Maybe even a little exaggerated.</p><p>But there is an interesting story behind it.</p><p>At IMAX Comprehensive Imaging Center in Mexico, Dr. David Fernando P&#233;rez Montemayor describes how AI is being used alongside radiologists in breast imaging. His description of AI is surprisingly simple. He calls it a <strong>&#8220;perfect coworker&#8221;</strong>, something that can support the radiologist and help identify findings that might otherwise be overlooked during a busy day.</p><p>And then he describes one particular patient.</p>

These devices cover areas such as radiology, cardiology, neurology, pathology, ophthalmology and other parts of healthcare.</p><p>And this is the part I find most interesting.</p><p>We often imagine healthcare AI as ChatGPT answering a doctor&#8217;s question or a futuristic robot operating on a patient.</p><p>The reality is much more practical.</p><p>AI is already being built into the tools clinicians use to look at images, monitor patients, detect abnormalities, support diagnosis and guide clinical workflows.</p>

headlines. Most of them describe tools that help doctors do something they were already doing, just a little faster or a little more accurately.</p><p>This is not that.</p><p>This study is about prediction from data that is already being collected. You do not need a new test. You do not need a new scan. The information needed to flag your melanoma risk years before any symptom appears is likely already in your health record right now. The AI just knows how to read it.</p><p>That distinction matters enormously. It means the barrier to deployment is not a new piece of equipment or a new clinical workflow. It is access to the model and the will to use it.</p><div><hr></div><h3>The window nobody talks about</h3><p>The difference between early stage and late stage melanoma is not just survival odds. It is the difference between a minor procedure and a fight for your life. That window can be as short as a few months.</p><p>An AI that flags risk years before the first visible symptom does not just improve outcomes at the margins. It fundamentally shifts which side of that window patients land on.</p><div><hr></div><h3>The question this raises</h3><p>If the data already exists and the model already works, why is this not standard practice everywhere?</p><p>The honest answer is that healthcare systems move slowly, deployment is uneven, and the patients who would benefit most from early detection tools are often the ones least likely to have access to them.</p><p>The science has moved. The systems have not caught up yet. They are deploying.</p><p>This is the clearest signal yet that AI in healthcare has crossed from early adoption into something closer to an institutional expectation. When governments are not piloting but deploying, the conversation shifts. It stops being about whether the technology is ready and starts being about governance, equity, safety and accountability.</p><div><hr></div><h3>Why a WHO report matters more than another startup announcement</h3><p>The health AI space is flooded with announcements. Every week brings a new funding round, a new benchmark, a new partnership between a technology company and a hospital system. Most of it is noise for the average clinician or patient trying to understand what is actually changing in their care.</p><p>A WHO report is different. It represents governments taking stock. It is about which systems, institutions and nations build the trust infrastructure that allows these tools to be used safely and at scale over the long term. The EU just published a blueprint for that effort. The US would do well to read it carefully.</p><div><hr></div><h3>What this means for clinicians and patients right now</h3><p>If you are a clinician practicing in the EU, AI tools are increasingly part of the institutional landscape around you whether you have chosen them or not. Understanding what they are, what they have been approved for and what oversight exists is no longer optional professional development. It is basic clinical literacy.</p><p>If you are a patient in any of these 27 countries, the tools assisting your care have reached a level of governmental recognition that did not exist even two years ago. Not dramatically, not all at once, but steadily and in ways that most patients are not aware of. The doctor reviewing your scan, the system flagging your test results, the software suggesting your treatment options, these may no longer be purely human judgments. AI is quietly becoming part of how medicine works, and patients deserve to understand what that means for them.</p><p style="text-align: justify;">This is not a reason to worry. But it is a reason to be informed.</p><div><hr></div><h2>Your data is the raw material</h2><p style="text-align: justify;">Every AI system in healthcare runs on data. Patient records, lab results, imaging scans, prescription histories, clinical notes. The more data a system has seen, the better it tends to perform. That data came from real patients, and some of it may have come from you.</p><p style="text-align: justify;">When you visit a hospital or clinic that uses AI tools, your health information may be used in ways that go beyond your direct care. It might feed into a system that helps the hospital predict patient demand. It might train a future version of a diagnostic algorithm. It might be shared with a third party technology company under a data agreement you never saw.