When I stepped into functional medicine as a nurse practitioner a decade ago, my heart was simple: help anyone who needed care, no matter their income. As a nurse I’d always believed healing should reach everyone. Contracting with Medicaid, Medicare, and private insurers felt like the obvious path—remove financial walls and serve the widest circle possible.

I quickly learned the system is a maze of smoke and mirrors.

Pennies on the Dollar

In a busy primary-care group I watched reimbursements shrink to fractions of what it actually cost to sit with a person, listen deeply, and address root causes. Tracking payments became an exhausting puzzle.

Worse, income increasingly depended on “compliance” with standards written by entities whose first loyalty is to shareholders, not the human in the exam room. I found myself documenting topics that had nothing to do with the patient in front of me just to protect the highest possible payment. That never sat right.

Fighting Goliath Alone

I moved to solo practice hoping lower overhead would let me keep fees reasonable. Instead I faced the same giant alone. Coding denials arrived constantly. I had to hire extra help simply to battle well-funded insurance billing departments—only to recover 10–40 % of what I’d billed. Overhead climbed while the actual care I wanted to give stayed under constant threat.

One moment still stings. I paid for my nurse to complete functional-medicine health-coaching training so we could walk beside people with metabolic struggles—teaching food, movement, sleep, and stress skills that create lasting change. Motivated patients showed up ready to work. Then the claims came back denied. Insurance would cover drugs and surgery for weight loss, we were told, but not coaching. Because patients believed their plans would pay, I absorbed the cost and delivered the service free. A solo practice cannot survive long on free labor.

The Volume Trap

Many clinicians respond the only way the math allows: see more people, shorter visits, higher throughput. Five-minute appointments become normal. Charts go unread. Patients are herded through so data can be collected and boxes checked. There is no built-in reward for discovering why symptoms cluster together or for guiding someone back to vitality. The quiet incentive is to keep the revolving door spinning—medications and procedures pad the system while true recovery remains optional.

Even the language of billing is locked down. The CPT codes we must use were created by an organization that collects royalties on them, influences how much we are paid for using them, and lobbies to keep the whole structure mandatory. Change is engineered to be nearly impossible.

Choosing a Different Path

I stepped outside that tidy bowl on purpose. By not contracting with insurance I reclaim the one thing that matters most: time and attention for the person sitting with me. We can explore upstream drivers—nutrition, lifestyle, environment, stress biology—without racing a clock or fighting a denial. Care becomes a partnership instead of a transaction.

This choice is not free of trade-offs. You invest your own resources. Yet for many who are tired of feeling processed, that investment buys something insurance rarely delivers: a practitioner who has actually studied your story, space to ask “why,” and a plan aimed at healing rather than endless management.

If you have ever left an appointment feeling unseen, you already understand the cost of the old model. Stepping outside it is how some of us protect the medicine we believe in—and how you can protect the version of health you deserve.
