing in the way of maintaining their independence and quality of life as they age&#8230;</p><ul><li><p><strong>High levels of inactivity</strong>. By age 65, <a href="https://www.cdc.gov/nchs/products/databriefs/db443.htm">only 15%</a> of adults meet the CDC&#8217;s physical activity guidelines.</p></li><li><p><strong>Inadequate levels of strength. </strong>Lack of strength and balance is the primary cause of falls amongst adults 65+, which affect one out of four Americans each year, is the leading cause of fatal and nonfatal injuries, and results in over <a href="https://www.cdc.gov/falls/data/index.html#:~:text=Each%20year%20about%20%2450%20billion,to%20non%2Dfatal%20fall%20injuries.">$50B</a> of medical costs per year.</p></li><li><p><strong>Skyrocketing obesity and chronic disease.</strong> Over <a href="https://hpi.georgetown.edu/obesity2/">60%</a> of Americans over 50 were overweight or obese in 2020 (a 32% increase in the past 30 years). Nearly <a href="https://www.ncoa.org/article/get-the-facts-on-healthy-aging">95%</a> of older adults have at least one chronic health condition.</p></li><li><p><strong>Increasing loneliness</strong>. More than one-third of adults aged 45 and older feel lonely, and nearly one-fourth of adults aged 65 and older are considered to be socially isolated, both of which are <a href="https://www.cdc.gov/aging/publications/features/lonely-older-adults.html">correlated</a> with poor diet, inactivity, and elevated risk of depression, anxiety, and heart disease.</p></li></ul><p>People&#8217;s inability to maintain their independence is going to have a massive ripple effect on our world, with major implications to our society (loneliness, increased family caregiving demands), economy (rising health costs, lost productivity), healthcare system (stress on our already <a href="https://www.cnbc.com/2021/11/08/aging-baby-boomers-raise-the-risk-of-a-long-term-care-crisis-in-the-us.html">overwhelmed care system</a>), and the lives of aging adults (loss of autonomy, ability to get dressed, and decreased quality of life from not being able to enjoy the activities they love).</p><div class="pullquote"><p><em>&#8220;The rapidly aging worldwide population will affect us more than global warming&#8230;it&#8217;s the seminal event of the 21st century.&#8221; - </em>Andrew Carle, founding director of the program in senior housing administration at George Mason University</p></div><h3><strong>Neglected by Innovation</strong></h3><p>As a technologist, investor, and designer, I&#8217;m amazed at how proportionally small the amount of investment and innovation that&#8217;s happening in this space (especially relative to sexier trends like AI, blockchain, and web3).&nbsp;</p><p>This population deserves products and services that are purpose-built for their life stage, tech savviness, and context of use. From web browsers to password managers to banking and healthcare services, I&#8217;m surprised how overlooked this audience is by technology.&nbsp;</p><p>In 2021, investment in companies focused on the aging population <a href="https://thegerontechnologist.com/who-is-investing-in-the-age-tech-revolution/">represented 0.1%</a> of total VC investment. Aside from the sheer size of the demographic, there are some pretty compelling reasons Silicon Valley should care more. Older adults&#8230;&nbsp;</p><ul><li><p>have the highest levels of disposable income (<a href="https://www2.deloitte.com/content/dam/insights/us/articles/us-generational-wealth-trends/DUP_1371_Future-wealth-in-America_MASTER.pdf">70% of US disposable income</a> is owned by those 55 and older)</p></li><li><p>are <a href="https://www.aarp.org/research/topics/technology/info-2019/2020-technology-trends-older-americans.html">increasingly comfortable with technology</a></p></li><li><p>have the <a href="https://www.bls.gov/opub/ted/2015/time-spent-in-leisure-activities-in-2014-by-gender-age-and-educational-attainment.htm">highest levels</a> of free time, and&nbsp;</p></li><li><p>have <a href="https://www.mckinsey.com/mhi/our-insights/age-is-just-a-number-how-older-adults-view-healthy-aging">increasing levels of interest</a> in investing in their a lot of time in the homes of people across the US who were struggling with obesity, interviewing them about their environment, health history, habits, and goals, trying to understand how to help them improve their health and behaviors. The learnings that stick with me from those interviews speak aptly to both the current gaps in our healthcare system and how we might &#8220;meet people where they are&#8221; to create a better support structure for healthy aging:</p><ol><li><p>Most people have good intentions and genuinely want to improve their health</p></li><li><p>But they&#8217;re also overwhelmed with the amount of information out there</p></li><li><p>And struggle to determine what&#8217;s credible and applicable to them</p></li><li><p>They often lack clear, pointed, expert guidance, available as life happens (which just isn&#8217;t what our health system is built to incentivize or reimburse)</p><p></p></li></ol><h3><strong>Reasons to be Optimistic&nbsp;</strong></h3><p>While many people assume that maintaining your health and strength as you age is a hopelessly uphill battle, there is no shortage of research highlighting that you can maintain (and even improve) your physical health as you age. Peter Attia&#8217;s <a href="https://peterattiamd.com/outlive/">recent book</a> is a great summation of scientifically backed approaches and habits to do just that. A <a href="https://www.nature.com/articles/s41591-022-02100-x">study</a> published in 2022 showed that as little as four 1-minute bursts of &#8220;lifestyle physical activity&#8221; can lead to a 30% reduction in cardiovascular risk <em>and </em>cancer. And there&#8217;s no shortage of inspiring stories in the news about people like <a href="https://www.washingtonpost.com/wellness/2024/01/16/fitness-aging-richard-morgan/">Richard Morgan</a>, who, at 93, has the physical fitness of a 40-year-old.</p><div class="pullquote"><p><em>&#8220;Exercise is by far the most potent longevity 'drug'. No other intervention does nearly as much to prolong our lifespan and preserve our cognitive and physical function. But most people don't do nearly enough.&#8221; - Peter Attia</em></p></div><p><strong>What&#8217;s missing</strong> is the adaptation of clinical best practices into terms, approaches, and accountability that speak to the unique nuances of each individual. This is an opportunity that technology, in particular AI and large language models, is perfectly suited to solve with truly personalized, proactive, and always accessible care and guidance that integrates comfortably with the flow of people's everyday lives.</p><p>This inflection point carries with it a host of exciting opportunities and questions that I&#8217;m actively exploring, including:</p><ul><li><p><strong>Access to trusted expertise</strong>: How can we provide every person with hyper-personalized, always-available access to clinical expertise throughout the flow of their daily lives (physical therapy, mental health, strength and conditioning, sleep coaching, nutrition)?</p></li><li><p><strong>Social connectivity</strong>: How can technology better understand the people in our lives and help us create connection, accountability, and motivation in our health behavior?</p></li><li><p><strong>Motivation: </strong>How can we build models that learn each person&#8217;s motivational levers and optimize the product experience to make the most effective use of them?</p></li><li><p><strong>Wearable data sensemaking</strong>: How can we help people better understand and contextualize the reams of data they&#8217;re collecting in a way that&#8217;s easy to understand and grounded in their unique goals and lifestyle?</p></li></ul><p>I&#8217;m excited and optimistic about the role technology can play in filling gaps in our current healthcare system.