</p><p style="text-align: justify;">This does not mean something sinister is happening. Most of the time it is not. But you have a right to ask. You have a right to know what data your healthcare provider collects, how it is stored, who it is shared with, and whether you can opt out of uses beyond your direct treatment.</p><p style="text-align: justify;">The question to ask your provider is simple: does your institution use AI tools in patient care, and how is my data handled in connection with those tools?</p><p patients</h2><p style="text-align: justify;">No AI system is perfectly accurate. Every diagnostic algorithm has a false positive rate and a false negative rate. That means it will sometimes flag a problem that is not there, and sometimes miss one that is. Understanding this is not the same as distrusting the technology. It is just being realistic about what it can and cannot do.</p><p style="text-align: justify;">More importantly, AI systems do not perform equally well across all patient groups. Models trained predominantly on data from certain populations can underperform when applied to others. There is documented evidence of algorithms performing less accurately for patients of certain ethnicities, body types, age groups, and geographic backgrounds. This is not a flaw in any one product. It is a systemic issue across the field that researchers and regulators are actively working to address.</p><p style="text-align: justify;">What this means for you as a patient is this: if an AI assisted recommendation feels wrong, if a risk score seems off, if a diagnosis does not match your experience of your own body, you are allowed to push back. You are allowed to ask whether the tool being used has been validated for patients like you. A good clinician will take that question seriously.</p><div><hr></div><h2>Explainability: you can ask why</h2><p style="text-align: justify;">One of the genuine challenges in modern AI is that many systems cannot fully explain their own reasoning. A radiologist can point to the exact feature on the scan that concerned them. Some AI tools will simply return a probability score with no clear account of how it got there.</p><p style="text-align: justify;">This is called the black box problem, and it is a real limitation. It does not mean the output is wrong. It does mean that the clinician using the tool, and you as the patient, may have less visibility into the reasoning behind a recommendation than you would with a traditional clinical decision.</p><p style="text-align: justify;">You have every right to ask your doctor: was AI involved in this recommendation? If so, what did it flag and why? A physician who has used the tool responsibly should be able to give you at least a partial answer. If the only answer is the algorithm said so, that is not sufficient and a good clinician will know it.</p><div><hr></div><h2>Consent and transparency</h2><p style="text-align: justify;">In many countries, including most of Europe and increasingly in North America, there are legal frameworks emerging around AI in healthcare. Patients have rights around automated decision making, rights to explanation, and in some cases rights to request that a significant decision not be made by an automated system alone.</p><p style="text-align: justify;">The honest reality is that most patients are not informed when AI is involved in their care. It happens in the background. The scan gets read, the alert fires, the risk score appears, and the clinical decision follows, often without any mention of what contributed to it.</p><p style="text-align: justify;">This is beginning to change. Some institutions are making proactive efforts to inform patients when AI tools are being used. Others are not. Until disclosure becomes standard practice, the most effective thing you can do is ask directly.</p><p style="text-align: justify;">You should not feel awkward about this. Patients routinely ask about the medications they are prescribed, the procedures being recommended, the qualifications of the specialist they are seeing. Asking about the technology involved in your care is the same instinct. It is reasonable. It is your right.</p><div><hr></div><h2>AI is not your doctor</h2><p style="text-align: justify;">Perhaps the most important thing to understand is this: no matter how sophisticated an AI system becomes, it is a tool in your care, not the decision maker. The clinician sitting across from you carries the responsibility. They are the ones who can weigh the algorithm&#8217;s output against your full history, your values, your circumstances, and your voice in the conversation.</p><p style="text-align: justify;">AI can make that clinician faster, better informed, and more consistent. It cannot replace the human judgment that makes medicine medicine. It cannot sit with you in the hard moments. It cannot understand what health means to you as an individual and not just as a data point.</p><p style="text-align: justify;">You remain the most important person in your own healthcare. That does not change because the tools have gotten smarter.</p><div><hr></div><h2>Three questions worth keeping in mind</h2><p style="text-align: justify;">The next time you are in a clinical setting, these three questions are worth having in your back pocket.</p><p style="text-align: justify;">Is AI being used in any part of my diagnosis or treatment recommendation today? How is my health data being stored and shared beyond my direct care? If I disagree with a recommendation, what is the process for getting a human review?</p><p style="text-align: justify;">You do not need to ask them every time. But knowing you can ask them changes your relationship to the system in a healthy way. It makes you an active participant in your care rather than a passive recipient of it.</p><p style="text-align: justify;">That has always been the goal of good medicine. AI does not change that. And the answer, when you think it through carefully, is more reassuring than most people expect.</p><div><hr></div><h2>The things AI is genuinely good at</h2><p>To understand the role of doctors going forward, you first have to be honest about what AI does well. It processes enormous volumes of data faster than any human. It does not get tired. It does not have an off day. It can hold thousands of clinical variables in consideration simultaneously and surface patterns that would take a physician years of practice to develop an instinct for.</p><p>In radiology, pathology, and dermatology, AI tools are already matching and in some cases exceeding average human performance on specific narrow tasks. In predictive analytics, AI is identifying patients at risk of deterioration hours before the clinical signs become obvious to anyone on the ward.</p><p>This is not a threat. It is a tool. A very powerful one. It cannot read the look on a family member&#8217;s face and understand that they need more time, not more information. It cannot weigh a clinical recommendation against everything it knows about this particular person, their values, their fears, their life circumstances, and make a judgment call that accounts for all of it.</p><p>Medicine is not just the application of knowledge to a biological problem. It is a human relationship built on trust, and trust requires a human on the other side of it.</p><p>The doctor&#8217;s role has always included things that have nothing to do with pattern recognition. Presence. Judgment. Moral responsibility. The ability to say, with genuine authority and genuine care, here is what I think we should do and here is why. AI can inform that conversation. It cannot have it.</p><div><hr></div><h2>The doctor as interpreter and guide</h2><p>One of the most important roles doctors will play in the coming decade is one that barely existed before: helping patients navigate AI-generated information.</p><p>Patients are already arriving at consultations with printouts from AI symptom checkers, AI-generated second opinions, and AI-summarized research papers. Some of it is accurate. Some of it is confidently wrong. Most of it lacks the context that makes it useful.</p><p>The doctor becomes the interpreter. The person who can take an AI output, understand where it came from, recognize its limitations, apply it to this specific patient in this specific moment, and translate it into something actionable and human.</p><p>That is not a diminished role. That is a more sophisticated one.</p><div><hr></div><h2>The doctor as the last line of ethical accountability</h2><p>AI systems optimize for what they are trained to optimize for. They do not carry moral weight. They do not bear responsibility when something goes wrong. They cannot be held accountable in any meaningful sense.</p><p>The doctor can. And must be.</p><p>As AI becomes more embedded in clinical workflows, the physician&#8217;s role as the final checkpoint, the person who reviews the recommendation and decides whether to act on it, becomes more important rather than less. Every AI alert that gets followed blindly without clinical judgment is a failure of that role. Every physician who understands the tool well enough to know when to override it is doing exactly what medicine demands.</p><p>Accountability is not a burden that AI can take on. It stays with the clinician. That is not a limitation of AI. It is a feature of medicine.</p><div><hr></div><h2>Doctors who will thrive</h2><p>The doctors who will feel most at home in the next era of medicine are not necessarily the ones who know the most about technology. They are the ones who are clear about what they bring that technology cannot.</p><p>Deep human judgment. The ability to hold uncertainty without collapsing into it. Communication skills that build genuine trust. Clinical intuition built from thousands of patient encounters that no dataset fully captures. The courage to make a call when the data is ambiguous and the patient needs an answer.</p><p>These are not soft skills sitting on the margins of medicine. They are the core of it. They have always been the core of it. AI just makes them more visible by taking on the parts that do not require them.</p><div><hr></div><h2>A new kind of partnership</h2><p>The most useful frame is not doctor versus AI. It is doctor with AI, each doing what it does best.</p><p>AI handles the volume, the pattern recognition, the data processing, the consistency across thousands of cases. The doctor handles the relationship, the judgment, the context, the accountability, and the irreducibly human elements of care that no algorithm will replicate.</p><p>That partnership, when it works well, produces something neither could achieve alone. Faster, more accurate diagnosis. Better informed clinical decisions. More time for the doctor to do the things that actually require a doctor.</p><p>The age of AI does not make medicine less human. You do not need to write a single line of code. But there are a few things that every practicing clinician needs to understand right now.</p><div><hr></div><h2>Understand what AI can and cannot do</h2><p style="text-align: justify;">The first thing to get clear on is this: AI in its current form is very good at pattern recognition across large amounts of data. Your electronic health record may already be surfacing AI-generated alerts, risk scores, or clinical suggestions. Most doctors either dismiss these automatically or follow them without much thought. Neither is the right approach.</p><p style="text-align: justify;">The skill you need to build is informed skepticism. When an AI tool flags something, ask: what data was this trained on? Does that population look like my patient? What is the false positive rate? Who validated this tool and in what setting?</p><p style="text-align: justify;">You would apply the same scrutiny to a new drug before prescribing it. Apply it here too. Not every AI alert is worth acting on. But understanding why it fired in the first place makes you a better clinician.</p><div><hr></div><h2>Get comfortable with the vocabulary</h2><p style="text-align: justify;">You do not need to go deep into the technical side, but being able to hold a basic conversation about AI in medicine is increasingly part of professional literacy. Terms like machine learning, large language models, training data, algorithm bias, and clinical validation are showing up in journals, hospital meetings, and conversations with colleagues.</p><p style="text-align: justify;">Knowing what these mean at a functional level means you can evaluate new tools, ask the right questions when your hospital introduces an AI system, and engage with the research literature that is coming out at a rapid pace. Start with one good explainer article a week. Within a few months, the language will feel natural.</p><div><hr></div><h2>Pay attention to bias and know it affects your patients</h2><p style="text-align: justify;">This one is not optional. AI systems learn from historical data and historical data in medicine has significant gaps. Models trained predominantly on data from certain populations can underperform when applied to others. This has already been documented in dermatology, cardiology, and pain management.</p><p style="text-align: justify;">As the clinician, you are the last line of defense. If an AI tool is giving you a risk score or a recommendation, it is your responsibility to consider whether that tool has been validated for your patient population. This is not about being cynical about the technology. It is about practicing good medicine with the tools available to you.</p><div><hr></div><h2>Start somewhere, even if it is small</h2><p style="text-align: justify;">The doctors who will feel most confident in the next decade of medicine are not necessarily the ones with the most technical knowledge. They are the ones who started engaging early, even in small ways.</p><p style="text-align: justify;">Read one paper a month on clinical AI. Attend one webinar from your specialty society on digital health. When a new AI feature appears in your EHR, do not click past it. Spend ten minutes understanding what it is doing and why.</p><p style="text-align: justify;">Curiosity is the skill here more than anything else. The doctors who stay curious about what these tools can and cannot do will shape how AI gets used in clinical practice. The ones who disengage will simply inherit whatever gets decided for them.</p><div><hr></div><h2>The bottom line</h2><p style="text-align: justify;">AI is not going to replace you. But the pace of change in medicine right now is real, and staying informed is part of your professional responsibility just as much as staying current on clinical guidelines.</p><p style="text-align: justify;">You spent years learning to read the body. Now is the time to learn, at least in broad strokes, how to read the technology that is becoming part of your daily practice. Not because it is exciting, though it often is. Clinical decision support</strong></p><p>Every physician carries a cognitive load that would crush a normal computer. Drug interactions. Contraindications. Rare diagnoses hiding behind common symptoms. Drug discovery</strong></p><p>This one is slower and less visible to patients &#8212; but potentially the biggest deal of all. AI is compressing the drug discovery timeline in ways that were unimaginable five years ago. AlphaFold&#8217;s protein structure predictions opened a door that had been shut for decades. Now biotech companies are using AI to identify drug candidates, predict toxicity, and design clinical trials with a speed and precision that human researchers simply can&#8217;t match alone.</p><div><hr></div><h2>The honest complications</h2><p>No good story ignores the hard parts, so here are the honest complications.</p><p><strong>Bias baked in.</strong> AI systems learn from historical data &#8212; and historical medical data is riddled with disparities. It&#8217;s here.
